  • Definition/Introduction

A personal health record (PHR) refers to the collection of an individual's medical documentation maintained by the individual themselves, or a caregiver, in cases where patients are unable to do so themselves. This personal information includes details such as:

  • The patient's medical history
  • Applicable diagnoses
  • Historical and ongoing medications, including over-the-counter and alternative treatments
  • Past medical and surgical interventions
  • Immunization status
  • Allergies and other relevant medical conditions that can impact the delivery of emergency care (e.g., Type 1 diabetes, etc.)
  • Whom to contact in the event of an emergency
  • Insurance information
  • Contact information for the patient's regular health providers

Any other information the patient feels is pertinent may also be included. This contrasts with electronic medical records and electronic health records (EMR and EHR), which are usually maintained by the treating physician or hospital to provide medical care and for billing purposes.

A PHR may be either physical or, as has become increasingly common moving forward, electronic. It includes all self-reported and self-recorded health data, including health issues and treatments, records of vital signs and activity recorded with personal devices including smartphones and smartwatches, nutritional data such as diet composition and calorie intake, etc. Several commercial applications are available that allow an individual to maintain a PHR, and some also allow integration of this data with the individual's EMR/HER, allowing them to take better charge of their own health.

The goal of a PHR is to allow the patient to keep their health data on hand for ready access for both themselves and anyone involved in their care while maintaining the privacy and security of this data. A PHR can hence assist in providing tailored medical care. [1]

  • Objective data points for vital signs, nutrition, physical activity, and disease course, which can allow individuals to follow their health in real-time and quantify the amount of effort and change that has occurred over time.
  • Gamification of the health data collection and reporting process can serve as a source of motivation in achieving health goals such as weight loss targets, motivational videos, calorie registers, etc.
  • Greater clarity of medications to be taken and better compliance with them, plus an improved assessment of the same
  • Identification of successes and failures in the delivery of care and the underlying reasons for them
  • Rapid emergency response with the availability of pertinent health data in the absence of a caregiver or bystanders

  • PHRs may be developed as a one-size-fits-all approach that may sometimes fail to take into account individual variations.
  • Certain advertised benefits of commercial PHR applications may not be supported by concrete evidence and can prove misleading to consumers.
  • Anxiety may be provoked by an urge to record all personal data.
  • Individuals may find patterns where none exist, leading to greater false positives and higher healthcare utilization.
  • There may be bias in self-reporting health data.
  • Lapses in security and confidentiality are a major concern, especially with cloud-based solutions.
  • Misuse of health data by entities with commercial interests.
  • Clinical Significance

PHRs are seeing widespread adoption in today's digital age, and physicians must adapt to this new data source. Interpreted correctly, a PHR can provide valuable data points to assess the clinical course before the presentation and provide a level of fidelity that could not be achieved by traditional patient interviews alone. This concept represents a crucial component of shared decision-making by the patient and their physician. [3]

Used correctly, PHRs can improve patient adherence to follow-up, allow patients to better monitor therapeutic goals such as blood pressure or blood glucose thresholds, allow recognition of improvement or worsening of control of existing medical conditions, improve compliance with medication regimens, especially when these regimens are complex – all of which culminate in the achievement of superior management of medical issues.

However, critical analysis is necessary, as many of these devices and applications use one-size-fits-all algorithms that may result in false positives and false negatives in interpreting medical conditions. Patients must be counseled about interpreting and acting on health data in close cooperation with their healthcare provider in order to avoid unwarranted anxiety or concerns over minor normal variations that are interpreted as something more sinister. This is especially of concern in a setting where PHRs are interlinked with EMR/EHR systems that provide patients with real-time test results that are yet to be interpreted and clinically correlated by a clinician. Moreover, PHRs may also contribute to a state of "information overload" "whereby extraneous data in a PHR becomes noise that can garble the signal of pertinent health information in the healthcare setting. In this setting, the upcoming use of artificial intelligence (AI) in the processing of patient data may serve as a useful adjunct in highlighting true abnormalities and help determine action signals that necessitate healthcare intervention. This can allow healthcare providers to look past the vast mountain of data and detect significant findings from the repository of information within an iindividual'sPHR.

  • Nursing, Allied Health, and Interprofessional Team Interventions

A caveat of PHRs is the importance of presenting information to the layperson in a manner that simplifies the concepts while retaining the accuracy and veracity of facts. This may depend heavily on the involvement of sensitized healthcare professionals from various disciplines for the development of PHR platforms, as they would be best poised to recognize cognitive errors of reporting and interpretation that may affect the use of PHRs by the public at large. All of this data must be handled within the confines of maintaining patient privacy while sharing data as necessary for medical purposes. [5]

Allied health providers can significantly contribute to developing individual PHRs by counseling and educating persons on the benefits and shortcomings of various platforms, the relevance of data recorded to their clinical problems, and filtering this data to make interpretation more straightforward for the clinicians and other healthcare personnel (e.g., nurses, therapists, techs, pharmacists) involved in the patient's care. A multi-disciplinary approach to creating PHRs, constant study and improvement of PHR design by a team of pan-professional individuals, and post-marketing surveillance of usage data, successes, and pitfalls are all critical components of developing this modality as a fresh addition to the healthcare armamentarium.

The personal statement gives you the opportunity to present a compelling snapshot of who you are and perhaps why you want to be a doctor. Use your personal statement to say what others can’t. The personal statement can be a tricky genre to master. On the one hand, you want to give the admissions committee a sense of your personality and who you are. On the other hand, you must sound focused and professional, which sounds like it might impede your ability to capture your personality.

But this does not have to be the case. What you need to do is figure out how to say what drives you to want to become a healthcare professional in as specific a way as possible. The more specific you can be, the more the admissions committee will feel as if they have a sense of who you are.

You don’t need gimmicks, jokes, artificial drama, or hyperbole to express who you are or why you would make a good medical student or doctor. All you need are carefully selected details that you can craft into a unique and compelling story that conveys a sense of purpose and motivation.

What Makes a Good Personal Statement?

  • There is no exact template for an effective personal statement. Often, however, strong personal statements combine a concise description of a personal experience with reflection on how this experience either led the writer to pursue medicine or indicates the writer’s character or commitment.
  • Good personal statements often have a strong sense of narrative. This does not mean that they read like short stories, though they can relate a few scenes or anecdotes from your life. They have a strong sense of narrative, rather, in how they convey the writer’s sense of dedication to medicine. Strong personal statements often give readers an idea of how applicants see their experiences as leading to the decision to pursue medicine.

How to Get Started

The personal statement is an exercise in self-reflection. Questions to consider:

  • Who are you?  I am driven to… I have learned to… I believe…
  • What are your most passionate interests or concerns?  What problem(s) most occupy your thinking and your efforts?
  • How did you develop those interests?  (Not just the story, but what drives you.)
  • What errors or regrets have taught you something important about yourself?
  • When does time disappear for you?  What does this tell you about your passions, your values?
  • What ideas, books, courses, events have had a profound impact on you?  How so?
  • To what extent do your current commitments reflect your most strongly held values?
  • When have you changed?  Consider yourself before and after; what does this change mean?
  • How do your interests and who you are relate to your goals in medical school and as a doctor?

Start a “shoebox”; a place to keep random notes for your personal statement; be ready to write at any time. Review these items occasionally; let them tell you more about what you want your personal statement to say. Start writing drafts, experiments; you will know when a paragraph begins to gel.

A Suggested Writing Process

Everyone writes differently, so these are potential strategies rather than rules.

  • Make a list of some of your most defining experiences – extracurricular activities, specific classes, volunteer work, research, hobbies, etc. Try not to include overly personal experiences (breakups, trouble with parents, illnesses in the family, and so on). It’s difficult to write about such things without being sentimental or cliché. You want experiences in which you did something and had to make a choice.
  • From this list, try to select an experience that particularly demonstrates your intellectual curiosity, your dedication to service, your composure under pressure, your leadership ability, or any other personal trait that you think is particularly relevant to your case that you would make a good doctor or medical student.
  • Start writing a draft based on this experience. You want to be specific, but don’t get bogged down with an abundance of anecdotes or minutiae. Try to use your draft to craft a succinct story that demonstrates your character and your motivations.
  • Set the draft aside for some time (a number of days or weeks), and then revisit it with fresh eyes. Be as honest with yourself as you can be: What works in this draft? What doesn’t work? What sounds cliché or unspecific? Would a reader who doesn’t know me at all get a sense of my personal character and dedication?
  • Revise, revise, revise: tighten the structure, add new things to make your point clearer, take away sentences or sections that now seem unnecessary, use the active voice as much as possible, and anything else that needs to be done. If what you have just doesn’t seem to be coming together, do not be afraid to start over.
  • Solicit feedback from a couple of trusted readers and revise again based on the suggestions that you find most useful. Don’t solicit feedback from too many people though – too many responses can be overwhelming.
  • Edit your work for grammatical mistakes, typos, clumsy repetitions, and so on. Make your prose impeccable before you submit your statement. Asking help from other readers can be especially helpful with editing, as sometimes it gets difficult to read your work with fresh eyes.

Things to Do

  • Use the experience that you describe to tell a story of personal progress, particularly progress towards your commitment to medicine.
  • Write with active verbs as much as possible.
  • Strive for concision.
  • Sound humble but also confident.

Things Not to Do – Common Pitfalls

  • Don’t talk in hyperbolic terms about how passionate you are. Everyone applying to medical school can say they are passionate. Instead, show your readers something you have done that indicates your passion.
  • Don’t adopt an overly confessional or sentimental tone. You need to sound professional.
  • Don’t treat the personal statement like a piece of creative writing.
  • Don’t put your resume in narrative form.
  • Don’t use jargon, abbreviations, slang, etc.
  • Don’t use too many qualifiers: very, quite, rather, really, interesting…
  • Don’t write in overly flowery language that you would normally never use.
  • Don’t include famous quotations. If you must quote, use something that shows significant knowledge.
  • Don’t write about yourself in an overly glorifying or overly self-effacing manner.

What to Remember

  • They are read by non-specialists, so write for an intelligent non-medical audience.
  • Actions sometimes speaks louder than words so give examples of experiences rather than describing them.
  • All information must be accurate – don’t pad, but don’t be falsely modest either.
  • The personal statement, in part, serves as a test of your communication skills.  How well you write it is as important as the content.

Factors Influencing Good Health in the Individual

Education is the first factor that influences the good health of a person. According to Murimi et al. (2017, p. 143), through excellent education levels, individuals become aware of the diverse approaches and styles that they can embrace to increase their fitness levels. For instance, through education, one comprehends the different foods to ensure body wellbeing. In other words, education imparts the necessary skills which a person can adapt to enhance their health.

Housing is the second factor that dictates the health of an individual. Understandably, the fitness a person can embrace is conditioned by a place where they live (Familydoctor.org., 2020, p. 1). Individuals who reside in densely populated areas have high chances of developing diseases such as obesity because they have less room for exercise and poor ventilation. Therefore, it is worth noting that the housing system determines the healthy lifestyle of a person.

Income is the third factor which influences good health in the individual. Arguably, the amount of money a person makes has a critical effect on people (Familydoctor.org., 2020, p. 1). People with high income and economic stability tend to be healthier. For instance, good financial status allows a person to buy a gym system which assures them of having good health. People who have less income tend to consider one variety of cheap foods, leading to malnutrition due to one nourishment component being high in the body. Thus, a person’s income highly determines their nutritional status and lifestyle, which they can embrace.

How Health can be Improved

Health among individuals can be improved when one decides to quit eating and drinking sugary foods and considering eateries which sell nutritive foods. Individuals should abstain from eating chuck foods and drinking soda, fruit juices, granola, chocolate milk, and yogurt (Simkin et al., 2016, p. 81). As a result, a person will lead a healthy lifestyle, which has zero chances of developing other chronic conditions, including obesity and arthritis.

Another way that people’s health can be improved is by treating themselves with new and smaller dishes. Individuals should try to eat fresh foods, ceasing from taking the same nutrients throughout the period (Jarvis, 2018, p. 20). People should discover new foods as a way of balancing their diet and nutrients intake. The body stores fats in different foods, putting individuals at risk of developing diseases like obesity.

People can embrace the other approach of ensuring that their health is improved by rethinking their house cleaning routines, including bedding and the surrounding environment. Despite mopping being a cleanliness procedure, it equally acts as a form of exercise, hence keeping individuals fit (Jarvis, 2018, p. 22). Moreover, cleanliness ensures that one reduces germs in the house by upholding this daily routine. For instance, the flu virus is known to live on hard surfaces for up to forty-eight hours (Wong and Lee, 2019, p. 1). Thus, wiping surfaces can help reduce the ordeals of infection. Regular cleaning ensures that individuals do not have asthma or other allergies. Therefore, embracing an effective daily routine of cleanliness is vital as it reduces people’s chances of deteriorating their health.

Current Diet and Lifestyle

I have no specific food that I eat for breakfast, lunch, and supper for different days. Mostly, I eat sweet bread every morning. Sometimes, I do not take breakfast during weekdays. At lunch, I eat different fast foods, more so, snacks. Fewer vegetables and fruits per week, concentrating more on meat, pizza, and fish. Less whole grains. Moreover, most of the grains which I eat are refined. I drink yogurt at least once per day during afternoons. Lastly, there are no specific foods for breakfast, lunch, and supper.

Personal Exercise Pattern and Exposure to stress

I ride a bicycle every morning and evening for at least one kilometer and perform twenty sit-ups every morning and evening. In addition, I engage in swimming at least once per week. The high workplace sounds are one of the factors that expose me to stress. Long working hours equally put me under pressure, hence developing stress. A colossal workload equally increases my stress levels.

Identification and Understanding of Health Issues

Ideal menu plan, lifestyle improvements.

I will be sleeping early. Eating a regular-well balanced diet. Maintain a healthy body weight. Engaging in regular physical activities. Increase the water consumption rate. Track my progress.

My improvements were useful, and I adequately achieved my expectations. I expected to lose one and a half pounds of weight, but I lost two pounds, which is a tremendous achievement. I intend to adopt the same healthy lifestyle to keep my body fit and diseases-free. Therefore, people should embrace a healthy diet and engage in physical activities to refresh their minds, hence reducing the daily stress levels and equally maintaining an excellent physique.

Conclusively, engaging in regular activities and considering a nutritive diet should be the ultimate goal of every person. Individuals should consider an effective health promotion plan to adopt good health. Regular exercises keep the body fit, enhancing both aerobic and anaerobic respiration. A better and nutritive foods give the body the required strength and immunity to fight against pathogens. There is a need for people to consider the foods which they purchase and equally engage in physical activities.

‘Total Disgrace’: Anger, Frustration as Mass Heating Failures Across Russia Leave Thousands in the Cold

P ODOLSK, Moscow region – Residents throughout Russia affected by unprecedented winter heating outages in recent days have expressed their frustration and urged local authorities to restore heating in their homes.

In Podolsk, a town some 30 kilometers south of the capital Moscow, at least 149,000 residents — nearly half of its population — were left without heating when a heating main burst at a nearby private ammunition plant.

“It’s a total disgrace. There is no heating and no hot water. We have to sleep in sleeping bags,” Yuri, a local resident, told The Moscow Times.

“I have no words to describe how bad the situation is," said Yuri, who declined to provide his surname. "We have had no heating for almost six days."

Heating issues have affected residents in the Moscow region, where temperatures have plunged to as low as minus 20 degrees Celsius in the past week, as well as people in the Far East Primorye region , the cities of Moscow and St. Petersburg , Penza , the southern Voronezh and Volgograd regions and more.

In the Tver region, a group of residents filmed an appeal to President Vladimir Putin, saying that they “are freezing from the cold” in the village of Novozavidovsky.

“We're literally being killed by the cold,” a woman in the video said, adding that they have been sending requests to local authorities since September after their houses were connected to a boiler room whose power was reportedly insufficient.

“This is some kind of torture and extermination of the population 100 kilometers from Moscow,” she added.

Residents of the Moscow region town of Elektrostal lit a fire in the street to draw the authorities’ attention to the heating problem.

“It’s impossible to stay in our houses. We're freezing!” a group of women in the video said.

Suffering from subzero temperatures, residents are placing the blame on local authorities and utility services for failing to take necessary precautions and not taking action to resolve the situation.

“We are sending complaints everywhere but no one listens to us. We have portable heaters working in every room, but the temperature inside is still 10 degrees Celsius,” Yelena from Podolsk said.

“There is a clinic and a hospital, as well as kindergartens, where there is no heating. And we have no answers, no assistance, no explanation,” Yelena added.

Podolsk authorities opened temporary heating centers and declared a state of emergency.

Local authorities linked the heating problems to the fact that the town is heated by a boiler plant owned by the Klimovsk Specialized Ammunition Plant, a private ammunition factory and one of the largest weapon cartridge production enterprises in the country.

“The facility is under tight security conditions, which limits our ability to oversee winter preparations,” the Moscow region’s Vice Governor Yevgeny Khromushin said last week. “We were unaware of the problem for nearly a day.”

An unidentified Moscow region official and two senior executives at the plant were arrested on suspicion of providing unsafe services, Russia’s Investigative Committee, which probes major crimes, said in a statement Tuesday.

Investigators said that Podolsk’s deputy mayor was accused of misusing authority by issuing a readiness certificate for the boiler house at the plant.

In the neighboring Tver region, the authorities opened a criminal case over the laundering of over 84 million rubles ($938,993) in heating bills paid by residents, the Astra Telegram channel reported this week, citing unidentified sources. According to investigators, the heads of the local water intake and boiler house misappropriated the heating payments for personal use.

Reacting to the heating failures, Putin on Tuesday asked Emergency Situations Minister Alexander Kurenkov to provide heat and electricity to the affected residents.

The outages appear to be the latest effect of several decades of crumbling infrastructure in Russia which have been linked to endemic corruption and mismanagement.

The overall decay of Russia's municipal infrastructure surpassed 70% in 2022, the pro-Kremlin newspaper Izvestia reported .

According to Sergei Pakhomov, head of the State Duma’s Construction, Housing and Utilities Committee, water pipes that were 90 years old or even older were still in use as recently as two years ago in some cases.

Housing, utilities and communal services are a common source of problems for Russians during the winter.

In St. Petersburg, residents regularly complain about extensive ice coverage on city streets and sidewalks, with many people ending up in the hospital over the years due to slipping and falling accidents.

In the Siberian republic of Khakassia, two villages were left without electricity last month due to apparent issues with outdated communication systems.

In the winter of 2020, five people in the Perm region were killed after a pipe burst.

When asked about the latest heating outages, Kremlin spokesman Dmitry Peskov acknowledged the problems and linked them to poor municipal infrastructure, saying that people “had to endure a lot of inconvenience in the cold and without electricity.”

"Despite all the titanic efforts to update all housing and communal services systems, there's still a certain part that remains considerably deteriorated. These programs will continue, but it is impossible to update all pipes and all housing and communal services systems in 10-15 years,” Peskov said.

As for now, residents affected by heating issues appear to lack optimism that the problems will be solved efficiently.

"It's been a week since we've had heating, and the temperature in my apartment is around 11 degrees Celsius,” Podolsk resident Lidiya told The Moscow Times.

“Unfortunately, no one knows when it will be repaired,” she added.
