For example, instead of simply saying “I am dedicated to professional ethics, and I have leadership skills”, you can write:

Do Address Gaps

If you have any gaps or setbacks in your resume or CV, a cover letter is a perfect place to briefly address them. Do not take up the entire space to make excuses for these problems, but try to explain why x, y, or z happened. For example, if you have a large gap in your resume due to illness or if you had to take a leave of absence from your position, address it like so:

“Having had to take a prolonged leave of absence from my position due to cancer diagnosis, I am more than eager to return to working with patients and doing my best to promote their well-being and health.”

Remember not to dwell on the gap or the setback. A brief explanation is best. If the hiring committee wants to further investigate it, you will be asked about this issue in the interview. And keep in mind that if there is a reasonable explanation for the setback in your cover letter, then the setback should not prevent the hiring committee from speaking with you in person. Leaving the gap or the setback unaddressed is more of a red flag than including a brief explanation in your letter.

Don’t Just Repeat CV/resume

It’s worth repeating that your cover letter is not a CV, so do not simply repeat the same information in your cover letter. While you can certainly bring up the jobs or positions you list in the CV, use the cover letter to express items you cannot include in the CV, such as your genuine interest in the position, your connection to the institution you are applying to, your connection to the geographical area, your soft skills, your background, and so on. You have a limited amount of space to convey your suitability for the position, so repeating the same information in the same format throughout your job application is just not a wise approach.

Your physician cover letter should not be more than a page, so around 3 to 5 paragraphs long. Use professional language and avoid a colloquial tone. Make sure to structure it as an academic essay, with an intro, body, and conclusion:

One challenge of cover letter writing is to follow a prescribed format yet simultaneously set yourself apart from your peers. This can be accomplished by clearly and concisely articulating your core professional goals and your commitment to the calling of medicine.

The cover letter is usually the first bit of written communication from job seekers that hits the desk of a hiring physician, staff recruiter, or human resources professional. As such, it can make the difference between getting an interview and your dossier landing in the “maybe” pile.

Although writing a cover letter may be an unfamiliar challenge for many residents, it’s not terribly difficult if you follow three basic rules: Keep it short, make it clear, and cover the basics. “Think about the needs of the reader,” says James W. Tysinger, Ph.D., deputy chair for education in the Department of Family and Community Medicine at the University of Texas Medical School and author of Resumes and Personal Statements for Health Professionals . “Let the reader know right away who you are and why you are writing.”

Brevity, Personalization, and Passion Tysinger recommends starting the letter with a simple introductory statement such as “I am a third-year family-practice resident at [insert program name]. I seek a position as a family practice physician in your practice” (or “in your fellowship program” or “at your institution,” as the case may be).

Ideally, the cover letter should be no more than three paragraphs that fit on a single page. “Simplicity is best,” Tysinger says. “Letters that go on for two or three pages, or that try to impress rather than inform, may cause the reader to lose interest.”

To increase the chance that your application ends up atop the “active” pile, whenever possible personalize the letter and indicate that you have some knowledge of the position and institution, says James St. Clair, a senior search consultant with J&C Nationwide in Atlanta. Always write to the appropriate person, St. Clair advises, and unless you’re answering a blind ad, avoid salutations such as “to whom it may concern,” which may give the impression that you’re uncertain about yourself and the process.

Both Tysinger and St. Clair also urge physicians to briefly mention why they are interested in pursuing a position in a particular region. “You want to create a compelling tie to the area rather than simply stating that you’re looking for a position as a general internist,” St. Clair says. If you or your spouse is a native of the area where the practice is located, for example, you could add the following: “My husband and I grew up in [name the city], and we are eager to move back there.”

Mazie Blanks, a 30-year veteran of the physician-recruiting field who now recruits for the Permanente Medical Group in Northern California, advises ratcheting personalization up into the realm of passion. “The cover letter is an opportunity to differentiate yourself from others,” Blanks says. “Show some passion, because people want to hire physicians who love what they do.” Because personalization and passion are so important, Blanks also discourages physicians from using cookie-cutter cover letters, such as those included in “one-size-fits-most” software packages.

Cover Letter Do’s and Don’ts Pay attention to basic details. Spell all names, titles, and addresses correctly and ensure the letter doesn’t contain any grammatical errors. Make sure your contact information is listed accurately.

Write directly and avoid generalities. Rather than “I would like to express my interest in the position that is currently available at your practice,” write the following: “I seek a position as a gastroenterologist at your clinic.” Specifically mention the appeal of the practice opportunity.

Highlight special expertise. Subspecialists and some specialists familiar with new procedures should briefly explain their expertise and/or the number of procedures they’ve performed.

Briefly address any time gaps in training or practice that are evident in your CV to prevent potentially negative reader assumptions.

Request confidentiality, if necessary, and make sure contact information (which may include e-mail addresses) directs respondents to channels that are suitable for confidential communications.

Obtain professional writing or editing help. If writing isn’t your strong suit, ask a program director or a professional writer to help you craft the letter. Remember, poorly written letters often give a negative first impression.

Use high-quality, easy-to-copy paper. White or off-white, 24-pound paper is best because the cover letter and CV are frequently photocopied and routed to several individuals. Avoid colored paper or paper with a high rag content, both of which may reproduce poorly.

Resources Writing tips and sample letters . Writing Help-Central, at www.writinghelp-central.com , provides tips for writing clear, concise cover letters and other documents. The American Medical Association ( www.ama-assn.org ) and the American Academy of Family Physicians ( www.aafp.org ) also provide helpful resources for physicians entering the job market.

One of the most common mistakes job seekers make is not addressing their application to anyone in particular. Make sure you do so with your physician cover letter. If you don’t know who will be reviewing your application, call the company and ask. You can find the name by checking out their website or LinkedIn page.

Your cover letter is a great opportunity to show the hiring manager that you’re more than just a resume. You can use this opportunity to share your personality and interests as well as demonstrate leadership skills and communication abilities.

For example, you might mention that you love working with people or have experience communicating health information in a way that’s easy for patients to understand. This will help them see how they can incorporate these attributes into their workplace!

Proofread your cover letter

Physician cover letters are only as good as the proofreading behind them. Cover letters are often read by hiring managers who are looking for an excuse to disqualify applicants. An error in a cover letter, such as grammar or spelling, will likely give them one. Read over your work carefully before submission and be sure to have someone else proofread it if you can’t do so yourself.

In the mid-1960s, physicians and educators recognized there was a shortage of primary care physicians. Eugene A. Stead Jr., MD, of the Duke University Medical Center, selected four Navy Hospital Corpsmen who had received considerable medical training during their military service from the Vietnam War, to be the first class of physician assistants/associates (PAs) in 1965. 

In 1967, the PA profession was formally established with the goal of improving and expanding healthcare.

What is a Physician Assistant (PA)?

PAs are licensed medical professionals who practice medicine in every specialty and setting. The PA education emphasizes health and wellness through patient-centered, team-based medical practice. The specific duties of a PA depend on the setting in which they work, their level of experience, their specialty, and state laws.

Generally, PAs can take medical histories, perform physical exams, diagnose, and treat illness, order and interpret tests, develop treatment plans, prescribe medication, counsel on preventative care, perform procedures, assist in surgery, make rounds in hospitals and nursing homes, and do clinical research. Like physicians, they can provide primary care. They work in hospitals, medical offices, community health centers, nursing homes, workplace clinics, educational facilities, and correctional institutions. 

Why become a PA?

The Physician Assistant (PA) profession is one of the fastest growing in the country and ranked No. 2 in Best Health Care Jobs based on U.S. News . The U.S. Bureau of Labor Statistics projects that the profession will increase 28 percent from 2021 to 2031, significantly faster than the average for all occupations. 

The PA profession allows for versatility, with the ability to specialize in one or several areas throughout their career. Being a PA provides the ability to give back to the community and develop relationships with patients and make a difference in their quality of life.

An initiative called the PA Compact , an inter-state agreement between the American Academy of Physician Associates ( AAPA ), the Federation of State Medical Boards ( FSMB ), and the National Commission on Certification of Physician Assistants ( NCCPA ), greatly simplifies receiving authorization to work in participating member states.

Currently, there are more than 168,300 PAs in the United States, and they make a mean annual wage of $130,490 (via U.S. Bureau of Labor Statistics ).

Getting the education and certification

To become a PA, you must graduate from an ARC-PA accredited PA program . PAs are educated at the master’s degree level, and admission into a PA program requires a bachelor’s degree and completion of prerequisite courses in basic and behavioral sciences.

Many PA programs also require prior health care experience with direct patient care, such as working as a medical assistant, emergency medicine technician, paramedic, phlebotomist, emergency room technician, surgical technician, military medicine/corpsman, or certified nursing assistant. 

Most PA programs have the following prerequisites: anatomy, physiology, biology, microbiology, chemistry, genetics, and statistics. 

The PA school curriculum is modeled on the medical school curriculum that involves both didactic (classroom) and clinical education training. The didactic phase includes classroom instruction on courses in basic medical sciences, diagnostic and procedural skills, pharmacology and therapeutics, behavioral sciences, and behavioral ethics. In the clinical phase, students complete clinical rotations in medical and surgical disciplines, including family medicine, internal medicine, obstetrics and gynecology, pediatrics, general surgery, emergency medicine, and behavioral medicine/psychiatry.

PA graduates are eligible to take the Physician Assistant National Certifying Exam ( PANCE ) administered by the National Commission on Certification of Physician Assistants ( NCCPA ). To obtain a state license, all states require that PAs graduate from an accredited PA program and pass the PANCE.

Admission requirements

  • Bachelor's degree (any major) completed at an approved accredited institution.
  • Complete prerequisites with a C or better
  • Overall cumulative and overall science GPA of 3.0 or higher as calculated by the Centralized Application Service for Physician Assistants ( CASPA ).
  • Patient Care Experience minimum 1000 hours.
  • Three Letters of Recommendation. One must be from a licensed clinician (PA, MD, DO, NP), one from a supervisor or manager, and the other from whoever the candidate chooses.
  • Community service is not required, but strongly recommended. Preference is given to candidates with 200 hours or more. 

Preference will be given to candidates who serve under-resourced or underserved populations (e.g., rural, immigrant/refugee, unhoused, and other vulnerable and marginalized communities), those who work in settings such as Federally Qualified Health Centers , Community Health Centers, Indian Health Service, prisons, primary care (family medicine, internal medicine, pediatrics, obstetrics/gynecology), urgent care, and behavioral health. This may be demonstrated by their patient care hours, health care hours, PA shadowing, or community service/volunteer hours.

Prerequisite courses

Must complete all required coursework before submitting CASPA application. All required coursework must be completed at an institution of higher education accredited by a TUC-approved accrediting body.

The prerequisite courses listed below must be completed with a grade of C or better:

Biological sciences including labs – 8 semester units or 12 quarter units that may be comprised of general biology, genetics, immunology, embryology, etc.

Chemistry including labs – 8 semester units or 12 quarter units that may be comprised of inorganic, organic, or biochemistry.

Human Anatomy including lab – 4 semester units or 5 quarter units that may be combined Human Anatomy & Physiology I & II with lab. Must be completed within 6 years of the expected year of matriculation.

Human Physiology including lab – 4 semester units or 5 quarter units that may be combined Human Anatomy & Physiology I & II with lab. Must be completed within 6 years of the expected year of matriculation.

Microbiology – 3 semester units or 4 quarter units (lab not required).

The Biden administration this week unveiled new guidance for a federal student loan forgiveness program designed for borrowers experiencing disabling medical conditions.

The new details for the Total and Permanent Disability discharge program coincide with an amended discharge application , and are associated with new TPD discharge regulations that went into effect last summer. The updated rules expand the categories of medical providers who can certify that a borrower meets the TPD discharge standard, paving the way for borrowers with disabilities to more easily obtain loan forgiveness.

The new guidance published by the Education Department is designed specifically for medical providers who have been asked to certify a borrower’s TPD discharge application. The guidance can help borrowers avoid rejection by ensuring that their provider knows how to complete the form correctly, and how to avoid common mistakes. Here are the details.

Student Loan Forgiveness Based On Health Conditions

The TPD Discharge program — a longstanding federal student loan forgiveness program — can completely eliminate a borrower’s federal student debt. Direct and FFEL-program loans can qualify.

To be eligible, a borrower must be unable to maintain substantial, gainful activity due to a physical or psychological condition that has persisted for five years, is expected to continue for five years, or is terminal. The program provides borrowers with three possible pathways to demonstrate that they meet this standard and therefore should qualify for loan forgiveness:

  • The borrower is certified by the Veterans Administration as 100% disabled due to a service-connected condition.
  • The borrower is receiving Social Security disability benefits under certain circumstances and criteria.
  • A borrower’s medical provider certifies on a form accompanying the TPD discharge application that they meet the legal standard for a discharge.

Previously, only a medical doctor or doctor of osteopathy could certify a TPD discharge application. Under the new regulations that went into effect in July 2023, additional providers can complete this certification including nurse practitioners, physician assistants, and certified psychologists. All medical providers must be licensed to practice in the United States.

Biden Administration’s New Medical Provider Guidance For TPD Loan Forgiveness

The Education Department’s new TPD discharge guidance is specifically designed for medical providers certifying that a patient meets the standard for loan forgiveness under the program. The guidance clarifies key elements of the program, explains how to avoid mistakes on the application, and outlines what providers should expect during the review process, given that many medical providers will be completing the TPD certification for the first time in their careers.

The guidance clarifies that a borrower’s inability to engage in “substantial gainful activity” means that they cannot engage in “a level of work performed for pay or profit that involves doing significant physical or mental activities or a combination of both.” It does not, however, mean that they cannot work at all. “Borrowers who work and earn income can qualify for TPD discharge provided they are not engaged in substantial gainful activity, ” notes the guidance, which is defined as “a level of work performed for pay or profit that involves doing significant physical or mental activities or a combination of both.”

The department’s guidance also instructs providers on how to properly complete the medical provider’s certification page on the TPD discharge application, which requires detailed information on both the provider’s credentials and the borrower’s medical conditions. “On the application form, authorized medical professionals must provide their state and professional license number, which we verify for validity,” says the guidance. Providers “ must answer all questions and write answers in every text box, even if to indicate ‘not applicable,’” says the department. “Missed questions or blank text boxes could result in rejection of the application.” In addition, when answering questions about the borrower’s diagnosis and conditions, “insurance codes or abbreviations should not be used.”

Finally, the guidance advises medical providers that the department could contact them while a borrower’s application for loan forgiveness is under review, particularly if there is incomplete or missing information. “The authorized medical professional must provide their mailing address, email, telephone, and fax so that we can follow up when needed,” says the guidance.

More than $14 Billion In Student Loan Forgiveness Approved Through TPD Discharge Program

The expansion of the TPD discharge program to include additional medical providers who can certify a borrower’s eligibility is just one of several reforms the Biden administration has enacted for the program.

The new regulations also make it easier for borrowers receiving Social Security disability benefits to qualify for loan forgiveness. Previously, borrowers must have been on a medical review cycle of at least five to seven years to receive TPD relief. Under the new rules, borrowers can also qualify if their next continuing disability review has been scheduled at three years, if they have a medical onset date for SSDI or SSI of at least five years before applying for a TPD discharge, or if they qualify for benefits based on a compassionate allowance. Many borrowers on SSI and SSDI could get a TPD discharge automatically through a data-sharing initiative between the Education Department and Social Security Administration.

The new regulations also eliminated post-discharge income monitoring, a burdensome administrative requirement that mandated that borrowers return forms to the department documenting their employment income every year for three years following a TPD approval. Failure to respond to the form on time — even if a borrower’s medical condition made doing so difficult or impossible — often resulted in discharge reversals.

As a result of these changes, the Education Department estimates that 548,000 borrowers have received student loan forgiveness through the TPD program, with $14.1 billion in total discharges.

Additional Student Loan Forgiveness For Borrowers With Disabilities

While the Biden administration’s reforms to the TPD discharge program are already yielding meaningful results for hundreds of thousands of borrowers, a new loan forgiveness plan could provide additional relief.

President Biden’s so-called “Plan B” student loan forgiveness initiative, designed as a backup plan to his first debt relief initiative that wound up getting blocked by the Supreme Court, is expected to go live sometime this fall . If that plan is enacted — despite anticipated legal challenges — borrowers could qualify for loan forgiveness under several categories. One such category includes borrowers who qualify for existing loan forgiveness programs (like the TPD discharge program) but have not applied.

Another category would provide relief based on hardship. A borrower’s age and medical conditions could be factors in determining whether a borrower qualifies for loan forgiveness under this prong, as could associated medical expenses or medical debts.

The Education Department published draft regulations for the new program last month. The regulations are currently open for public comment through next week.

The US Food and Drug Administration (FDA) has 3 regulatory programs intended to expedite both clinical development and FDA’s review of novel therapeutics targeting serious conditions: fast track, accelerated approval, and breakthrough. 1 Fast track and breakthrough provide opportunities for increased interaction between manufacturers and the FDA as well as rolling review of marketing applications. 1 Breakthrough and accelerated approval allow for approval based on preliminary clinical evidence, including trials using surrogate markers as primary end points to shorten the duration and size of clinical trials, reducing clinical development times. 1 A fourth regulatory program, priority review, was designed to expedite the review process from 10 to 6 months. Previous studies have evaluated the implications of the FDA’s expedited regulatory programs for the combined clinical development and review times for novel therapeutics, but their findings have been inconsistent and have not focused on clinical development times, the target of these 3 programs. 2 , 3 Given their growing use, 4 we assessed whether expedited regulatory programs were associated with faster clinical development times for FDA-approved novel therapeutics.

In accordance with the Common Rule, this cross-sectional study was exempt from ethics review and informed consent because it used public, nonidentifiable data and was not human participant research. We followed the STROBE reporting guideline.

We used the FDA Novel Drug Summary to identify all new drugs and biologics that were approved between January 2015 and December 2022 and their corresponding expedited regulatory programs. Drugs@FDA was used to identify Investigational New Drug (IND) application dates, corresponding with the start of human testing, and New Drug Application (NDA) or Biologics License Application (BLA) submission and approval dates. For therapeutics without IND application dates on Drugs@FDA, we searched patent extension notices using the Federal Register , ClinicalTrials.gov, or Google for these dates or phase-1 study start dates. For therapeutics with multiple formulations, we selected the earliest IND application dates. Clinical development times and combined clinical development and review times were calculated as the time from IND application to NDA or BLA submission and approval, respectively, and were compared across expedited regulatory program used (primary analysis: ≥1 vs none; secondary analyses: fast track, accelerated approval, or breakthrough vs none) along with therapeutic type and area.

Analysis was performed using Mann-Whitney tests in RStudio (Posit). Two-tailed P  < .05 were considered statistically significant.

The FDA approved 367 therapeutics between 2015 and 2022, of which 7 had multiple formulations, 6 were developed outside of the US, and 14 had no publicly available IND dates. Of the remaining 340 approvals, 196 (57%) used at least 1 expedited regulatory program: 118 (35%) fast track, 64 (19%) accelerated approval, and 109 (32%) breakthrough.

Median (IQR) clinical development times for approvals using 1 or more vs no expedited regulatory programs were 6.0 (4.0-8.2) vs 7.2 (5.3-9.5; P  = .001) years, respectively ( Table 1 ). Median (IQR) combined clinical development and review times were 7.0 (4.8-9.0) and 8.3 (6.5-11.4; P  < .001) years, respectively, for 1 or more vs no expedited regulatory programs ( Table 2 ). Differences were consistently statistically significant only for cancer and hematology and other category therapeutics. Compared with approvals using no expedited program (7.2 [5.3-9.5] years), approvals using accelerated approval (4.9 [4.0-7.4] years; P  < .001) and breakthrough (5.2 [3.8-7.6] years; P  < .001) had significantly shorter median (IQR) clinical development times, but not fast track (6.6 [4.2-9.3] years; P  = .10).

We found that FDA approvals using expedited regulatory programs had shorter clinical development times and combined clinical development and review times by approximately 1 to 2 years, demonstrating the association of these programs with faster therapeutic clinical testing and approval. Study limitations included the reliance on publicly available IND submission dates, which were not uniformly reported. Given that expedited regulatory programs often facilitate faster approval through postmarketing studies required to address residual clinical uncertainties, ongoing efforts are needed to ensure that these studies are completed and reported in a timely manner. 5 , 6
