A Guide to Guidelines for Nursing Education Researchers

This installment of the Methodology Corner discusses the important role of research and quality improvement project reporting guidelines in the development of a field's evidence base. Several prominent guidelines, all with relevance to nursing education researchers, are described. Research and quality improvement reporting guidelines can be useful to researchers and quality improvement leaders long before the dissemination stage of a project and should be consulted from the earliest stages of a project. Consulting these guidelines earlier, rather than later, can increase the likelihood that studies and quality improvement reports will contribute meaningfully to the literature to which nurse educators look when making evidence-informed decisions. Appendix 8 Nursing Research: Definitions and Directions

In order to provide further insight into the need for, philosophy, and scope of nursing research this appendix presents a position statement issued by the Commission on Nursing Research of the American Nurses' Association. It is quoted here in its entirety: 1

Recent years have seen a growing awareness among the public that valuable resources are finite and their use must be carefully considered. In this context, increasing attention is being given to the relative cost of various strategies for utilizing health care resources to meet the present and emerging needs of the nation. Concurrently, nurses are assuming increased decision-making responsibility for the delivery of health care, and they can be expected to continue to assume greater responsibility in the future. Therefore, the timeliness and desirability of identifying directions for nursing research that should receive priority in funding and effort in the 1980s is apparent.

The priorities identified below were developed by the Commission on Nursing Research of the American Nurses' Association, a nine-member group of nurses actively engaged in research whose backgrounds represent considerable diversity in preparation and experience. The priorities represent the consensus of the commissioners, developed through a process of thoughtful discussion and careful deliberation with colleagues.

Accountability to the public for the humane use of knowledge in providing effective and high quality services is the hallmark of a profession. Thus, the preeminent goal of scientific inquiry by nurses is the ongoing development of knowledge for use in the practice of nursing; priorities are stated in that context. Other guiding considerations were the present and anticipated health problems of the population; a historic appreciation of the circumstances in which nursing action has been most beneficial; nursing's philosophical orientation, in which emphasis is on a synthesis of psychosocial and biomedical phenomena to the end of promoting health and effective functioning; and projections regarding the types of decisions nurses will be making in the last decades of the twentieth century. New, unanticipated problems will undoubtedly confront the health care resources of the country; yet it is clear that many of the problems of the future are already manifest today. New knowledge is essential to bring about effective solutions. Nursing research directed to clinical needs can contribute in a significant way to development of those solutions.

• Definition of Nursing Research

Nursing research develops knowledge about health and the promotion of health over the full lifespan, care of persons with health problems and disabilities, and nursing actions to enhance the ability of individuals to respond effectively to actual or potential health problems.

These foci of nursing research complement those of biomedical research, which is primarily concerned with causes and treatments of disease. Advancements in biomedical research have resulted in increased life expectancies, including life expectancies of those with serious injury and those with chronic or terminal disease. These biomedical advances have thus led to growth in the numbers of those who require nursing care to live with health problems, such as the frail elderly, the chronically ill, and the terminally ill.

Research conducted by nurses includes various types of studies in order to derive clinical interventions to assist those who require nursing care. The complexity of nursing research and its broad scope often require scientific underpinning from several disciplines. Hence, nursing research cuts across traditional research lines, and draws its methods from several fields.

• Directions for Research

Priority should be given to nursing research that would generate knowledge to guide practice in:

Promoting health, well-being, and competency for personal care among all age groups;

Preventing health problems throughout the life span that have the potential to reduce productivity and satisfaction;

Decreasing the negative impact of health problems on coping abilities, productivity, and life satisfaction of individuals and families;

Ensuring that the care needs of particularly vulnerable groups are met through appropriate strategies;

Designing and developing health care systems that are cost-effective in meeting the nursing needs of the population.

Examples of research consistent with these priorities include the following:

• Identification of determinants (personal and environmental, including social support networks) of wellness and health functioning in individuals and families, e.g. avoidance of abusive behaviors such as alcoholism and drug use, successful adaptation to chronic illness, and coping with the last days of life.
• Identification of phenomena that negatively influence the course of recovery and that may be alleviated by nursing practice, such as, for example, anorexia, diarrhea, sleep deprivation, deficiencies in nutrients, electrolyte imbalances, and infections.
• Development and testing of care strategies to do the following: Facilitate individuals' ability to adopt and maintain health enhancing behaviors (e.g. alterations in diet and exercise). Enhance patients' ability to manage acute and chronic illness in such a way as to minimize or eliminate the necessity of institutionalization and to maximize well-being. Reduce stressful responses associated with the medical management of patients (e.g. surgical procedures, intrusive examination procedures, or use of extensive monitoring devices). Provide more effective care to high-risk populations (e.g. maternal and child care service to vulnerable mothers and infants, family planning services to young teenagers, services designed to enhance self-care in the chronically ill and the very old). Enhance the care of clients culturally different from the majority (e.g. Black Americans, Mexican-Americans, Native Americans) and clients with special problems (e.g. teenagers, prisoners, and the mentally ill), and the underserved (the elderly, the poor, and the rural).
• Design and assessment, in terms of effectiveness and cost, of models for delivering nursing care strategies found to be effective in clinical studies.

All of the foregoing are directly related to the priority of developing the knowledge and information needed for improvement of the practice of nursing.

While priority should be given to this form of clinical research, there is no intent to discourage other forms of nursing research. These would include such investigations as those utilizing historical and philosophical modes of inquiry, and studies of manpower for nursing education, practice, and research, as well as studies of quality assurance for nursing and those for establishment of criterion measures for practice and education.

From the Bedside to Research: Navigating Change in Nursing

As we celebrate National Nurses Week, the AcademyHealth Interdisciplinary Research Group on Nursing Issues shines a spotlight on the indispensable contributions of nurses to the health care sector. National Nurses Week is a time to celebrate and reflect on the evolving role of nurses and their significant impact on the health care system. As frontline caregivers, nurses face a myriad of challenges, yet they continue to lead and innovate, shaping the future of health care with their resilience and pioneering spirit.

Addressing Nurse Compensation

The establishment of the  Commission for Nurse Reimbursement in 2023 marks a significant shift in the financial recognition of nurses within the health care system. Historically, nursing services have been bundled into the general costs of health care, much like hospital supplies, leading to undervaluation and lack of visibility in the billing process. This commission, led by co-chairs Rebecca Love, RN, BS, MSN, FIEL, and Sharon Pearce, DNP, CRNA, FAANA, FAAN, was formed in response to a critical staffing shortage and aims to transform nursing care into a billable service. By advocating for changes in how Medicare and other entities reimburse for nursing services, the commission seeks to elevate the financial stature of nursing, enhancing job satisfaction, and retention, and ultimately stabilizing health care services. This initiative not only acknowledges the indispensable role of nurses but also strives to create a sustainable work environment that attracts and retains skilled professionals

Combating Workplace Violence

Workplace safety is another critical issue in the nursing sector, particularly the rising incidents of violence that nurses face. On average,  two nurses per hour are assaulted in acute care settings, underscoring the urgent need for better protection. Strategies to combat this include training staff in de-escalation techniques, implementing stricter security protocols, and establishing support systems for those affected by violence. Ensuring the safety of nurses is crucial for allowing them to provide the best possible care to their patients.

Policy Engagement: Amplifying Nurses' Voices

Nurses play a vital role in health care policymaking, given their direct patient care experience and patient-centered perspectives. Their active participation in legislative discussions helps ensure that health policies are practical and effectively implemented. Nurses advocate for policies that reflect the realities of patient care, steering the health care system toward more effective and humane outcomes.

Nurse Researchers: Pioneers of Innovation

Nurse researchers are at the forefront of refining nursing practices and introducing innovations that improve patient outcomes. Their work in 2024 is particularly vital as they research staffing models, patient care techniques, and the integration of new technologies such as digital health records and predictive analytics tools. These innovations are transforming everyday nursing practices, making health care more effective and responsive.

Embracing Technological Innovations

The integration of Artificial Intelligence (AI) and Telehealth has been a game-changer in health care delivery,  spearheaded by nurses . AI enhances diagnostic accuracy and treatment protocols by analyzing vast amounts of patient data. Telehealth, on the other hand, expands access to medical services, especially in remote or underserved areas, ensuring timely and efficient care. These technologies provide nurses with tools that augment their clinical skills and allow for better care across various settings.

As we observe National Nurses Week, it is essential to acknowledge the adaptability and innovative spirit of nurses globally. Despite numerous challenges, they continue to lead in the health care sector. With initiatives like the Commission for Nurse Reimbursement and the adoption of new technologies like AI, nurses are setting new standards in health care delivery. This week, and every week, we reaffirm our commitment to supporting and empowering the nursing profession as it continues to navigate these transformative times. From bedside care to research labs, nurses play an indispensable role in enhancing our health care system.

Inaugural Class of Doctor of Nursing Practice Graduates from Andy and Barbara Gessner College of Nursing/Six Graduates Raise the Bar for Nursing Education and Help Fill Nursing Leadership Shortage

Three years after its inauguration, the Doctor of Nursing Practice (DNP) at the Andy and Barbara Gessner College of Nursing is graduating its first class. The DNP doctorate prepares family nurse practitioners and nurse executives to address the shortages of primary care providers, nursing educators and leaders in a complex health care environment. The program raises the bar for preparation necessary for advanced nursing practice from the master’s degree to the doctorate level.

The six new advanced degree nursing professionals, who were all practicing nurses before entering the program, will continue their careers focused on providing leadership for evidence-based practice including translating research into practice, evaluating evidence, applying research in decision-making, and implementing viable clinical innovations to change practice.

“DNP-prepared nurses are our leaders who will improve healthcare by driving progress,” said Kathryn Tart, founding dean, professor and Humana Endowed Dean’s Chair in Nursing. “The DNP degree supports the national movement toward making it the necessary degree for advanced nursing practice.”

Of the six new graduates, Debra Bayliss and Arlena Davis are elite Albert Schweitzer Fellows, participating in a program that serves and empowers vulnerable people to build healthier communities and live healthier lives. Together Bayliss and Davis worked to bring easily accessible mental health care to Houston’s homeless population.

UH System Regent Durga D. Agrawal Advocate and Pioneer of the DNP Degree

Key to the Doctor of Nursing Practice degree being offered at the Andy and Barbara Gessner College of Nursing – and in the state of Texas – is a member of the UH System Board of Regents, Durga D. Agrawal, president and CEO, Piping Technology & Products, Inc. In 2013 when he was a member of the Texas Higher Education Coordinating Board, Agrawal gave his full support immediately when the DNP program was presented. You could say he was in the right place at the right time.

And Agrawal has definitely been some places.

“As a young boy growing up in a small village in India of 600 people and no electricity, water supply, and without high school, we did not have medical doctors. My family relied on nurses and compounders who provided healthcare services until I reached high school,” said Agrawal of his longtime respect of the nursing profession. “In addition to bringing the DNP program to Texas, in February 2017 Sushila and I had the opportunity to meet with Prime Minister Modi in his office in Delhi. I recommended to him the DNP program in Texas and how it can help provide healthcare to masses in rural India.” The program was successfully implemented by the Nursing Council of India in January at two institutions, Krishna Institute of Medical Sciences and MGM Bombay College of Nursing.

In Texas today approximately 3,000 DNP professionals practice with approximately 5,000 more enrolled in one of many DNP programs across the state.

“Given the limited number of doctors and healthcare professionals, I believe that this program is having a significant impact in the healthcare industry to help alleviate these shortages in Texas,” said Agrawal. “Looking ahead, I envision this program continuing to grow and set a benchmark for nursing education nationwide.”
