Finding a clinical attachment in the UK

If you are an international medical graduate (IMG) considering practising medicine in the UK, you may have investigated possible ways to gain clinical work experience. A great way to familiarise yourself with a UK doctor’s work and the National Health Service (NHS), which will be your employer, is to find clinical attachments. 

Moreover, completing a clinical attachment has been shown to be particularly beneficial before or shortly after passing part two of your PLAB (Professional and Linguistic Assessment Board) exam. As such, this article aims to provide you with insight into possible pathways to clinical attachments as an IMG.

What is a clinical attachment?

A clinical attachment is an opportunity to gain work experience in medicine, as well as an overview of the medical processes and systems in the UK. This takes place in the form of clinical placements which typically last two to four months and cost from £400. 

Your duties and responsibilities are limited; you will mostly be observing a consultant in a relevant specialty. After a certain period of clinical observation, you may undergo risk assessment which will allow you to begin taking on some clinical responsibilities.

Your duties will be restricted to:

Observing consultations

Participating in patient administration

Taking patient histories

Physical examination

Observing surgery

Eligibility

First and foremost, you must have already graduated from medical school, and hence possess an MBChB degree or equivalent; if you are still a medical student, you cannot apply. However, this does not mean you must wait until graduation to gain valuable clinical experience in the UK if you are contemplating working there in the future. 

You may wish to undertake a medical elective through a UK university as part of your medical degree programme. Occupational health clearance from the NHS trust

There are no specific nationwide application platforms for securing a clinical attachment. Instead, some individual NHS trusts or hospitals run their own clinical attachment programme. This means you can simply go onto their webpages and follow their application process. These programmes match you to a consultant working within your chosen specialty.

Furthermore, you may visit hospital websites and navigate to a list of consultants working within your specialty of interest. There, you should also find their contact details and you can email them directly to ask for potential hospital placements or work. 

If you are unsure about individual consultants, you could also email the department administration team to ask about options and availability. Do not be discouraged if a consultant or department rejects your request, or does not reply at all, as they will have to dedicate a good amount of time and effort to your supervision, which can be rather difficult to manage for them. Keep on emailing consultants; after all, every rejection only brings you closer to a positive response.

Alternatively, you could try asking friends or family who already work in an NHS hospital, or know someone who does. Having contacts can prove advantageous in general, and therefore, you should try to take on every networking opportunity you come across, such as conferences and other international events.

Regardless of which route you take to find clinical work experience in the UK, there are specific details you should include in your email to consultants or departments when requesting a clinical attachment.

Cardiff and Vale University Health Board are pleased to offer clinical attachment placements . A clinical attachment is a short term unpaid placement for a qualified international doctor who needs experience of the NHS. This is an observer role only and the doctor doing a clinical attachment will not have direct patient contact or take on any responsibility .

Clinical attachments are available for the following:

  • Doctors who have qualified abroad and would like to gain some experience of UK culture and the NHS
  • Doctors returning to practice following a break
  • Doctors requesting experience of a particular specialty to aid career choice

All Clinical Attachment doctors will still need to undertake the same checks as any other member of staff working for the NHS, including Occupational Health clearance and the Disclosure Barring Service (DBS) clearance or International Police Checks.

The Application Process

If you wish to apply for a clinical attachment at Cardiff & Vale UHB you will FIRST NEED TO FIND A SUPERVISING CONSULTANT who will be willing to supervise and support you whilst on your attachment.

The Medical Resourcing & Systems team will not be able to assist with this process in finding a supervising consultant .

If there are no individual consultant email addresses located on our webpages, there may be a department email address or phone number you can call to make an inquiry. Ask about who would be best to contact in the department about arranging an attachment.

You can  also try contacting the Department Directorate Office, Medical Secretary or Rota co-ordinator for the specialty you are interested in

Take note, you may need to email dozens of Consultants to get a response but the more you email, the more likely it is you’ll be successful.

Do not be discouraged if a consultant or department rejects your request, or does not reply at all, as they will have to dedicate a good amount of time and effort to your supervision, which can be rather difficult to manage for them and the UHB receive a lot of Clinical Attachment applications on a daily basis. Keep on emailing consultants; after all, every rejection only brings you closer to a positive response.

Alternatively, you could try asking friends or family who already work in an NHS hospital, or know someone who does. Having contacts can prove advantageous in general, and therefore, you should try to take on every networking opportunity you come across, such as conferences and other international events.

Once your request is agreed with a Supervising Consultant you will need to follow the digital application process below but  If your supervising consultant has not agreed or you do not include their details, your application  will be  automatically rejected by our systems.

PLACES ARE LIMITED PER MONTH AND IF THE MONTHLY QUOTA IS REACHED THE APPLICATION WILL AUTOMATICALLY BE REJECTED EVEN IF AGREED WITH A CONSULTANT.

If this happens you will need to speak to your supervising consultant for a later starting date.

East Sussex Healthcare NHS Trust (ESHT) are pleased to offer clinical attachment placements at Conquest Hospital and Eastbourne District General Hospital. A clinical attachment is a short term unpaid placement for a qualified doctor who needs experience of the NHS. This is an observer role only and the doctor doing a clinical attachment will not have direct patient contact or take on any responsibility. Clinical attachments are available for the following:

  • Doctors who have qualified abroad and would like to gain some experience of UK culture and the NHS
  • Doctors returning to practice following a break (eg maternity leave or ill health)
  • Doctors requesting experience of a particular specialty to aid career choice

ESHT gives priority to high calibre doctors who have an intention to apply for a vacant position in the Trust. However, please note that doing a clinical attachment does not automatically secure a position at the Trust.

In accordance with Home Office recommendations, attachments are limited to a maximum of 4 weeks.  Only one attachment per person is permitted.

Please Note:  

If you wish to apply for a clinical attachment at ESHT  you will first need to find a Consultant Supervisor  who will be willing to supervise and support you whilst at ESHT. Following this either you or your Clinical Supervisor may apply for a clinical attachment. If this information is not included, your application  will be  rejected.

Due to the large volume of requests we receive for clinical attachments, we cannot guarantee a place for you as places are limited. You may have to be put on a waiting list even if you have found a consultant supervisor and provided the required documents. 

If you do not hear from us within 4 weeks, please assume your application has not been successful.

Occupational Health

If you have a medical condition for which adjustments are required to enable you to carry out your placement, you must inform the Medical Education Department in advance of your placement.

Information will be given to you on starting your placement as to actions to take in the event of you sustaining a needle-stick injury or body fluid exposure.

Please contact the Occupational Health and Wellbeing Department if you develop any of / or a combination of the symptoms listed below:

  • Unexplained weight loss
  • Unexplained fever, particularly at night
  • Unexplained breathlessness or persistent chest pain
  • Unexplained cough lasting longer than three weeks, especially if you are producing sputum

If your Clinical Attachment Application is successful, an administration fee of £50 will be payable before your arrival.  Payment details will be sent with your confirmation letter.

Accommodation (once you have an Attachment agreed)

If your clinical attachment is agreed, we will provide you with contact details for our accommodation officers at Eastbourne DGH and Conquest Hospital, Hastings who will be able to advise you of availability of hospital accommodation and fees.

ASPH Clinical Attachment Programme

Clinicians

A Clinical Attachment at Ashford and St Peter’s Hospital’s NHS Foundation Trust is an unpaid six-week placement to enable doctors to get experience in the UK.

The six week programme consists of a settling in and Induction week, followed by five weeks rotation through different specialties. These specialties will be varied, but we will try to take into account the interests of candidates as much as possible.

The six weeks period will include multiple different Education Opportunities, including, but not limited to:

  • Corporate Induction
  • IT Training
  • Resus Training
  • Grand Rounds
  • Teaching sessions in various specialties

During the programme, candidates will be fully supported by an Educational Supervisor for the entire six weeks, a Clinical Supervisor for each specialty and a team of others that will be able to provide pastoral and educational support.

During the last week of an attachment programme, we will invite candidates to take part in an OSCE style assessment, after which, if deemed suitable, the candidate will be offered a two year fixed term contract at Ashford and St Peters. During this employment, we will aim to give you a rotational programme containing at least one of your preferred specialties, access to the Foundation Programme e-Portfolio system and an Educational/Clinical Supervisor, which will set the doctor up with the same skills and experiences as a doctor finishing the Foundation Programme. If after an offer of employment is made, the doctor decides not to work with us, they would be liable to pay fees associate with the attachment, including any accommodation costs incurred by the Trust. Provided offers of employment are accepted, there are no fees to pay.

Our programme has a good success rate for taking PLAB2 after the programme has ended, and provides excellent insight into the NHS and the work we do.

Candidates must have in order to undertake a Clinical Attachment with us:

  • Expected capability to gain full GMC Registration soon after attachment. This includes:  
  • One year of internship
  • Completion of PLAB1 exam
  • Suitable IELTS or OET result
  • Other route to join the GMC Register, including holding suitable postgraduate qualifications  
  • Allowance for Visa or other right to be in the UK.

All Clinical Attachment doctors will still need to undertake the same checks as any other member of staff working the Trust, including Occupational Health clearance and the Disclosure Barring Service (DBS) clearance.

The programme is run three times a year, February, May and October (although this may be subject to change depending on prevailing circumstances), and this will be advertised on NHS Jobs approximately two months before the attachment Start Date.

The Honorary contract is subject to the following provisions:

  • A Clinical Attachment, must not in any circumstances  
  • Initiate, alter or stop the treatment of a patient on your own diagnosis
  • Order diagnostic investigations, request radiological examinations or order blood to be crossed-matched
  • Take part in obtaining or witnessing the signature by or on behalf of a patient on a form of consent to treatment  
  • A Clinical Attachment must not take the history from, examine or undertake a procedure on a patient unless his/her prior informed consent had been obtained and must be under the supervision of a supervising consultant.  
  • All attendance in hospital clinical areas must be supervised. Doctors on clinical attachment are not normally permitted to visit intensive therapy units or visit areas other than their base department without the express permission of the head of department or supervising consultant.  
  • The Clinical Attachment Doctor is required at all times to behave in accordance with departmental policies and protocols and the policies and procedures established by the Trust.  
  • The Clinical Attachment may be withdrawn at any time if they are found to be in breach of any Trust policies or procedures or the terms of this agreement. It may also be withdrawn for any other reason at the discretion of the Medical Director. The Doctor will normally be given a reason for withdrawal of the attachment.  
  • The honorary contract allows The Attachment Doctor to access Education, Library and other research facilities operated by the Trust and the provision of hospital records. Use of library facilities is subject to departmental guidelines.

Sample industrial attachment letter and how to write an industrial attachment application letter

Industrial attachment also known by many as internship normally lasts for three months but the duration differs from institution to institution. Some companies pay for industrial attachment while others do not pay (in an article we had written earlier, we provided a list of companies that pay for industrial attachment).  To increase your chances of getting an industrial attachment you have to write an appealing attachment letter.

When writing an industrial attachment application letter or internship application letter be precise and ensure that there are no grammatical or spelling errors in your letter.  Company directors have no time to read very long letters.  Also, ensure that you have attached your CV to accompany your industrial attachment application letter.  Do not be worried about the content in your CV, the company directors or human resource department understand that you are a student so they don’t expect much in your CV.

How to write application letter: Formal format

  •  At the top of the application letter should be your address then followed by the address of the company or receiver. It is not advisable to write your name at the top of a letter.   (Check for the sample application letter below)
  • Thereafter the salutation .  Use  Sir or Madam if you do not know the name of the person you are writing to.
  • Reference (This is the title of the application letter) example Application for internship in the engineering department
  • Opening paragraph : This paragraph should state the position you are applying to or for and how you knew of the position. It should also contain a basic introduction of who you are.
  • Body paragraph : Explain why you are interested in the position you are applying for, demonstrate your understanding of the position, talk about your qualification and skills in brief as they will be contained in your CV.
  • Conclusion Paragraph : State that you are ready to take the opportunity fairly. Thank the employer for her/his consideration in advance.
  • Closing remarks or complimentary remarks: End your letter “Yours Sincerely”  if you have begun it with a person’s title and family name. If you have begun “Dear Sir or Madam”, then end your letter “Yours faithfully”

About the Program

The Clinical Attachment is a clinical training program offered by the Postgraduate Medical Education Deanship at Mohammed Bin Rashid University of Medicine and Health Sciences(MBRU) at Dubai Health. Healthcare providers and students interested in professional development in the administrative, medical, dental, and allied health professions will benefit by acquiring high-quality education and training opportunities that will serve the needs of patients through the advancement of healthcare delivery. The training consists of observership or hands-on experiences, with opportunities for clinical rotations at Dubai Health facilities supported by evaluation from expert supervisors. Clinical Attaché’s will be equipped with the knowledge and fulfilling their clinical experience gap. After the successful completion of the program, the Attaché will receive a Certificate of Completion of Training from Dubai Health.

Eligibility: UAE nationals, residents and visitors who are: individual healthcare providers, doctors with a gap of practice, and university students.

Duration: The minimum period of attachment requested is 2 weeks and the maximum is 36 weeks in one year. Placement is subject to the availability of training spots.

Thank you for considering the MBRU-Dubai HealthClinical Attachment program. Being the Director of Clinical Attachment, it gives me immense pleasure to welcome you to the Clinical Attachment program which is offered by Postgraduate Medical Education.

The Clinical Attachment provides an opportunity for all healthcare and non-healthcare professionals interested, to explore and identify healthcare systems in the Emirate of Dubai. Join us for Clinical Attachment to observe or practice clinical hands-on training in DAHC facilities in line with international standards of patient safety, security, and confidentiality.

  • All clinical attachment requests are subject to pre-placement review and the applicant will not be able to commence until the review has been completed.
  • All Attaché must submit the Official Health Clearance Certificate at or before the time of commencement. You will not be permitted to be in contact with patients without such evidence.

Observership Attachment

Trainee spends a period of time in a monitored observed environment to gain knowledge through observation of clinical care provided at DAHC facilities.

Applicants are required to provide the following:

  • Evidence of immunity to: hepatitis B, measles and rubella, and Covid19
  • Evidence of negative status with respect to: HIV, hepatitis B, hepatitis C, tuberculosis, and syphilis (VDRL)
  • Attained within the last 2 years, an ILETS Academic overall score 6 or TOEFL iBT minimum of 80

Hands-on Attachment

Trainee spends a period of time in a monitored, supervised environment where he/she is able to carry out clinical tasks as assigned by a supervisor at DAHC Facilities.

  • Evidence of negative status with respect to: HIV, hepatitis B, hepatitis C, tuberculosis, syphilis (VDRL)
  • Valid License
  • Malpractice insurance
  • Recommendation letter
  • Attained within the last 2 years, an ILETS Academic overall score 6 or TOEFL iBT minimum of 8

  •  Visa copy: illustrates that the applicant has permission to stay in the UAE for the duration of the clinical attachment.
  • If you are a student, provide an updated letter “To Whom It May Concern” from your University.
  • If you are an employee, provide an updated letter “To Whom It May Concern” from your Employer.
  • Otherwise, provide an updated “Police Clearance Certificate” from the relevant Ministry of Interior Agency /Police Department/Equivalent.

Clinicians

A Clinical Attachment at Ashford and St Peter’s Hospital’s NHS Foundation Trust is an unpaid six-week placement to enable doctors to get experience in the UK.

The six week programme consists of a settling in and Induction week, followed by five weeks rotation through different specialties. These specialties will be varied, but we will try to take into account the interests of candidates as much as possible.

The six weeks period will include multiple different Education Opportunities, including, but not limited to:

  • Corporate Induction
  • IT Training
  • Resus Training
  • Grand Rounds
  • Teaching sessions in various specialties

During the programme, candidates will be fully supported by an Educational Supervisor for the entire six weeks, a Clinical Supervisor for each specialty and a team of others that will be able to provide pastoral and educational support.

During the last week of an attachment programme, we will invite candidates to take part in an OSCE style assessment, after which, if deemed suitable, the candidate will be offered a two year fixed term contract at Ashford and St Peters. During this employment, we will aim to give you a rotational programme containing at least one of your preferred specialties, access to the Foundation Programme e-Portfolio system and an Educational/Clinical Supervisor, which will set the doctor up with the same skills and experiences as a doctor finishing the Foundation Programme. If after an offer of employment is made, the doctor decides not to work with us, they would be liable to pay fees associate with the attachment, including any accommodation costs incurred by the Trust. Provided offers of employment are accepted, there are no fees to pay.

Our programme has a good success rate for taking PLAB2 after the programme has ended, and provides excellent insight into the NHS and the work we do.

Candidates must have in order to undertake a Clinical Attachment with us:

  • Expected capability to gain full GMC Registration soon after attachment. This includes:  
  • One year of internship
  • Completion of PLAB1 exam
  • Suitable IELTS or OET result
  • Other route to join the GMC Register, including holding suitable postgraduate qualifications  
  • Allowance for Visa or other right to be in the UK.

All Clinical Attachment doctors will still need to undertake the same checks as any other member of staff working the Trust, including Occupational Health clearance and the Disclosure Barring Service (DBS) clearance.

The programme is run three times a year, February, May and October (although this may be subject to change depending on prevailing circumstances), and this will be advertised on NHS Jobs approximately two months before the attachment Start Date.

The Honorary contract is subject to the following provisions:

  • A Clinical Attachment, must not in any circumstances  
  • Initiate, alter or stop the treatment of a patient on your own diagnosis
  • Order diagnostic investigations, request radiological examinations or order blood to be crossed-matched
  • Take part in obtaining or witnessing the signature by or on behalf of a patient on a form of consent to treatment  
  • A Clinical Attachment must not take the history from, examine or undertake a procedure on a patient unless his/her prior informed consent had been obtained and must be under the supervision of a supervising consultant.  
  • All attendance in hospital clinical areas must be supervised. Doctors on clinical attachment are not normally permitted to visit intensive therapy units or visit areas other than their base department without the express permission of the head of department or supervising consultant.  
  • The Clinical Attachment Doctor is required at all times to behave in accordance with departmental policies and protocols and the policies and procedures established by the Trust.  
  • The Clinical Attachment may be withdrawn at any time if they are found to be in breach of any Trust policies or procedures or the terms of this agreement. It may also be withdrawn for any other reason at the discretion of the Medical Director. The Doctor will normally be given a reason for withdrawal of the attachment.  
  • The honorary contract allows The Attachment Doctor to access Education, Library and other research facilities operated by the Trust and the provision of hospital records. Use of library facilities is subject to departmental guidelines.
