CREOG Journal Club

CREOG Journal Club modules provide ob-gyn residency programs with everything they need to lead and present a relevant and interesting paper at the next journal club, or simply to explore a review in more depth on their own.

Elements of the Journal Club Module

Introductory modules include three PowerPoint presentations titled “Journal Club Curriculum Study Designs,” “Journal Club Curriculum: How to Start,” and “Journal Club Curriculum: How to Review the Literature.” They are accompanied by a document titled “User Friendly Guide to a review of the Literature”.

Once a program has provided the introduction, programs may use the "Journal Clubs in a Box." The Journal Club in a Box provides the article and questions as published for Journal Club review in the Green Journal, along with a PowerPoint review of topics by a researcher.

The PowerPoint may focus on statistical designs as appropriate, given the paper one month and Power analysis the next. The User-Friendly Guide to a review of the Literature is intended to be used monthly to stimulate discussion.  These are meant to serve as an additional educational resource for Program Directors as they develop a Journal Club curriculum.

Journal Club in a Box Series

This ongoing series includes an article, list of questions, PowerPoint, and suggested areas of discussion by an epidemiologist to assist programs with running their journal clubs.

Format of an obstetrics and gynecology journal club and four years' experience

Departmental journal clubs have experienced varied degrees of success and longevity. Although a high degree of enthusiasm is difficult to maintain, clear objectives are key factors that encourage active participation. In this report we present a journal club format that has demonstrated popularity and 4 years of longevity. On the basis of this experience, we believe a journal club forum offers medical students and residents the optimal opportunity to learn an approach to critical reading of medical reports. In addition, they gain understanding of experimental design and research protocols and ultimately acquire knowledge of the current medical literature.

Journal clubs that go beyond discussing the “key take-aways,” have global impact.

If you’ve ever sat through a journal club meeting where the article headlines were just re-hashed, you’re not alone. Over time, journal clubs tend to get stale. Pitfalls, like choosing from a limited range of topics, and having only two or three people who are consistently willing to rotate leading discussions, are common.

Yet the need for clinicians to stay abreast of research, technical advancements, and controversial professional issues is a constant which can’t be fulfilled by attending annual meetings.

William C. Dodson, M.D. , of Penn State Hershey Obstetrics and Gynecology , has taken on the challenge of making journal clubs a key part of ongoing, meaningful medical education worldwide. In 2009, as part of Dodson’s longtime leadership role with Obstetrics and Gynecology (i.e. the Green Journal), he and Cathy Spong, M.D., from the National Institutes of Health (NIH), began to create a monthly journal club feature aimed at enhancing the journal club experience ( journals.lww.com/greenjournal ). “Each month, for two articles from the current issue, we develop facilitator questions intended to lead club members to critically weigh the most salient points of each manuscript. It’s like providing a study guide for club facilitators, so even if they aren’t experts on a given topic or technique, they can lead discussion.”

With an obstetrics and gynecology readership worldwide, Dodson’s efforts have a global reach. In fact, at China’s Heilongjiang University of Chinese Medicine in Harbin, Ilene Tsui, a Penn State Hershey medical student, conducted a randomized controlled study to evaluate use of the journal club feature as an English-language teaching tool for medical students and professionals in obstetrics and gynecology. Participants assigned to twenty-four structured journal club meetings showed significant improvements from baseline on a multiple choice examination on related topics. In addition, the study suggests that a journal club significantly and selectively improves both written and verbal medical English proficiency. The findings 1 were presented at the American College of Obstetricians and Gynecologists Annual Clinical Meeting in April.

“This study shows the journal club has significant value as a teaching tool. We designed [the monthly features] to help lower barriers to understanding recent advances in the field, so that clinicians anywhere may find the discussions interesting and helpful to women’s health care around the globe,” said Dodson.

Journal Club: How to Build One and Why

Journal clubs are a longstanding tradition in residency training, dating back to William Osler in 1875. The original goal of the journal club in Osler’s day was to share expensive texts and to review literature as a group. Over time, the goals of journal clubs have evolved to include discussion and review of current literature and development of skills for evaluating medical literature. The ultimate goal of a journal club is to improve patient care by incorporating evidence into practice.

Why are journal clubs important?

In 2004, Alper et al . reported that it would take more than 600 hours per month to stay current with the medical literature. That leaves residents with less than 5 hours a day to eat, sleep, and care for patients if they want to stay current, and it’s simply impossible. Journal clubs offer the opportunity for residents to review the literature and stay current. Furthermore, Lee et al . showed that journal clubs improve residents’ critical appraisal of the literature.

How do you get started?

The first step to starting a journal club is to decide on the initial goal. A good initial goal is to lay the foundation for critical thinking skills using literature that is interesting to residents. An introductory lecture series or primer on study design is a valuable way to start the journal club experience. The goal of the primer is not for each resident to become a statistician, but rather to lay the foundation for understanding basic study designs and the strengths and weaknesses of each design.

The next step is to decide on the time, frequency, and duration of the journal club. This depends on the size of your residency program and leadership support. Our journal club at Johns Hopkins is scheduled monthly during the lunch hour instead of a noon conference lecture. It is essential to pick a time when most residents in your program will be available to attend and a frequency that is sustainable.

How do you get residents to come?

Generally, if you feed them, they will come. In a cross-sectional analysis of journal clubs in U.S. internal medicine residencies, Sidorov found that providing food was associated with long-lasting journal clubs. Factors associated with higher resident attendance were fewer house staff, mandatory attendance, formal teaching, and an independent journal club (separate from faculty journal clubs).

The design or format of your journal club is also a key factor for attendance. Not all residents will have time during each rotation to read the assigned article, but you want to encourage these residents to attend nonetheless. One way to engage all residents is to assign one or two residents to lead each journal club, with the goal of assigning every resident at least one journal club during the year. If possible, pick residents who are on lighter rotations, so they have more time outside of clinical duties to dissect the article. To enhance engagement, allow the assigned residents to pick an article on a topic that they find interesting.

Faculty leadership should collaborate with residents on article selection and dissection and preparation of the presentation. Start each journal club with a 10- to 20-minute presentation by the assigned residents to describe the article (as detailed below) to help residents who did not have time to read the article to participate.

What are the nuts and bolts of a journal club?

To prepare a successful journal club presentation, it helps for the structure of the presentation to mirror the structure of the article as follows:

Background: Start by briefly describing the background of the study, prior literature, and the question the paper was intended to address.

Methods: Review the paper’s methods, emphasizing the study design, analysis, and other key points that address the validity and generalizability of the results (e.g., participant selection, treatment of potential confounders, and other issues that are specific to each study design).

Results: Discuss the results, focusing on the paper’s tables and figures.

Discussion: Restate the research question, summarize the key findings, and focus on factors that can affect the validity of the findings. What are potential biases, confounders, and other issues that affect the validity or generalizability of the findings to clinical practice? The study results should also be discussed in the context of prior literature and current clinical practice. Addressing the questions that remain unanswered and potential next steps can also be useful.

Faculty participation: At our institution, the faculty sponsor meets with the assigned residents to address their questions about the paper and guide the development of the presentation, ensuring that the key points are addressed. Faculty sponsors also attend the journal club to answer questions, emphasize key elements of the paper, and facilitate the open discussion after the resident’s presentation.

How do you measure impact?

One way to evaluate your journal club is to assess the evidence-based practice skills of the residents before and after the implementation of the journal club with a tool such as the Berlin questionnaire — a validated 15-question survey that assesses evidence-based practice skills. You can also conduct a resident satisfaction survey to evaluate the residents’ perception of the implementation of the journal club and areas for improvement. Finally, you can develop a rubric for evaluation of the resident presenters in each journal club session, and allow faculty to provide feedback on critical assessment of the literature and presentation skills.

Journal clubs are a great tradition in medical training and continue to be a valued educational resource. Set your goal. Consider starting with a primer on study design. Engage and empower residents to be part of the journal club. Enlist faculty involvement for guidance and mentorship. Measure the impact.

Why Develop a Journal club in the Department of Obstetrics and Gynecology? • Keep current • Promote implementation of new knowledge in practice • Promote better understanding of the research process • Improve patient outcomes • Helpful as KWLA is implementing evidence-based practice and is working toward “Magnet Status”

Background: • Initiated in July 2005 • More developed in 2006 • Running smoothly in 2007

Making Journal Club a Reality • One RN (Lynn Krimsky) and one LVN (Althea smith) were identified to organize the journal club. The leader and alternate with one person with overall responsibility for the project. • Both were interested and committed to the concept and willing to organize it.

Goal • To identify clinical/non clinical problem and find articles relating to that problem.

Process • The lead will find a book or an article. • Copies of chapters of the book or the article will be distributed to staff. • A discussion will take place followed by an interactive session encouraging everyone’s involvement. • The group is mixed (inexperienced and experienced). • The experienced staff will teach less experienced or non experienced staff about how to look for highlights and outcome of the articles.

Setting a Time and Place • Alternate months Wednesday afternoon • MDs ET • Location Varies. Currently a serious barrier • Lunch usually available through pharmaceutical companies according to KP policies.

Selection of Article(s) • A topic of general interest to the staff • Everyone can suggest an article of their choice based on EBP, clinical or client related. • KWLA librarian is always available and willing to locate appropriate articles.

Examples of Journal Club Activities: • Completed the book “ If Disney Ran Your Hospital” by Fred Lee. • HPV vaccine according to the new guidelines.

Future Plans: • Mentor staff on how to Develop/Select forms to make a Journal Club Article Summary. • Take more responsibility and get more involved with selection of articles of interest.

Explore all metrics

The available data on the fluorite potential of the Khabarovsk krai, the Jewish Autonomous Region and Amur oblast have been synthesized. Fluorite deposits and occurrences were ascribed to the rare-earth–fluorite, beryllium–fluorite, fluorite–tin-ore, base-metal–fluorite, and fluorite mineralization types. Fluorite also occurs in the ore and phosphorite deposits of the fluorite-bearing mineralization type. The features of their localization in different tectono-stratigraphic areas of the region are shown. Fluorite-bearing districts were identified and their economic potential was assessed. Most promising fluorite occurrences are located along the periphery of the Siberian platform and in the southern part of the Bureya massif. Inferred fluorite resources were calculated and the prospects for their industrial development were estimated.
