Once a quarter, Bich Le, 52, travels from her home outside of Minneapolis to St. Augustine, Fla., where she moves into her father’s guest room for three weeks.

The health care executive is one of five siblings who take turns caring for their widowed 90-year-old father, who has lung cancer and requires constant assistance. While she’s in Florida this month, she will miss her daughter’s final high school prom; she missed it last year, too, due to her caregiving duties .

The drugs Ms. Le’s father takes to manage pain can “negatively impact how he treats people,” she said. When he becomes volatile, Ms. Le said, she mostly tries to ignore it and “not add to the stress of the situation.” She tells herself to “just care for him and just let it go.” But sometimes, when she’s exhausted, his temper grates.

“What runs through my brain is: ‘A simple thank you would really go a long way,’” she said. “‘You have me, or you have a nursing home.’”

Caregiving can be fraught for the estimated 53 million Americans who assist family members and friends. And factors like financial strain and isolation can add to psychological distress. In a 2017 survey of 1,081 caregivers conducted by AARP , 51 percent of respondents reported feeling worried or stressed. But there was a surprising upside: The majority — 91 percent — also reported feeling pleased that they were able to help.

How can caregivers hold on to that feeling amid the stress, fatigue and resentment that also come with the role? There are strategies for feeling “less burdened or stressed by the daily problems” they encounter, said William Haley, a professor of aging studies at the University of South Florida.

Ms. Le, for example, makes time every day to laugh with her siblings about the more absurd moments of caring for her father. “You kind of have to take it with some humor,” she said, “because otherwise I think you’d just be in the fetal position.”

Bone-tired caregivers often field unrealistic suggestions to do something indulgent for themselves: Book a spa treatment, take a yoga class or a weekend trip.

“Somebody saying, ‘You need to take care of yourself’ suggests that person doesn’t actually know what it’s like to be a family caregiver,” said Anne Tumlinson, a health care consultant from Washington, D.C. who founded The Daughterhood, a free online community for caregivers. “As a practical matter, there’s only so much you can do.”

Rev. Nicholas Sollom, a chaplain at Yale New Haven Hospital, isn’t a fan of any relaxation rituals that create new tasks. Instead, he advises caregivers to keep their routines as simple and sustainable as possible: “Make sure you got enough sleep, make sure you’re eating, make sure you’re staying hydrated,” he said. It sounds simple, but these are the things that often get overlooked when you’re focused on someone else’s survival.

“It’s all super basic to just being alive,” he added, but these few things “can be a game changer” for maintaining energy.

Seek support and services.

Without robust government support and resources, most U.S. caregivers have to figure things out on their own, Ms. Tumlinson said. And on top of overseeing another person’s health, housing and finances, she said, many caregivers have to navigate complex family dynamics when their siblings, spouses or parents become dependent on them.

“There comes a point where every caregiver goes, ‘Oh, I’m on another planet now,’” she said.

Yet the more you can educate yourself about your loved one’s illness — and find practical and emotional support — the less likely you are to feel overburdened, Dr. Haley said. He recommended contacting your local area agency on aging , a state-designated nonprofit that provides free directories for services that address the needs of older people. Through these organizations, you can explore resources like adult day care services and Meals on Wheels.

Geriatric care navigators with experience helping families find services and make caregiving plans can also help prepare you for challenges you might encounter, like getting expenses reimbursed by Medicare. They can be hired short-term for planning, or on an ongoing basis to oversee care. Care navigators can be costly, but if the person needing assistance has long-term care insurance, the expense may be partly covered.

Working with a therapist can help you develop coping skills , and support groups run by many disease organizations, including the Alzheimer’s Association , are also a good place to find information, coping strategies and social connection at no cost.

Many caregivers create their own support networks, said Mr. Sollom, adding that it’s ideal to find a crew of low-drama, reliable people “who you know you can call at four in the morning.”

Typically, Ms. Le said she experiences an 80/20 ratio of positive to negative caregiving emotions. But when she feels herself “edging” toward her 20 percent zone, she calls close friends to get perspective that pulls her back. Leaning on these people is “really a survival tactic,” she said.

Remember your “other” life.

In the thick of caregiving, it’s important to connect with aspects of your life that matter to you, said Allison Applebaum, director of the Caregivers Clinic at Memorial Sloan Kettering Cancer Center in New York and the author of “Stand by Me: A Guide to Navigating Modern, Meaningful Caregiving.”

Her father, who had Lewy body dementia, was often hospitalized during the decade she cared for him. But as a lifelong ballet dancer, Dr. Applebaum realized she could use the bars on his hospital bed to stretch and practice positions, an epiphany that brought her peace during those hospital stays, she said.

If you’re feeling disconnected from yourself, try taking stock of your interests, and notice what you’re missing — maybe it’s going to church, golfing, or spending time in nature, said Daniel Goldman, a psychotherapist in Madison, Wis.

Scott Klace, 63, has been a primary caregiver for his mother-in-law, who has dementia, since she moved in with him and his wife last year. His mother-in-law recently started attending an adult day care center twice a week. When asked what he’ll do with the free time, Mr. Klace initially seemed perplexed: “Maybe take the dog to the park? I don’t even know what I’ll do,” he said.

But the more he thought about it, the more excited he became. “It might be something as simple as going to the movies. I used to go to the movies all the time. I bet I’ve seen two movies in the last one and a half years,” he said.

If you can, put the time on the calendar; actually scheduling a walk or time with friends “puts you on the hook,” making it less likely your plans will evaporate amid daily chores, Mr. Goldman said.

If the person you care for can’t be alone, arrange for someone else to be with them while you’re away. Ideally, you might find a family member, friend or neighbor who can step in at a set time each week, he said.

Caregivers can also use technology for support, even if they can’t completely step away. Out-of-town relatives and friends can connect with the person needing care via FaceTime or Zoom to give the local caregiver a moment to rest in another room.

Acknowledge the contradictions of caregiving.

On a genuinely crummy day, advice to “look on the bright side” might make you want to scream. But tallying up positives can help: A small study from 2017 found that dementia caregivers who were prompted to consider what they gained from their experience described things like finding purpose and new feelings of mastery. They also experienced fewer symptoms of depression than participants who received only standard education about caregiving skills and dementia symptoms.

Mr. Klace, who recently spent several frustrating months trying to re-enroll his mother-in-law for Medicare Part B, admitted that caregiving “sucks sometimes, it really does.” But helping in simple ways — paying her bills; making sure she’s got enough to eat — makes him “feel a little better about myself,” he said.

For exhausted caregivers who may not know what they feel, Mr. Sollom recommends a brief daily practice: Think of three things that were challenging during the day. Maybe a loved one was denied for a clinical trial, a doctor ignored your concerns and you got stuck on the phone with an unhelpful bank representative. Then, come up with three things that you’re grateful for: A nurse brought you a cup of tea, you and your loved one took a short walk and a friend you’ve been missing called to check in.

Taking a wider view helps caregivers become “aware that they’re actually putting their energy into things that matter,” Mr. Sollom said.

Frequently, without any prompting, the caregivers he counsels start by listing their burdens and end up expressing gratitude or a sense of accomplishment. “I often wonder if they are consciously doing this,” he said. “Or if it’s more of an unconscious resiliency process.”

Family caregivers are struggling at work, need support from employers to stay, AARP finds

Family caregivers are struggling to work while juggling caregiving responsibilities, which could threaten a large part of the workforce unless employers are more supportive, a new study reports. 

AARP and S&P Global surveyed 1,200 self-identified caregivers who work full time or part time at large U.S. companies and provide at least six hours of care each week to an adult in order “to understand caregivers’ experiences in the workplace and the significance of employer benefits for balancing work and family care obligations,” the organizations said in a press release. 

An analysis of a new study released Thursday by AARP and S&P Global “underscores the importance for employers to implement policies and benefits that are friendly and supportive of adult caregiving to retain this critical segment of employees in the workforce,” the two organizations said.

The need is critical as the nation's population of adults ages 65 and older is projected to surpass the population of children by 2030, the organizations said.

What is a family caregiver?

AARP research has found that of the more than 48 million family caregivers nationwide, 61% are working while juggling caregiving responsibilities, including assisting with daily living activities, medical or nursing tasks, coordinating services, transportation, shopping and advocating for their loved ones. Most family caregivers provide at least 20 hours of care each week, equal to an unpaid part-time job, according to AARP. These caregivers are providing more than $600 billion in unpaid care for their loved ones, AARP has said.

“As Americans grow older, more families have more members with caregiving needs – like young children and their parents – while also holding down a job or career," Susan Reinhard, AARP senior vice president and director of the Public Policy Institute said in a statement about the report, which was provided exclusively to USA TODAY ahead of a media briefing Thursday.

"The majority of family caregivers juggle their responsibilities for loved ones every hour of every day," Reinhard said. "As a result, working family caregivers are overwhelmed by the sheer volume of daily tasks. At AARP, we’re engaging the private sector to be sure companies big and small are thinking through possible solutions to better support their employees who are family caregivers."

Family caregivers "need and deserve greater support from their own employers,” Reinhard also said in a press release. “As our population ages, it’s critical that employers support family caregivers in the workforce with the policies, such as paid leave, that can ease their everyday burdens.” 

What did the study find?

An analysis of the study, "Working while caregiving: It's complicated," conducted in 2023 by AARP and S&P Global shows that the unpredictable nature of caregiving for an adult is among the biggest stressors that caregivers face, the organizations said.

Here are some highlights:

◾ 67% of family caregivers have difficulty balancing their jobs with caregiving duties. 

◾ 27% of working caregivers have shifted from full-time to part-time work or have reduced hours, while 16% have turned down a promotion. 

◾ 16% have stopped working entirely for a period of time – and 13% have changed employers – to meet caregiving responsibilities. 

◾ 80% of respondents agree that companies were more understanding of child care issues than adult caregiving responsibilities. Study authors said that was particularly the case among caregivers who have an under-18 child in the home and therefore have recent experience in both situations.

◾ Caregivers without children reported less satisfaction with company support than caregivers with children – 69% versus 89%, respectively. 

◾ Remote workers were more likely to feel penalized or discriminated against at work because of caregiving responsibilities compared to in-office or hybrid workers – 49% versus a combined average of 29%. Study authors said, "This may reflect employer challenges in assessing and engaging with remote employees’ work-life needs."

◾ Among working caregivers providing more than 21 hours of care a week, 37% are experiencing significantly increased difficulty due to inflation. For those providing fewer than 10 hours of care, 25% say inflation has made providing care significantly more difficult. 

“Despite the progress observed since 2020, the latest data shows the majority of employees with adult caregiving responsibilities continue to face barriers at balancing work and caregiving obligations and need greater support from employers through enhanced benefits and policies to stay engaged in the workforce,” said Alexandra Dimitrijevic, co-chair of S&P Global Research Council in the press release. “Employers can help by paying forward-looking attention to employee needs and the demographics shift of the workforce in the coming years.” 

Some employers are making progress

Since the last joint report by AARP and S&P Global in 2020, employers, "having become attuned to the needs of working parents over the past decade and especially during the pandemic, are now moving from awareness to action in providing support for employees with adult caregiving responsibilities," the organizations said. 

According to the new report, access to a flexible work schedule at the time of caregiving increased from 32% in 2020 to 45% in 2023. The availability of caregiving policies or benefits also increased in every category except unpaid leave. 

However, the organizations said employers can provide more support. The report identifies industry best practices that companies can take to support caregivers, including: 

◾ Offer and support flexible schedules and flexible work locations (hybrid/remote). 

◾ Offer employer-supported access to support groups, career coaching and financial advising resources. 

◾ Offer paid leave specifically for caregivers and/or flexible leave that can be used to help with caregiving duties.

◾ Host free sessions to highlight how caregiving employees can optimize employer benefits and policies, as a way to address the lack of awareness in using benefits. 

◾ Ask senior leaders to share their stories of how they have used the caregiver-supportive benefits and policies, signaling to both people managers and their teams that they are encouraged to use them.

◾ Train people managers on caregiver-inclusive managerial practices. Ensure managers are aware of caregiver-supportive benefits and policies and message their direct reports that it is safe to use them all without incurring career risk.

◾ Start or support an Employee Resource Group for parents and caregivers or create a caregiving initiative across all ERGs.

Federal administration offering some help

During a media briefing about the study on Thursday, Xavier Becerra, secretary of the U.S. Department of Health and Human Services said when it comes to caregiving, more Americans are realizing how much pressure is put on the shoulders of a few caring for their older relatives. 

“Family caregivers are providing more support and have an even harder time getting the type of rest that they need as they care for their loved ones,” Becerra said. “It's becoming a real crisis for families in America.” 

Becerra said more families are having trouble finding the help they need or can’t afford to pay for that care or get care at a facility “and have to take care of that burden themselves.” 

Becerra said noted “the first-ever national strategy to support family caregivers, which we have been working across the board to implement. At the end of April, we took action to both improve the quality of care and strengthen support for caregivers, including those who make caregiving a profession and those who are doing the unpaid work of care at home as they support their loved one.” 

That includes final rules to nursing home staffing standards and rules to improve access to care in Medicaid and Children’s Health Insurance Program to home care services, he said. 

It also includes an executive order last April signed by President Joe Biden, which included 50 directives to federal agencies to increase access to affordable, high-quality care, and provide support to rising child care and family caregiving costs.

“It's going to take more than an administration,” Becerra said. “It'll take all of us; the voices of those who receive care and certainly the experience of caregivers.” 

Congressional bill could help caregivers

A bill reintroduced in January by a bipartisan group of U.S. Senators and members of the U.S. House of Representatives could provide help in the form of a federal tax credit for eligible working family caregivers. 

AARP has been working with legislators for eight years on previous versions of the bill. But senior AARP leaders in January told USA TODAY that they believe the time is finally right for it to become law – and bring relief to many caregivers, including members of Congress.

The bill has made no progress in Congress, but it does have more Congressional co-sponsors. In January, six senators and two representatives introduced the bill. It now has 33 additional sponsors in the House for a total of 39 in both chambers.

The Credit for Caring Act, if passed, would provide financial support for individual caregivers by providing up to a $5,000 nonrefundable federal tax credit for eligible working family caregivers, that would cover 30% of qualified expenses they incurred above $2,000.

AARP representatives had no new comment regarding the status of the bill, but Reinhard during the media briefing said the act could help caregivers.

Wide-Ranging Veterans Bill Gets Agreement Between House and Senate

The Department of Veterans Affairs caregiver program, the beleaguered electronic health records program, veteran homelessness support and more would see changes under a bipartisan, bicameral agreement unveiled Tuesday to advance a slew of legislative efforts that had been stalled.

The Senator Elizabeth Dole 21st Century Veterans Healthcare and Benefits Improvement Act combines several bills that have been introduced this Congress seeking to bolster services for veterans and their families. The agreement -- which was negotiated by House Veterans Affairs Committee Chairman Mike Bost, R-Ill.; Senate Veterans Affairs Committee Chairman Jon Tester, D-Mont.; and Sen. Jerry Moran of Kansas, the top Republican on the Senate committee -- is expected to move through Congress this summer.

"Over the past year and a half, we have listened to veterans and stakeholders to find the gaps within VA's services and consider common sense legislation to improve them where we can," Bost said in a statement. "This bill would expand economic opportunities for veterans, give veterans more choice in where they choose to live out their sunset years or receive health care, improve mental health support for veteran caregivers, hold VA accountable to the taxpayers and veterans they serve, and so much more."

An array of veterans legislation has been stalled this congressional session even though many of the bills have garnered bipartisan support, in part because lawmakers struggled to find ways to pay – or offsets, in congressional language – for the costs of the bills. In general, congressional rules require offsets for increases in what's known as mandatory spending.

The agreement unveiled this week is fully offset, negotiators touted in a summary, unlocking the stalled measures.

Among the negotiated package's more substantial changes is an expansion of VA-funded home- and community-based nursing care services so aging and disabled veterans can get those services through any VA medical center in the country.

The bill would also require the VA to offer alternative programs veterans' families can enroll in if they are kicked out of the Program of Comprehensive Assistance for Family Caregivers, which provides financial support for family members who act as caregivers. Potentially thousands of families are facing disqualification from the program under new eligibility rules, though the VA has paused any action until 2025 as it reviews the criteria.

The legislation would also create a grant program for family caregivers to get mental health care.

In addition, the bill would make several changes that aim to ease veterans' ability to get VA-funded health care from non-VA doctors. Specifically, it would ban the department from overriding a VA doctor's referral for their patient to get outside care and require the VA to conduct outreach to inform veterans of their ability to get outside care.

On the electronic health records program, the bill could allow the VA to pivot away from the troubled Oracle Cerner system. The VA paused rolling out the system at any more of its health care facilities to work on fixes after several scathing watchdog reports and growing lawmaker anger about patient safety issues resulting from system glitches.

The bill would allow the VA to proceed with updating its electronic health records system either by making changes to the Oracle Cerner program or "by establishing a new program," according to the bill text. It would also formally define the roles the deputy VA secretary, under secretary for health, and assistant secretary for information and technology have in overseeing the modernization program.

For veteran homelessness, the bill would revive through 2026 authorities the VA had during the COVID-19 public health emergency that were credited with helping reduce the number of unhoused veterans. Under the bill, the VA would again be able to provide homeless veterans with free transportation to get to medical appointments, jobs or support programs. The department would also be able to fund food, shelter, clothing, hygiene products, communications devices and other items homeless veterans need for their "safety and survival."

And the bill would increase the per diem rate the VA can pay to organizations providing short-term transitional housing from 115% of costs to 133%, as well as allow the department to go as high as 200% in particularly high-cost areas.

"In the past few years, we've done right by veterans by passing critical legislation like the PACT Act , but there's still work to be done to ensure we're fully honoring our commitment to them and their families," Tester said in a written statement. "By improving veterans' access to the care they need, bolstering long-term care options and assistance for homeless veterans, strengthening life-saving mental health services, and increasing transparency and oversight of VA's new electronic health record program, this package is a common sense step toward delivering veterans and their families the kind of support they earned and deserve."

Other provisions of the wide-ranging measure include extending and tweaking a high-tech job training program for veterans through 2026; ensuring that only veterans are responsible for paying back GI Bill benefits if they transferred the benefit to a dependent but become ineligible because of a less-than-honorable discharge or failure to complete their service obligation; and making it easier for the VA to automate disability claims decisions by requiring contractors to submit exam paperwork in a machine-readable format.

The bill is being formally introduced by Rep. Juan Ciscomani, R-Ariz.

In addition to bipartisan support from lawmakers, the bill has garnered endorsements from leading veterans service organizations, including the American Legion, Disabled American Veterans and Paralyzed Veterans of America, among others, according to a news release. The Elizabeth Dole Foundation, founded by the bill's namesake to support caregivers, is also endorsing the measure.
