Defining the Question: Foreground & Background Questions

In order to most appropriately choose an information resource and craft a search strategy, it is necessary to consider what  kind  of question you are asking: a specific, narrow "foreground" question, or a broader background question that will help give context to your research?

Foreground Questions

A "foreground" question in health research is one that is relatively specific, and is usually best addressed by locating primary research evidence. 

Using a structured question framework can help you clearly define the concepts or variables that make up the specific research question. 

 Across most frameworks, you’ll often be considering:

  • a who (who was studied - a population or sample)
  • a what (what was done or examined - an intervention, an exposure, a policy, a program, a phenomenon)
  • a how ([how] did the [what] affect the [who] - an outcome, an effect). 

PICO is the most common framework for developing a clinical research question, but multiple question frameworks exist.

PICO (Problem/Population, Intervention, Comparison, Outcome)

Appropriate for : clinical questions, often addressing the effect of an intervention/therapy/treatment

Example : For adolescents with type II diabetes (P) does the use of telehealth consultations (I) compared to in-person consultations  (C) improve blood sugar control  (O)?

Framing Different Types of Clinical Questions with PICO

Different types of clinical questions are suited to different syntaxes and phrasings, but all will clearly define the PICO elements.  The definitions and frames below may be helpful for organizing your question:

Intervention/Therapy

Questions addressing how a clinical issue, illness, or disability is treated.

"In__________________(P), how does__________________(I) compared to_________________(C) affect______________(O)?"

Questions that address the causes or origin of disease, the factors which produce or predispose toward a certain disease or disorder.

"Are_________________(P), who have_________________(I) compared with those without_________________(C) at_________________risk for/of_________________(O) over_________________(T)?" 

Questions addressing the act or process of identifying or determining the nature and cause of a disease or injury through evaluation.

Prognosis/Prediction:

Questions addressing the prediction of the course of a disease.

Beyond PICO: Other Types of Question Frameworks

PICO is a useful framework for clinical research questions, but may not be appropriate for all kinds of reviews.  Also consider:

PEO (Population, Exposure, Outcome)

Appropriate for : describing association between particular exposures/risk factors and outcomes

Example : How do  preparation programs (E) influence the development of teaching competence  (O) among novice nurse educators  (P)?

SPIDER (Sample, Phenomenon of Interest, Design, Evaluation, Research Type)

Appropriate for : questions of experience or perspectives (questions that may be addressed by qualitative or mixed methods research)

Example : What are the experiences and perspectives (E) of  undergraduate nursing students  (S)  in clinical placements within prison healthcare settings (PI)?

SPICE (Setting, Perspective, Intervention/phenomenon of Interest, Comparison, Evaluation)

Appropriate for : evaluating the outcomes of a service, project, or intervention

Example : What are the impacts and best practices for workplace (S) transition support programs (I) for the retention (E) of newly-hired, new graduate nurses (P)?

PCC (Problem/population, Concept, Context)

Appropriate for : broader (scoping) questions

Example : How do nursing schools  (Context) teach, measure, and maintain nursing students ' (P)  technological literacy  (Concept))throughout their educational programs?

Background Questions

To craft a strong and reasonable foreground research question, it is important to have a firm understanding of the concepts of interest.  As such, it is often necessary to ask background questions, which ask for more general, foundational knowledge about a disorder, disease, patient population, policy issue, etc. 

For example, consider the PICO question outlined above:

"For adolescents with type II diabetes does the use of telehealth consultations compared to in-person consultations  improve blood sugar control ?

To best make sense of the literature that might address this PICO question, you would also need a deep understanding of background questions like:

  • What are the unique barriers or challenges related to blood sugar management in adolescents with TII diabetes?
  • What are the measures of effective blood sugar control?
  • What kinds of interventions would fall under the umbrella of 'telehealth'?
  • What are the qualitative differences in patient experience in telehealth versus in-person interactions with healthcare providers?

Needing a suitable infrastructure solution for moving water to future drill pad locations and impoundments, a natural gas client looked to leverage RETTEW’s extensive experience with water systems and innovative thinking with respect to power generation.

Electrical power was unavailable at the station’s rural location. RETTEW identified on-site power generation as the most economical and environmentally responsible approach to satisfy the power requirements. The booster pump station features an 800-kilowatt Capstone generator on a 1-megawatt frame running off of 1,250 Btu-rich gas produced from nearby wells. The generator provides electrical power for the booster pump station consisting of two 250-horsepower pumps. This solution avoided the cost of electrical extension from the grid and produces power that is significantly cheaper than grid-based electric.

ECE 2024 Capstone Design Fair highlights, part 1: Improving the battery system for neural implants

In April, ECE’s fourth-year undergraduate students showcased their team-based projects at the 2024 Capstone Design Fair, held over three nights in the Myhal Centre.

Capstone is a yearlong project design course (ECE496) that asks the students to develop an initial concept into a working prototype. This year, coordinator Professor  Bruno Korst  says the department incorporated industry reporting processes to better mirror what students can expect in the workplace.

“I conducted multiple interviews with former students and hired two project administrators who currently work in industry. They all said written reports have given way to brief meetings on deliverables — what’s been done, what’s up next — and then everyone moves on.

“So we simplified the documentation for the students and had stand-up meetings over the two semesters. It set an industry pace, which meant that the students had to pick it up. You can see it in the results this year, which to my eye are more focused.”

“Developing a working engineering prototype requires a combination of teamwork, dedication and the application of a variety of ECE skills acquired from our classrooms and labs,” says ECE Chair Professor  Deepa Kundur .   “Each year, it’s inspiring to see how much the students have learned and their pride in their projects. The design fair is one of the highlights in our academic year.”

This Capstone team project highlight is part 1 of 3.

Next up, part 2: guiding consumers with an app that accurately shows prescriptive lens in frames, part 1: wireless power transfer to multiple implantable neurological devices.

Deep-brain stimulation is a therapeutic procedure that can help with the symptoms of Parkinson’s disease and other neurological disorders. It involves implants in the brain, which deliver pulses of electric current to specific areas.

The team of Selena Liu , Aurora Nowicki and Kimberley Orna (all Year 4 ElecE) developed a way to improve the battery system that powers those implants. Their Capstone project was mentored by Mohammad Abdolrazzaghi (ECE PhD candidate), a member of ECE professor Roman Genov ’s lab, and it also received support from ECE professor George Eleftheriades . The project received the John W. Senders Award for Imaginative Design.

“The state of the technology right now requires patients to have a battery pack implanted beneath their collarbone, with implanted electrodes and wires that run through their neck,” says Nowicki. “They need to get this battery replaced every five or seven years. It’s very invasive.”

But what if one could power the implants wirelessly?

“When people think of wireless charging, they often picture the inductive power transfer of an electric toothbrush or phone,” says Nowicki.

“But that only works when the distance is less than five centimetres,” Liu says, “and it’s not precise enough for our purposes. We used what’s called far-field or power beaming, which is how your cell phone communicates with a cell tower, in the radio frequency.”

The team’s solution incorporated an antenna array worn around the patient’s head.

“We send power that’s specifically conditioned — by that, I mean we shift the phase and change the amplitude — to six different antennas on a headband,” says Orna. “That enables the antennas to beam power into the head at a very precise location.”

The power is controlled by a graphical user interface (GUI) they built using Python. It offers two modes of operation to the clinician: automatic and manual. Automatic mode adjusts the six different waveforms into an interference pattern that targets the x-y-z location of the implant as inputted in the GUI. Manual mode gives clinicians some therapeutic options by allowing them to manipulate the power or temporal delay of each waveform.

“A real advantage for our solution versus electrodes, which don’t move once they’re implanted, is when the patient has multiple devices in the brain. The clinician would be able to control an exact power transfer to the x-y-z location of each one,” says Liu.

To demo their proof of concept, they successfully lit up LED lights as stand-in implants in a model head filled with solution that simulates cerebrospinal fluid.

The team miniaturized the integrated system — the microcontroller, the radio frequency synthesizer, signal generator, and amplifier — into a printed circuit board that fit a 3D-printed housing case about the size of a phone. The case is small enough to either be strapped to the headband or worn as a fanny pack.

Even though Capstone is now over, their ambitions for the project are not.

“Our hope to get this PCB manufactured so that we can test at the frequency of 950 MHz, which is a frequency that is safe for sending power into the brain, at high efficiency, that minimizes any heat losses,” says Liu.

Commencement

Washington State University honored more than 280 graduating students this year, including graduates from the fall 2023, and spring and summer 2024 terms. The ceremony included a commencement address by Frank Armijo and recognition of an honorary doctorate degree for Patsy Whitefoot.

Multicultural Graduation

Ahead of commencement, graduating students had an opportunity to attend the Multicultural Graduation Celebration on campus with their families and friends, hosted by the Rainbow Alliance, Dreamers Club, and Black Student Union. Order of the Engineer Ceremony, Pledge of the Computing Professional, and SEAS Graduation Celebration

The Order of the Engineer and Pledge of the Computing Professional ceremonies were initiated to foster a spirit of pride and responsibility in the engineering and computing professions, bridging the gap between training and experience, and representing visible symbols that identify engineers and computer scientists.

Graduating engineering and computer science students and professionals from the Tri-Cities community gathered to participate in the ring and pledge ceremonies. The Engineer’s ring is made of stainless-steel and worn on the fifth finger of the working hand by engineers who have accepted the Obligation of an Engineer in a ring ceremony. Computing professionals receive a lapel pin written in binary code spelling out “honor”, signifying their pledge to honor their profession.

Following the ceremonies, the School of Engineering and Applied Sciences held a graduation celebration where four seniors were recognized for their achievements.

2024 Capstone Expo

For the past year, seniors in Virginia Commonwealth University’s  College of Engineering  have been working to make the world a better place for humankind.

Whether assembling a wheelchair-lifting system, constructing a hybrid bike for veterans with limited mobility or exploring safe human-industrial robot collaboration, the senior class unveiled their research projects on April 26 at the college’s annual  Capstone Design Expo .

A signature event of the college, the expo represents the culmination of the graduating class’s education and showcases their innovative research and design prototypes from their yearlong senior Capstone Design course. The required class teaches leadership, teamwork and design skills.

Faced with challenges when developing prototypes, students gain valuable insights into the intricacies of designing innovative solutions. For many, the process highlights the importance of flexibility and adaptability in problem-solving.

The long-term nature of the projects sets this course apart from the rest of the curriculum. Students discovered the challenges of turning their initial ideas into a functioning product – and that building a solution is far tougher than designing it.

Here are summaries of the 2024 Engineering Capstone Design Expo winners.

BIOMEDICAL ENGINEERING

Mini Sensors to Track Movement of Preterm Babies  (BE 24-103)

Ashlyn Jones, Ireleigh O’Malley, Sarah Johnson, Adriana Bautista and Ishika Singh (advisor: Thea Pepperl)

“Premies” — babies born prematurely — are more susceptible to health challenges and require constant monitoring. This project tracks newborns’ movements using sensors to identify any potential issues, such as neurological complications or infections. It aids in early detection, ensuring these babies receive care for optimal health and development.

“We faced challenges when prototyping and choosing the most suitable wearable design to meet our sponsor’s specifications, since we needed to ensure the sensor was protected, the device as a whole could be sterilized, and maximized the baby’s comfort,” Bautista said.

CHEMICAL AND LIFE SCIENCE ENGINEERING

Subcritical Hydrothermal Catalytic Degradation of PFAS  (CLSE 24-204)

Abdullah Alqallaf, Jazzamine Threatt, Holly Sheppard and Anthony Anzelmo (advisor: Frank Gupton)

Poly- and perfluoroalkyl substances — commonly referred to as PFAS, or “forever chemicals” — take nearly forever to breakdown and, since they contaminate drinking water, can build up in the body over time. Forever chemicals are used to create common household products such as cleaning solutions, nonstick cookware and personal care items, and demand to find ways to break down PFAS is steadily increasing every year.

This project developed a continuous method of degrading PFAS found in water. Implementing two separate methods, the students removed up to 76.5% and 98.8% total PFAS, respectively, setting the stage for future experiments to see how adsorption affects the final concentration of PFAS in the treated solution.

COMPUTER SCIENCE

Sentiment Voice: Integrating Emotion AI and VR Performance in Arts  (CS 24-319)

Can artificial intelligence “read” people’s emotions when they’re inside virtual reality? The students developed a system using the Unity Platform that tracks a user’s emotional changes.

Based on what users say and how their facial expressions change, the environment — in this case, a computer-generated Tokyo — changes around them, showing how someone’s emotions can project into the environment around them.

Based on research by VCU’s Semi Ryu, Ph.D., the project offers potential in multiple settings, including treating patients with PTSD or helping users process trauma and stressful situations.

ELECTRICAL AND COMPUTER ENGINEERING

Microgrid Model and Design  (ECE 24-405)

Dominion Energy sponsored this project, in which the students designed an energy management system — essentially, a tiny computer controller that functions as a brain — to forecast electric demand. The “brain” helps decide how to use electricity most efficiently and at the best price by determining where the electricity should come from — the sun, the wind or a diesel generator.

As the world continues to shift toward renewable energy, the students wanted to think small (as in, microgrids) as well as big.

“Since microgrids rely on small, local generation, this makes them optimal for deploying renewable energy resources,” Holt said. “Implementing renewable energy throughout communities using wind turbines, rooftop solar panels and even solar carports reduces the need for large power farms that consume large amounts of land. Having power available locally also reduces stress on long-distance transmission networks and increases reliability in times of high grid instability. Ultimately, microgrids are an efficient way of implementing a sustainable, renewably powered future.”

MECHANICAL AND NUCLEAR ENGINEERING

Fabrication of Magnetic Filaments for Fused Deposition Modeling Technology  (MNE 24-503)

Thomas Pierce, Micaiah Akyeampong, Vincent Mazzochette and Kamau Bey (adviser: Radhika Barua)

Additive manufacturing — building something by adding material one layer at a time — reduces reliance on traditional supply chains by enabling on-demand production of spare parts and components.

One of the most common methods of additive manufacturing is fused deposition modeling, better known as 3D printing, which extrudes a plastic filament in a series of layers to create the 3D object.

This team set out to produce a magnetized filament by combining recyclable polymer with magnetic powders and conductive fillers. Ultimately, the proposed filament could be used for 3D-printing electromagnetic shielding, which is used for applications such as surface ship radar, weather radar, air traffic control, inflight Wi-Fi and spacecraft telemetry.

MULTIDISCIPLINARY

VMFA Horizontal Track System  (MULTI 24-618)

Cole Jordan, Zach Mazzola, Andrew Shalaski and Matthew Hayden (advisers: Reza Mohammadi, Dan Resler)

Art conservationists meticulously preserve and repair paintings that tie us to the past by using fine precision tools, including microscopes. But the risks – even just a minuscule flake of paint chipping off during movement or inspection – are high.

This group set out to design a track system to allow a microscope to travel over large pieces of art. Think of a claw machine at the arcade, where the claw can be positioned and then lowered – though in this case, the “prize” isn’t touched.

The current industry standard for such inspection costs hundreds of thousands of dollars. This system can be built in stages, starting with manual motion based on a system of rails and carriages — like a drawer — that can then be upgraded to automated motion with motors. It can be installed for about $8,500.

BEST INDUSTRIAL

Advanced Rudder Actuation  (MNE 24-509)

Keely Cooley, Alex Sellers, Thomas Goldstein and Daniel Woodworth (adviser: Worth Longest)

Few modifications have been made to control systems in submarines over the past century, with new technologies having issues of scalability and cost. The team wanted to explore better maneuverability, stealth and efficiency of submarines.

After creating more than 15 novel rudder and control designs, the team chose the top three for further analysis: a whale-inspired tubercle design, a triple-hinged flap and a compliant mechanism design. The students then prototyped and tested each design in a wind tunnel, comparing the results with a control test sample and to each other. The major measures of success were maximum lift-to-drag ratios of hydrofoils, minimal acoustic effects and signatures, and robustness of the design.

Preliminary results showed that while the whale tubercle design acted as intended, a few changes to the design could have increased the lift-to-drag ratio in comparison with the control sample. Analysis is still underway for the compliant and segmented hydrofoils, which are expected to perform better than the tubercle design.

EXCELLENCE IN DESIGN

Design to Reduce Pulse Oximeter Reading Inaccuracies Due to Skin Pigmentation  (BME 24-107)

Patients of color often receive delayed oxygen treatment due to inaccuracy in readings from the pulse oximeter — the device that clips on to a finger to measure oxygen in blood. Current models report a larger number than usual, which can be dangerous as doctors don’t realize the patient actually needs more oxygen.

The students developed a new oximeter that improves how it measures the oxygen in blood, regardless of skin color.

“Health care has a long history of racial and gender-related disparities that have long affected the quality of treatment many individuals receive,” Pathcin said. “This has taken the form of issues like the one our project addresses, where the technology itself is not suited for darker skin tones, and medical practices founded on misinformation and medical myths.”

PEOPLE’S CHOICE

Formula SAE Aerodynamic Front Wing Design  (MNE 24-516)

Carson Clark, Jonathan Morton, Garrett Giddings and Ryan Noll (advisers: Bradley Nichols, Laleh Golshahi)

Ram Racing, the multidisciplinary student organization that designs, builds and races Formula 1, Formula E and Mini Baja race cars from scratch, wanted to improve the performance of its next race car.

Members of this project group who have experience racing the car have noticed a serious performance problem when the car enters a corner. The car lacks traction at the front wheels and slides, rather than turns, when cornering, which slows its speed in that part of the racecourse.

Speaking to other FSAE race teams from other Virginian colleges, the Capstone team learned that many race teams have little to no background research or analysis for their front wing design. The student team designed a two-foil front splitter for a Formula SAE car to produce maximum downforce while maintaining laminar flow over the front tires.

“It’s the opposite of an airplane wing,” Noll said. “Instead of lifting the plane up, it forces the car down.”
