Trans-feminine (Male to Female) Surgeries & Treatments

Adam’s apple reduction.

Reduces the size of the Adam’s apple in trans women. During this procedure, the surgeon shaves down the thyroid cartilage through a small incision in the crease under the chin. The scar is well hidden, and the procedure has no impact on the patient’s voice.

Brow Softening

With a brow softening procedure, the ridge of the bone surrounding the upper eye sockets is shaved down, the brow is lifted, and the shape of the forehead is recontoured by shaving and surgically altering the bones for a more feminine aesthetic appearance.

Nose Reshaping

With nose reshaping, the cartilage of the nose and surrounding tissue is shaped to have smaller, more feminine features.

Cheek Implants

Implants are used to create rounder, softer, and more prominent cheek features.

Lip Lift & Lip Implants

Lip lifts and lip implants create fuller, softer lips for a more feminine appearance.

Hormone treatments are often used to enhance the breasts for patients transitioning from male to female. Some patients prefer a larger breast size and will undergo breast augmentation . Breast augmentation uses silicon or saline devices that are surgically implanted under the skin.

Trans-Masculine (Female to Male) Surgeries and Treatments

Forehead lengthening/augmentation.

For this procedure, the surgeon will lengthen the forehead by making it larger and more angular for a more masculine appearance. The surgical incision can be made within the hairline to avoid visible scarring.

Cheek Augmentation

The surgeon places implants between the bone and the skin to create a more chiseled and masculine shape. Cheek augmentation leaves no visible scars as the surgical incision is made inside the mouth.

Rhinoplasty

With a trans-masculine rhinoplasty, the goal is to create a larger and/or wider nose.

Chin & Jaw Contouring

With chin recontouring, the size of the chin is increased for a more angular appearance. During this procedure, the bone is moved, or implants are used to create a more masculine aesthetic. Jaw contouring involves the widening of the face, providing definition between the lower face and neck.

Adam’s Apple Enhancement

With thyroid cartilage enhancement, the surgeon places an implant in the throat area.

This surgery involves the removal of female breast tissue through a bilateral mastectomy and contouring the chest to achieve a more masculine shape.

Specialists in the  Transgender Health Program  and Transgender Surgical Program at Massachusetts General Hospital are dedicated to ensuring a welcoming and affirming environment for all patients. We offer patients a wide spectrum of gender-affirming services to support them throughout their entire care journey, including comprehensive primary care, surgical treatment, hormone management, case management and behavioral health support.

The Transgender Surgical Program is a collaboration with specialists from the Mass General  Division of Plastic and Reconstructive Surgery , the  Department of Urology  and the  Department of Obstetrics and Gynecology . Our multidisciplinary surgical team works closely with providers from the Transgender Health Program and every patient to develop a safe and all-inclusive surgical plan that aligns with the patient’s personal goals.

Gender-Affirming Surgical Procedures

Our expert team performs a variety of highly specialized gender-affirming surgical procedures tailored to meet the needs of transgender and gender-nonconforming patients.

• Vaginoplasty , sometimes referred to as bottom surgery, is a surgery that is used to create a vulva, labia, and vaginal canal
• Orchiectomy , also known as testicle removal surgery and/or bottom surgery, is a procedure in which the testicles are surgically removed
• Hysterectomy and/or oophorectomy is a surgery that involves the removal of the uterus and, in some case, the ovaries
• Phalloplasty , sometimes referred to as bottom surgery, is surgery to create a penis. It can include a variety of different procedures, depending on individual goals
• Mastectomy , sometimes referred to as top surgery, is a surgery to remove breast tissue from the chest
• Breast augmentation is surgery that uses implants made of silicone or saline to enhance the size of a person’s natural breasts
• Facial feminization or masculinization is surgery to alter facial features—the chin, nose, cheeks, forehead, etc.—to create a more feminine or masculine facial structure
• Voice feminization surgery is a procedure that alters the pitch and quality of an individual's voice to align it with a more feminine sound though surgery that involves lengthening the vocal cords or adjusting the tension of the vocal cords. Before undergoing surgical intervention, patients will work with a speech therapist specializing in voice feminization
• Voice masculinization surgery is a procedure that alters the pitch and quality of an individual's voice to align it with a more masculine sound though surgery to decrease the tension of the vocal cords. In addition to surgery, some individuals may benefit from speech therapy to further refine their vocal skills and communication. This procedure is not common, as only 75% of people can masculinize their voice with hormone therapy

Frequently Asked Questions About Gender-Affirming Surgery

For genital or “bottom” surgery, the first step is to schedule an initial visit with the Transgender Health Program. To schedule this appointment, call 617-726-3525 or email us . For breast or “top” surgery, you are not required to schedule an intake visit with the Transgender Health Program, unless you need a referral for support services. Instead, please contact the Transgender Surgery Program team at 617-726-3525 to learn how to proceed with a surgical consult. Providers should fax referrals to 617-724-7126.

Following confirmation from the Transgender Health Program that you are ready to move forward with bottom surgery, the Transgender Surgery Program team will contact you to set up a surgical consult.

Fertility preservation offers the opportunity to freeze eggs or sperm to be used for building a family in the future. This can be done prior to initiation of gender affirming hormone therapy or surgery. For some, it can also be used after hormonal therapy has already been started. Resources and care are also available for coordinating use of eggs or sperm from another person (donor) or for another individual to carry a pregnancy (gestational carrier).

Patients who desire fertility preservation or family building through Mass General must:

• Complete a consultation with the Mass General Fertility Center
• Be of reproductive age (requirements vary by family building plan)
• Follow center-specific guidance and protocols for selected treatment

During your surgical consultation, your physician will ask you about your fertility preservation goals and will assist you with setting up a consultation with a fertility specialist. We are committed to supporting you and guiding you through this process.

Required documents prior to gender-affirming surgery vary depending on the type of procedure. Our surgical program coordinator will assist you with questions and provide you with more information during your surgical consult.

Once we’ve scheduled a date for your surgery, we will submit a prior authorization for surgery to your insurance company for approval. Some out-of-network insurances may require you to obtain prior authorization for surgical consultations. Please reach out to your insurance company to determine what is required. Our dedicated surgical coordinator is happy to assist you with this process.

There are medical rates at many of the surrounding local hospitals. Depending on the procedure, you may need to remain local for a few weeks. If you are interested in medical rates, our dedicated surgical coordinator will be happy to provide you with more information.

Gender Confirmation Surgery

The University of Michigan Health System offers procedures for surgical gender transition.  Working together, the surgical team of the Comprehensive Gender Services Program, which includes specialists in plastic surgery, urology and gynecology, bring expertise, experience and safety to procedures for our transgender patients.

Access to gender-related surgical procedures for patients is made through the University of Michigan Health System Comprehensive Gender Services Program .

The Comprehensive Gender Services Program adheres to the WPATH Standards of Care , including the requirement for a second-opinion prior to genital sex reassignment.

Available surgeries:

Male-to-Female:  Tracheal Shave  Breast Augmentation  Facial Feminization  Male-to-Female genital sex reassignment

Female-to-Male:  Hysterectomy, oophorectomy, vaginectomy Chest Reconstruction  Female-to-male genital sex reassignment

Sex Reassignment Surgeries (SRS)

At the University of Michigan Health System, we are dedicated to offering the safest proven surgical options for sex reassignment (SRS.)   Because sex reassignment surgery is just one step for transitioning people, the Comprehensive Gender Services Program has access to providers for mental health services, hormone therapy, pelvic floor physiotherapy, and speech therapy.  Surgical procedures are done by a team that includes, as appropriate, gynecologists, urologists, pelvic pain specialists and a reconstructive plastic surgeon. A multi-disciplinary team helps to best protect the health of the patient.

For patients receiving mental health and medical services within the University of Michigan Health System, the UMHS-CGSP will coordinate all care including surgical referrals.  For patients who have prepared for surgery elsewhere, the UMHS-CGSP will help organize the needed records, meet WPATH standards, and coordinate surgical referrals.  Surgical referrals are made through Sara Wiener the Comprehensive Gender Services Program Director.

Male-to-female sex reassignment surgery

At the University of Michigan, participants of the Comprehensive Gender Services Program who are ready for a male-to-female sex reassignment surgery will be offered a penile inversion vaginoplasty with a neurovascular neoclitoris.

During this procedure, a surgeon makes “like become like,” using parts of the original penis to create a sensate neo-vagina. The testicles are removed, a procedure called orchiectomy. The skin from the scrotum is used to make the labia. The erectile tissue of the penis is used to make the neoclitoris. The urethra is preserved and functional.

This procedure provides for aesthetic and functional female genitalia in one 4-5 hour operation.  The details of the procedure, the course of recovery, the expected outcomes, and the possible complications will be covered in detail during your surgical consultation. Female-to-male sex reassignment

At the University of Michigan, participants of the Comprehensive Gender Services Program who are ready for a female-to-male sex reassignment surgery will be offered a phalloplasty, generally using the radial forearm flap method. 

This procedure, which can be done at the same time as a hysterectomy/vaginectomy, creates an aesthetically appropriate phallus and creates a urethera for standing urination.  Construction of a scrotum with testicular implants is done as a second stage.  The details of the procedure, the course of recovery, the expected outcomes, and the possible complications will be covered in detail during your surgical consultation.
