ChatGPT for Research and Publication: A Step-by-Step Guide

Introduction

This commentary provides a concise step-by-step guide on using ChatGPT, an advanced natural language processing (NLP) model, for research and publication purposes. The guide assesses crucial aspects, including data preprocessing, fine-tuning techniques, prompt engineering, and ethical considerations. By addressing challenges related to biases, interpretability, and plagiarism, this commentary offers insights and recommendations for the responsible and ethical use of ChatGPT. The guide empowers researchers to ethically integrate ChatGPT effectively into their workflows, enhancing productivity and improving the quality of their scientific publications. Through clear instructions and guidelines, researchers can tap into the transformative potential of ChatGPT, driving scientific progress in a language-driven world.

In recent years, the field of NLP has witnessed remarkable advancements, bringing us closer to the realm of humanlike language generation. Among these advancements, ChatGPT, based on the groundbreaking GPT-3.5 architecture developed by OpenAI, stands as an impressive language model capable of generating coherent and contextually relevant text responses. With its ability to understand and respond to user inputs, ChatGPT has opened up new possibilities for various applications, including research and publication. 1 – 3

The traditional process of conducting research and publishing scientific papers has been significantly transformed by the emergence of ChatGPT. Researchers and authors can now leverage the power of this sophisticated language model to streamline and enhance their workflow, leading to improved efficiency and higher-quality publications. However, using ChatGPT effectively in the research and publication domain requires a comprehensive understanding of its capabilities, limitations, and best practices.

In this commentary I offer my thoughts for a step-by-step guide for researchers and authors who seek to harness the potential of ChatGPT in their research endeavors and publication efforts. By exploring various aspects, such as data preprocessing, fine-tuning techniques, prompt engineering, and ethical considerations, this guide will equip researchers with the necessary knowledge to harness the full potential of ChatGPT in their scientific pursuits. Moreover, this commentary will delve into the challenges associated with using ChatGPT for research and publication, including biases, interpretability, and concerns regarding plagiarism. By addressing these challenges directly, I aim to provide researchers with valuable insights and recommendations to navigate these important issues and ensure the responsible and ethical use of ChatGPT as a research tool. 4

The significance of my guide lies in its potential to bridge the gap between the rapid progress of language models like ChatGPT and the research and publication process. By elucidating the intricacies of integrating ChatGPT into scientific workflows, researchers will be empowered to leverage this advanced technology effectively, thereby enhancing the overall quality and impact of their research output. 5 In the following sections, I present a comprehensive overview of the steps involved in using ChatGPT for research and publication.

Step 1: Title and Title Page Creation by ChatGPT

ChatGPT can be a valuable tool in generating titles for research papers. Its ability to understand and generate humanlike text allows it to analyze and synthesize information provided by researchers to craft concise and impactful titles. By leveraging its vast knowledge base and language capabilities, ChatGPT can assist in capturing the essence of a research paper, conveying the main focus and contributions succinctly. Researchers can collaborate with ChatGPT by providing relevant information, such as the subject, objectives, methodology, and key findings of their study. ChatGPT can then generate multiple title options, offering different perspectives and angles that researchers can consider. This collaboration with ChatGPT can save time and stimulate creativity, helping researchers refine their titles to accurately represent their work and engage potential readers. ChatGPT can then be used to create the entire title page and then can also customize based on each journal’s recommendations.

Thus, we see that ChatGPT can write an entire title page based on just the title and author details. We notice that ChatGPT has created an email address that is incorrect and needs manual rectification. However, the rest of the title page, including keywords and the running title, is appropriate.

Step 2: Abstract/Summary Creation by chatGPT

ChatGPT can assist in condensing complex information into a clear and engaging abstract/summary, helping researchers communicate the significance and novelty of their research to a wider audience. By leveraging the language proficiency of ChatGPT, researchers can save time and effort in crafting abstracts while ensuring that the key aspects of their study are accurately represented.

In this example, we demonstrate that ChatGPT can create an entire abstract just by using the title alone. However, the more information researchers provide (preferably the entire body of the paper should be entered into chatGPT), the more accurate the abstract becomes.

Step 3: Introduction Creation by ChatGPT

By collaborating with ChatGPT, researchers can provide key information, such as the background, significance, and objectives of their study. ChatGPT can then generate a well-structured introduction that sets the context, highlights the relevance of the research, and outlines the paper’s objectives. Also, ChatGPT can be used to generate keywords and generate an abbreviations list from the article by using prompts. However, it is important to note that the generated introduction should be reviewed, customized, and refined by the researchers to align with their specific study and writing style.

In the example below, we note that ChatGPT has not only created an introduction but also the objectives of the study, which can then be edited by the human author.

Step 4: Can ChatGPT Create a Literature Review?

Yes, ChatGPT can help generate a literature review, but it is important to note that it may not have access to the most up-to-date research articles and studies due to copyrights and limited access to some journals. Additionally, a literature review typically requires a comprehensive analysis of multiple sources, so the generated response may not cover all relevant studies. Nonetheless, it can assist in providing a basic literature review on a given topic, which will need human authors to add to and edit it.

As we can see, ChatGPT is not as good at giving a detailed review of the literature as it is at summarizing contents or creating an introduction. Thus, its use is limited, if there is any at all, in this section of the paper.

Step 5: Can ChatGPT Assist in Brainstorming the Methodology of Studies?

ChatGPT can be a helpful tool in conceptualizing the methodology for research papers. By engaging in a conversation with ChatGPT, researchers can discuss their research objectives, study design, data collection methods, and data analysis techniques. ChatGPT’s natural language understanding allows it to provide suggestions and insights based on its knowledge base and understanding of research methodologies. Although ChatGPT can assist in generating ideas and providing guidance, it is important for researchers to critically evaluate and adapt the suggestions to align with their specific research goals and requirements.

Although the methodology is something that is unique to each paper and needs a human researcher to conceptualize it, we see in this example that ChatGPT can assist by giving ideas and examples based on the input of the title by the human researcher. Thus, ChatGPT can be part of brainstorming sessions when conceptualizing a study, although this section needs significant editing by a human, unlike the introduction or summary.

Step 6: Do Not Use ChatGPT for Fabricating Patient Data or Results!

This section of the paper must be authentic, and ChatGPT has a limited role, if any, because patient data have to be original. ChatGPT also currently cannot analyze clinical data compared with statistical software, like SPSS Statistics and Base SAS. However, Microsoft appears to be developing an Excel copilot that uses AI to create graphs and plots, and its use needs to be evaluated once it is released to the public. 6

Step 7: Discussion and Conclusions

This section of the paper can be generated by ChatGPT if all results are pasted as input; however, this section also needs manual editing because inaccuracies are common. By discussing their research with ChatGPT, researchers can also identify potential limitations, discuss the broader implications of their findings, and propose future research directions. Although ChatGPT can generate suggestions and facilitate the thought process, it is important for researchers to critically evaluate the information provided and ensure that the Discussion and Conclusion sections align with the specific research objectives and findings of their study. Ultimately, ChatGPT can serve as a supportive tool in developing a comprehensive and well-rounded discussion and conclusion for research papers.

Step 8: References

As per the author's experience, although ChatGPT is capable of creating references for an article, most of them are incorrect. So, using ChatGPT for creating references is not recommended. However, ChatGPT can convert references into any journaling style if the references are entered into ChatGPT and it is asked to convert them into a specific style.

Disadvantages of Using ChatGPT in Research

Although ChatGPT offers numerous advantages for assisting in the writing of research papers, there are also some important potential disadvantages to consider:

  • Lack of domain expertise: ChatGPT is a general-purpose language model trained on a diverse range of Internet text, which means it may lack the specific domain expertise required for certain research topics. It may generate responses that are not accurate or well informed in specialized fields, potentially leading to incorrect or misleading information in research papers.
  • Inconsistency and variability: ChatGPT’s responses can be inconsistent and vary depending on the input phrasing or prompt formulation. This can lead to unpredictability in generating reliable and coherent content, requiring additional effort to refine and ensure accuracy in research papers.
  • Limited control over output: Although researchers can guide the model’s responses through prompts, ChatGPT’s generation process is still primarily autonomous. Researchers have limited control over the precise content and structure of the generated text, which may require careful editing and review to align with specific research goals, standards, and above all, accuracy.
  • Biases and ethical considerations: Language models like ChatGPT can inadvertently reflect biases present in the training data. These biases may perpetuate existing societal or cultural biases in research papers, potentially leading to unfair or discriminatory content. The careful examination and mitigation of biases are crucial to ensure ethical and unbiased research output. 7
  • Lack of interpretability: ChatGPT’s decision-making process is complex and not easily interpretable. Researchers may struggle to understand the reasoning behind the model’s generated responses, making it challenging to assess the reliability and credibility of the information provided. Ensuring transparency and interpretability in research papers becomes more challenging with such models. ChatGPT should cite the sources for its data, like Google Bard does.
  • Plagiarism concerns: Because of its vast training data from the Internet, ChatGPT may inadvertently generate text that resembles or replicates existing content without proper citation or attribution. Researchers must be cautious about unintentional plagiarism and ensure that generated content is appropriately referenced and original. So, all ChatGPt-generated articles need to be double checked using antiplagiarism software.

In this commentary I have provided a comprehensive step-by-step guide for researchers and authors on harnessing the power of ChatGPT in the realm of research and publication. By exploring crucial aspects, such as data preprocessing, fine-tuning techniques, prompt engineering, and ethical considerations, the guide equips researchers with the necessary knowledge and tools to effectively integrate ChatGPT into their scientific workflows. 8

Through clear instructions, examples, and guidelines, researchers can navigate the complexities of using ChatGPT, leading to enhanced productivity and improved quality in their research output. However, and to reiterate, I cannot overemphasize that ChatGPT has, at present, many disadvantages, including inconsistencies, bias, and plagiarism concerns, that must be addressed by the human author before the article is submitted for publication to a journal, as well as prior to publication, because the human author(s) is solely responsible for their research integrity and accurate reporting.

In conclusion, I have attempted to provide researchers with a comprehensive understanding of how to effectively leverage ChatGPT for research and publication purposes. It has also highlighted the problems and precautions that the human author(s) must take before publishing ChatGPT-generated content. By embracing this step-by-step guide, researchers can unlock the full potential of ChatGPT, driving scientific progress and shaping the future of research and publications.

A study that identified buzzword adjectives that could be hallmarks of AI-written text in peer-review reports suggests that researchers are turning to ChatGPT and other artificial intelligence (AI) tools to evaluate others’ work.

The authors of the study 1 , posted on the arXiv preprint server on 11 March, examined the extent to which AI chatbots could have modified the peer reviews of conference proceedings submitted to four major computer-science meetings since the release of ChatGPT.

Their analysis suggests that up to 17% of the peer-review reports have been substantially modified by chatbots — although it’s unclear whether researchers used the tools to construct reviews from scratch or just to edit and improve written drafts.

How ChatGPT and other AI tools could disrupt scientific publishing

The idea of chatbots writing referee reports for unpublished work is “very shocking” given that the tools often generate misleading or fabricated information, says Debora Weber-Wulff, a computer scientist at the HTW Berlin–University of Applied Sciences in Germany. “It’s the expectation that a human researcher looks at it,” she adds. “AI systems ‘hallucinate’, and we can’t know when they’re hallucinating and when they’re not.”

The meetings included in the study are the Twelfth International Conference on Learning Representations, due to be held in Vienna next month, 2023’s Annual Conference on Neural Information Processing Systems, held in New Orleans, Louisiana, the 2023 Conference on Robot Learning in Atlanta, Georgia, and the 2023 Conference on Empirical Methods in Natural Language Processing in Singapore.

Nature reached out to the organizers of all four conferences for comment, but none responded.

Buzzword search

Since its release in November 2022, ChatGPT has been used to write a number of scientific papers , in some cases even being listed as an author . Out of more than 1,600 scientists who responded to a 2023 Nature survey , nearly 30% said they had used generative AI to write papers and around 15% said they had used it for their own literature reviews and to write grant applications.

In the arXiv study, a team led by Weixin Liang, a computer scientist at Stanford University in California, developed a technique to search for AI-written text by identifying adjectives that are used more often by AI than by humans.

By comparing the use of adjectives in a total of more than 146,000 peer reviews submitted to the same conferences before and after the release of ChatGPT, the analysis found that the frequency of certain positive adjectives, such as ‘commendable’, ‘innovative’, ‘meticulous’, ‘intricate’, ‘notable’ and ‘versatile’, had increased significantly since the chatbot’s use became mainstream. The study flagged the 100 most disproportionately used adjectives.

Scientific sleuths spot dishonest ChatGPT use in papers

Reviews that gave a lower rating to conference proceedings or were submitted close to the deadline, and those whose authors were least likely to respond to rebuttals from authors, were most likely to contain these adjectives, and therefore most likely to have been written by chatbots at least to some extent, the study found.

“It seems like when people have a lack of time, they tend to use ChatGPT,” says Liang.

The study also examined more than 25,000 peer reviews associated with around 10,000 manuscripts that had been accepted for publication across 15 Nature Portfolio journals between 2019 and 2023, but didn’t find a spike in usage of the same adjectives since the release of ChatGPT.

A spokesperson for Springer Nature said the publisher asks peer reviewers not to upload manuscripts into generative AI tools , noting that these still have “considerable limitations” and that reviews might include sensitive or proprietary information. ( Nature’s news team is independent of its publisher.)

Springer Nature is exploring the idea of providing peer reviewers with safe AI tools to guide their evaluation, the spokesperson said.

Transparency issue

The increased prevalence of the buzzwords Liang’s study identified in post-ChatGPT reviews is “really striking”, says Andrew Gray, a bibliometrics support officer at University College London. The work inspired him to analyse the extent to which some of the same adjectives, as well as a selection of adverbs, crop up in peer-reviewed studies published between 2015 and 2023. His findings, described in an arXiv preprint published on 25 March, show a significant increase in the use of certain terms, including ‘commendable’, ‘meticulous’ and ‘intricate’, since ChatGPT surfaced 2 . The study estimates that the authors of at least 60,000 papers published in 2023 — just over 1% of all scholarly studies published that year — used chatbots to some extent.

Gray says it’s possible peer reviewers are using chatbots only for copyediting or translation, but that a lack of transparency from authors makes it difficult to tell. “We have the signs that these things are being used,” he says, “but we don’t really understand how they’re being used.”

Weber-Wulff doesn’t think tools such as ChatGPT should be used to any extent during peer review, and worries that the use of chatbots might be even higher in cases in which referee reports are not published. (The reviews of papers published by Nature Portfolio journals used in Liang’s study were available online as part of a transparent peer-review scheme .) “Peer review has been corrupted by AI systems,” she says.

Using chatbots for peer review could also have copyright implications, Weber-Wulff adds, because it could involve giving the tools access to confidential, unpublished material. She notes that the approach of using telltale adjectives to detect potential AI activity might work well in English, but could be less effective for other languages.

Background: Medical documentation plays a crucial role in clinical practice, facilitating accurate patient management and communication among health care professionals. However, inaccuracies in medical notes can lead to miscommunication and diagnostic errors. Additionally, the demands of documentation contribute to physician burnout. Although intermediaries like medical scribes and speech recognition software have been used to ease this burden, they have limitations in terms of accuracy and addressing provider-specific metrics. The integration of ambient artificial intelligence (AI)–powered solutions offers a promising way to improve documentation while fitting seamlessly into existing workflows.

Objective: This study aims to assess the accuracy and quality of Subjective, Objective, Assessment, and Plan (SOAP) notes generated by ChatGPT-4, an AI model, using established transcripts of History and Physical Examination as the gold standard. We seek to identify potential errors and evaluate the model’s performance across different categories.

Methods: We conducted simulated patient-provider encounters representing various ambulatory specialties and transcribed the audio files. Key reportable elements were identified, and ChatGPT-4 was used to generate SOAP notes based on these transcripts. Three versions of each note were created and compared to the gold standard via chart review; errors generated from the comparison were categorized as omissions, incorrect information, or additions. We compared the accuracy of data elements across versions, transcript length, and data categories. Additionally, we assessed note quality using the Physician Documentation Quality Instrument (PDQI) scoring system.

Results: Although ChatGPT-4 consistently generated SOAP-style notes, there were, on average, 23.6 errors per clinical case, with errors of omission (86%) being the most common, followed by addition errors (10.5%) and inclusion of incorrect facts (3.2%). There was significant variance between replicates of the same case, with only 52.9% of data elements reported correctly across all 3 replicates. The accuracy of data elements varied across cases, with the highest accuracy observed in the “Objective” section. Consequently, the measure of note quality, assessed by PDQI, demonstrated intra- and intercase variance. Finally, the accuracy of ChatGPT-4 was inversely correlated to both the transcript length ( P =.05) and the number of scorable data elements ( P =.05).

Conclusions: Our study reveals substantial variability in errors, accuracy, and note quality generated by ChatGPT-4. Errors were not limited to specific sections, and the inconsistency in error types across replicates complicated predictability. Transcript length and data complexity were inversely correlated with note accuracy, raising concerns about the model’s effectiveness in handling complex medical cases. The quality and reliability of clinical notes produced by ChatGPT-4 do not meet the standards required for clinical use. Although AI holds promise in health care, caution should be exercised before widespread adoption. Further research is needed to address accuracy, variability, and potential errors. ChatGPT-4, while valuable in various applications, should not be considered a safe alternative to human-generated clinical documentation at this time.

Introduction

Medical documentation is an integral aspect of clinical practice, ensuring accuracy and comprehensive patient management and serving as a communication tool among health care professionals. In recent years, it has become increasingly evident that inaccuracies in medical notes lead to miscommunication, diagnostic discrepancies, and patients’ perceptions of subpar medical care [ 1 ]. Beyond the immediate implications of documentation errors, documentation demands have been identified as a significant contributor to physician burnout [ 2 ]. With health care professionals spending an increasing amount of their working hours on paperwork, there is less time and energy left for direct patient care.

To counter this, many institutions have adopted the use of intermediaries, such as medical scribes or speech recognition software, to shoulder the documentation load and allow clinicians to focus on patient interactions. However, both of these solutions have significant limitations and concerns regarding documentation accuracy and lack of impact on many provider-specific metrics surrounding after-hours charting and burnout [ 3 , 4 ]. In addition, the financial implications of employing medical scribes render them inaccessible to numerous health care practices [ 5 ]. Consequently, there is a continued search for innovative solutions to create effective and accurate documentation while seamlessly integrating into existing workflows.

With the rapid and exponential growth in computing capacity, artificial intelligence (AI) is being increasingly used in health care, holding the promise of revolutionizing medical documentation, thus potentially alleviating the burden on physicians [ 6 ]. AI-powered systems can analyze vast amounts of data quickly, identify patterns, and suggest diagnostic options. Although the allure of AI is undeniable, questions regarding its accuracy, reliability, and suitability in the clinical setting remain. The maturation of speech recognition technology has led to large-scale adoption by health care organizations, allowing for real-time transcription services. This, combined with software using large language models (LLMs), now enables the creation of structured medical notes in close temporal relation to the clinical encounter, thereby decreasing the clinician documentation burden [ 7 , 8 ]. Multiple software vendors are developing and deploying documentation assistance software powered by ambient AI, referred to as ambient digital scribes. There is already significant interest on the part of clinicians and health care organizations to adopt them. However, little data exist on the safety and quality of the documentation, with analysis made more difficult by the proprietary AI engine used by each vendor.

One such AI system is OpenAI’s ChatGPT-4, a state-of-the-art LLM known for its ability to engage in text-based communication with users (as a chatbot), which is used in some commercial ambient digital scribe solutions. Released in November 2022, ChatGPT-4 is trained on a vast amount of text data from the internet and uses an LLM to answer the users’ prompts. Health care providers envision numerous applications for ChatGPT-4, such as answering patient questions, automated insurance prior authorizations, and creating differential diagnoses [ 9 , 10 ]. It is important to note that open AI platforms, such as ChatGPT-4, are not recommended for clinical use due to the many regulatory and privacy issues. Despite this, there is a continued interest in whether ChatGPT-4 could serve as a freely available tool to assist as a documentation intermediary, bridging the gap between health care professionals and the tedious task of recordkeeping.

However, it is imperative that prior to the widespread adoption of these tools, their safety and efficacy need to be evaluated in a structured and clinically contextually relevant manner. Therefore, the goal of this study was to use transcripts from simulated patient-provider encounters to determine the accuracy, readability, and reproducibility of ChatGPT-4–generated Subjective, Objective, Assessment, and Plan (SOAP) notes.

As part of a project designed to evaluate the accuracy and efficacy of human scribe–generated notes, we created 14 simulated patient-provider encounters. All encounters used professional standardized patients and represented a wide range of ambulatory specialties. A standardized patient is an individual trained to simulate a medical scenario for health care education, assessment, and research. Briefly, for each case, a storyboard was created by subject matter experts and used for training the standardized patient to ensure standard content delivery according to best practices [ 11 ]. After an initial dry-run, each scenario was conducted in a simulated ambulatory patient exam room equipped with audio-video capture. At the end of the scenario, audio-video files were exported for use. These cases represented a variety of diagnoses ( Table 1 ).

Audio files for each case were professionally transcribed. For each encounter, a list of key reportable elements was created for each case using the transcripts and informed by the initial storyboard, by 2 clinical experts of the study team. This is being used as the scoring rubric for subsequent analysis. These encounter transcripts were then fed into ChatGPT-4 using a standard prompt (“generate a clinical note in SOAP format for the following”). The SOAP format is a widely used clinical documentation format that concatenates data elements of the clinical interview into headers representing SOAP-related components. The SOAP format is a standard model for medical documentation, providing a clear, concise framework for health care professionals to record and share patient information. Each transcript was run through the model three times to assess output fidelity associated with replicability, thus generating three documentation versions for each case for a total of 42 ChatGPT-4 generated SOAP notes (the prompt and full output are present in Multimedia Appendix 1 ). A new discussion space was created for each case to prevent the various transcripts from conflating each other. Each prompt request was conducted consecutively within the same discussion space.

After acquiring the generated notes, various comparisons were made to assess the output’s accuracy and quality. Within a single case, the 3 versions were analyzed based on errors generated. A list of errors was defined as follows: (1) omissions—where expected documentation elements or data were missing; (2) incorrect—where the data element was referred to but incorrect; and (3) additions—information added that was not in the transcript. The framework for defining quality in clinical documentation based on omissions, incorrect information, and additions is a structured approach to evaluate the accuracy and completeness of medical records. These characteristics (omissions, incorrect information, and additions), if present, help define the quality of documentation given their implications. For example, omissions can lead to gaps in patient care, misdiagnosis, or delays in treatment. Incorrect information can compromise patient safety and lead to negative health consequences. Additions, while not always harmful, can be inaccurate and reduce the efficiency of care delivery. This framework is particularly useful in assessing the performance of health care documentation processes, such as those generated by medical scribes, and in quantifying appropriate information retrieval [ 4 , 12 ]. A correct data element was defined as one without the previously outlined errors.

To ensure and assess note quality, we outlined critical data elements for each clinical case. Members of the study team independently selected these crucial data elements and subsequently compiled them to guarantee comprehensiveness. They used these elements to generate a gold standard checklist and an associated gold standard History and Physical Examination note. Then, 2 raters graded the 3 ChatGPT-4 versions of each encounter based on whether they correctly included, missed, or wrongly presented the corresponding data element. We enumerated the number of errors and correct data elements for each version. Afterward, we compared the correct data elements across the 3 ChatGPT-4 versions for presence and consistency, as follows: (1) across all three versions, (2) across two versions, (3) only in a single version, or (4) not present at all. Finally, we compared the percentages of appropriate data elements across the versions to the transcript’s length and the number of data elements.

The data elements were divided into three documentation-related sections: (1) Subjective , further subdivided into the history of present illness and other patient-reported information, including medications, allergies, family history, social history, and past medical history; (2) Objective , which includes vital signs, physical exam, and any reported test results; and (3) Assessment and Plan , which includes the provider reported differential, plan, and follow-up instructions. The percentages of correct data elements were then compared based on these categories.

Lastly, the Physician Documentation Quality Instrument (PDQI) scoring system, which is a validated tool to assess note quality, was used to evaluate the quality of the generated notes [ 13 ]. Using a set of predefined criteria, the PDQI facilitates the objective analysis of documentation practices. Within the PDQI, 9 criteria assess if the document is (1) up to date, (2) accurate, (3) thorough, (4) useful, (5) organized, (6) comprehensible, (7) succinct, (8) synthesized, and (9) consistent. The items are then scored based on a 5-point Likert scale, with the highest value representing the ideal characteristic. A maximum score of 45 represents the document that extremely shows the associated attribute, and a minimum score of 9 points indicates that the attribute is not at all present. The PDQI score was calculated for the 3 versions of the generated note, averaged, and compared across the 14 cases by a member of the study team (AK).

Statistical Analysis

All statistical analyses were performed using GraphPad Prism (version 10; GraphPad Software Inc). For between-group comparisons, we used the Friedman test for overall and between-group comparisons given the nonnormal distribution of the data as determined by the Kolmogorov-Smirnov test. Pearson r test was used for univariate correlations. A P value <.05 was considered statistically significant.

Ethical Considerations

The study was deemed exempt from an institutional review board approval as it did not include human subjects and therefore did not pose any risks.

We first looked at the overall structure of the notes. Consistently, ChatGPT-4 was able to generate a SOAP-style note. Overall, there was a significant variance in note length between the 3 replicates, with the transcripts of some cases being very similar in length, while others showed nearly a 50% variance between replicates ( Figure 1 ).

We classified errors into 3 types: errors of omission, those related to incorrect facts, and errors associated with information addition. Overall, the total number of errors ranged from 5.7 to 64.7 errors per case, with significant differences between the replicates ( Figure 2 A). When we subdivided errors into the 3 basic types, errors of omission were by far the most common, comprising, on average, 86.3% of all errors, followed by addition errors (10.5%) and incorrect facts (3.2%). Examples of these types of errors are illustrated in Table 2 . There was significant variance both in the total number and distribution of errors between cases and between replicates of the same case ( Figure 2 B).

For accuracy, we assessed the overall congruence between replicates. The frequency of correct reporting across the 3 replicates was compared against the gold standard History and Physical Examination. Overall, the mean percentage of elements reported correctly across all 3 replicates for the 14 cases was 53% (range 22%-79%). Interestingly, nearly 30% of data elements were reported correctly in only 1 or 2 of the replicates, suggesting issues with both accuracy and congruency ( Figure 3 ).

Breaking down ChatGPT-4’s performance based on individual categories, there was a significant variance in the accuracy in each section of the note. Specifically, the highest accuracy was observed in the Objective section of the note (median 86.9, IQR 75.4%-96.9%) and was significantly higher compared to the History and Physical examination (median 63.8%, IQR 54.2%-76.8%; P =–.02), Other (median 75.2%, IQR 68.5%-82.4%), and Assessment and Plan (median 66.9%, IQR 36.4%-83.5%; Figure 4 ).

The combination of variance in note structure as well as the number and type of errors resulted in similar variance in overall note quality as determined by PDQI-9. Overall, the mean PDQI-9 score was 29.7 (range 23.7-39.7), with significant variance between replicates within a case ( Figure 5 ).

Finally, we wished to determine whether characteristics in the parent transcript were associated with note quality. Overall, transcript length and the total number of scorable elements (as a measure of information density or complexity) both correlated inversely with the total percentage of elements reported correctly across the 3 replicates for each case ( Figure 6 ). We observed similar findings for PDQI-9 (details are not shown).

Principal Results

Our study highlights the significant variations in errors, accuracy, and quality of SOAP notes generated by ChatGPT-4. With regard to errors, they are not limited to specific sections of the note and include errors of omission as well as commission. Although the number of errors is consistent with regard to the number of data elements, another important finding is that the error rate is not consistent across replicates of the same case. This means that the model is not making the same errors repeatedly, making it difficult for health care providers to predict where errors may occur. This variability introduces a level of unpredictability, which can impact clinical oversight.

Comparison With Prior Work

In the context of medical research, our investigation has shed light on the critical issue of documentation accuracy, which has been a recurring concern in prior studies. Our findings align with the existing body of research on digital scribes, revealing noteworthy variations in accuracy, particularly in the context of nonobjective data [ 4 , 14 ]. In the realm of ChatGPT-4, the study conducted by Johnson et al [ 15 ] delved into its performance in giving precise and comprehensive medical information. This inquiry enlisted the expertise of 33 physicians, spanning 17 different specialties, who formulated questions that were subsequently posed to ChatGPT. Approximately 57.8% of the generated responses were assessed as accurate or nearly correct. This outcome underscores the imperative for exercising caution when solely relying on AI-generated medical information and the need for continuous evaluation, as others have noted [ 16 ]. However, in another study by Walker et al [ 17 ] aimed at evaluating the reliability of medical information provided by ChatGPT-4, multiple iterations of their queries executed through the model yielded a remarkable 100% internal consistency among the generated outputs [ 17 ]. Although promising, it should be noted that the queries used in their experiment consisted of direct single-sentence questions pertaining to specific hepatobiliary diagnoses. This mode of input differs significantly from the transcription of patient encounters. Our research, in contrast, stands out by probing the reproducibility of note generation—a relatively less explored topic in existing literature.

The PDQI-9 scores also highlight the overall variance in quality. In previous research, the PDQI-9 score of 26.2 was rated “terrible or bad,” versus a PDQI-9 score of 36.6, which was rated “good or excellent” [ 13 ]. In our study, the mean PDQI-9 score of 29.7 is closer to the “terrible or bad” range. These observations suggest that although ChatGPT-4 can consistently generate a SOAP-style note, it introduces errors and struggles with maintaining uniformity and accuracy. These issues could pose potential challenges if implemented in a clinical setting.

An essential aspect of our research was to identify the factors contributing to inaccuracies in AI-generated notes. Notably, we found an inverse correlation between note accuracy and transcript length as well as the amount of reportable data. This observation has profound implications for large language models like ChatGPT-4, indicating their challenges with longer and denser information. This raises questions about their effectiveness in handling complex medical cases.

These findings have significant clinical implications. The high variability in PDQI-9 scores, coupled with a high error rate, indicates low-quality notes. Recently, there have been concerns regarding ChatGPT-4’s capacity to generate what can be classified as “hallucinations”—synthesized data that may be misinterpreted as factual information. These data are often incomplete and sometimes misleading [ 18 ]. This has implications for the quality of patient care, potentially leading to diagnostic errors and eroding trust in AI, both among health care providers and patients. Acknowledging the increasing documentation burden contributing to physician burnout, generative AI technology for clinical note documentation may save time [ 2 , 19 ]. However, if our data are representative of similar accuracy rates with other AI-powered systems, any time savings could be negated by the need for corrections. This mirrors previous studies with human scribes, where widespread adoption had little impact on after-hours charting or chart completion time [ 20 - 22 ].

Limitations

Our research is not without limitations. Primarily, the generated SOAP notes underwent processing through an open AI model, in contrast to the proprietary closed models commonly used in the generative AI domain of health care. It is pertinent to note that proprietary technologies, such as DAX Copilot (a collaborative venture of Microsoft and Nuance), have restricted accessibility, available only to entities holding contractual agreements with the parent company. Furthermore, these models evolve iteratively. Consequently, the errors as well as the correct elements in our current data set might not manifest in subsequent versions. However, it is important to note that the methodology reported here establishes a means by which these systems can be evaluated systematically. It should be acknowledged that this study only used transcripts, eliminating the confounder of any potential errors introduced by the speech recognition aspect [ 23 , 24 ]. Integrating this aspect will be critical for a more complete evaluation of fully integrated generative AI–powered documentation assistants. Another limitation is the inability to draw conclusions regarding the correlation between types of cases and associated errors. A substantially larger volume of encounters would be required to delineate this relationship. Additionally, despite its standardized criteria, using the PDQI can still be influenced by the subjective judgment of the reviewer and can be a time-consuming process, particularly for longer documents. Conversely, the instrument does cover a broad range of quality dimensions, facilitating a more holistic evaluation. Furthermore, it can be used as a diagnostic tool to identify strengths and weaknesses, guiding targeted quality improvement initiatives. Finally, in large language models, such as ChatGPT-4, the temperature of the model is a parameter that controls the randomness or predictability of the model’s output. It is a component that tunes the model to generate responses that are either more varied and creative or more deterministic and conservative. With this in mind, ChatGPT-4’s temperature allows for variability, but this setting is not accessible to the end user [ 25 , 26 ]. Further, even setting the temperature to zero does not appear to ensure uniformity of response [ 27 ]. Along these lines, the absence of real-time feedback within the application also limits the model’s ability to adjust its responses based on user input, and therefore, hinders the model’s opportunity to learn from real-world interactions and refine its output.

Conclusions

In conclusion, our study used standardized simulated patient-provider interactions to evaluate the quality and reliability of AI-generated clinical notes. The generated notes do not meet the acceptable quality standards for clinical use. Our methodology provides a foundation for future assessments of AI technology in terms of quality and safety. At this time, AI should not be considered a safe alternative to digital scribes.

Five key insights into ChatGPT students need to know

ChatGPT has taken the world by storm in a relatively short time. The creators of the trending chatbot, OpenAI, released a demo for ChatGPT in 2022. Since then, educators have been forced to discuss the artificially intelligent elephant in the room.

Discussions have been rooted in finding a balance between fear of compromising academic integrity and using artificial intelligence as an instructional tool.

In February 2023, the Provost’s Ethics Colloquium held a public discussion at the Lory Student Center at Colorado State University regarding the impact of ChatGPT on academics. Matthew Hickey , the associate dean for research and graduate studies in the College of Health and Human Sciences, moderated the discussion and provided his own insights and advice for the ever-evolving chatbot.

1. Understanding the potency of ChatGPT and AI

Hickey notes that it did not take long for ChatGPT to ascend to the level of proficiency that it has. From the early days of the beta at the end of 2022, the chatbot can now crank out an entire essay in less than a minute, carry out a comprehensive and intelligent-sounding conversation with the user, and even write code. With how quickly the program has developed, Hickey cites the uncertainty that both he and his colleagues faced, especially in terms of academic accountability mechanisms.

Not only is ChatGPT a concern academically, but it has already resulted in job loss – specifically in writing-centric industries. ChatGPT and other programs have become so concerning that OpenAI and other AI experts have signed a statement for mitigating the risk of human extinction as a result of AI. The statement equates the human extinction resulting from AI to that of pandemics and nuclear war.

 2. ChatGPT should be a tool, not a ghostwriter

While using ChatGPT to write an essay for a class is not exactly comparable to nuclear war, it still takes a toll on the student. Hickey says that copying and pasting from ChatGPT can cause intellectual harm.

“We’ve got to create a climate on campus where the students themselves can realize that, ‘I’m actually doing myself harm because I’m not learning, and that’s gonna hurt me when I’m going out looking for a job, etc.,’” Hickey said. “Because we’re not going to be able to rely on these large language models and in every setting we find ourselves in for the rest of our lives.”

 3. Transparent and mindful use of ChatGPT

Hickey does not feel the need to ban ChatGPT from his classroom but instead expects transparency from his students if they use it as a tool. He wants to be sure that his students are turning in their own writing and not dragging and dropping entire sentences or sections from the program, something he considers a violation of academic integrity. Hickey says it can be beneficial to probe the chatbot as if it were another conversational partner in a classroom setting.

While Hickey allows the use of ChatGPT in his classes so long as his students communicate their use to him, he knows that some educators on campus may not have the same policy and disallow the chatbot altogether. In this case, he feels that students should respect the rule and refrain from using it.

 4. ChatGPT runs on an illusion of intelligence

A glaring issue brought up by Hickey and many other users of ChatGPT is how good it is at making things up and sounding confident about it.

“ChatGPT and other large language models are trained to generate plausible texts,” Hickey said. “It’s not trained to deliver truthful text. It’s just plausible.”

Hickey cited a physician’s use of ChatGPT to model the pulmonary blood flow responses to particular stressors. What the chatbot generated was completely implausible for humans, but for medical students who may not know any better, the results could be misleading and problematic. This is why Hickey stresses the importance of knowing that ChatGPT can be used as a tool, but also that it is still full of flaws and requires fact-checking.

5. Artificial intelligence is here to stay

Technology continues to grow and develop, and with that AI. There is no avoiding it, and ignoring the reality, as Hickey suggests, will not be beneficial. Educators will likely be implementing AI like ChatGPT into courses as a tool, similar to how Hickey and other instructors have.

This goes beyond CSU. Abram Anders, the associate director of the Student Innovation Center at Iowa State University, also has been working on AI tools to use in the classroom. Anders was a keynote speaker at the Provost’s Ethics Colloquium last November.

“You don’t have to be a technical optimist or an AI optimist to engage these tools,” Anders said at the Provost’s Ethics Colloquium. Anders referred to a student of his who is pursuing a career in creative writing, and the student initially held a pessimistic view of AI, fearing it would hinder career prospects. However, through the course, the student realized that AI still requires human guidance, alleviating concerns about the impact on their career potential.

Regardless of the future steps universities and educators take, ChatGPT and AI programs are here to stay.

How does ChatGPT work? As explained by the ChatGPT team.

Sometimes the best explanations of how a technology solution works come from the software engineers who built it. To explain how ChatGPT (and other large language models) operate, I turned to the ChatGPT engineering team.

"How does ChatGPT work, under the hood?"

I asked this from Evan Morikawa at OpenAI. Evan joined OpenAI in 2020 – two years before ChatGPT launched – and has led the Applied engineering team as ChatGPT launched and scaled. His team was the one that created ChatGPT, and Evan has been there from the very beginning.

With this, it’s over to Evan. My questions are in italic.

A refresher on OpenAI, and on Evan

Evan: how did you join OpenAI, and end up heading the Applied engineering group – which also builds ChatGPT?

OpenAI is the creator of ChatGPT, which is just one of the business’ products. Other shipped things include  DALL·E 3  (image generation,)  GPT-4  (an advanced model,) and the  OpenAI API  which developers and companies use to integrate AI into their processes. ChatGPT and the API each expose several classes of model: GPT-3, GPT-3.5, and GPT-4.

The engineering, product, and design organization that makes and scales these products is called "Applied," and was founded in 2020 when GPT-3 was released. It’s broadly chartered with safely bringing OpenAI's research to the world. OpenAI itself was founded in 2015 and at the company’s core today is still a research lab with the  goal  of creating a safe and aligned artificial general intelligence (AGI.)

I joined OpenAI in October 2020 when Applied was brand new. I do  not  have a PhD in Machine Learning, and was excited by the idea of building APIs and engineering teams. I managed our entire Applied Engineering org from its earliest days through the launch and scaling of ChatGPT.  We cover more on Evan’s story in  Inside OpenAI: how does ChatGPT ship so quickly?  

How does ChatGPT work?

For those of us who have not spent the past few years building ChatGPT from the ground up, how does it work?

When you ask ChatGPT a question, several steps happen:

  • Input.  We take your text from the text input.
  • Tokenization.  We chunk it into tokens. A token roughly maps to a couple of unicode characters. You can think of it as a word. 
  • Create embeddings.  We turn each token into a vector of numbers. These are called embeddings. 
  • Multiply embeddings by model weights.  We then multiply these embeddings by hundreds of billions of model weights.
  • Sample a prediction.  At the end of this multiplication, the vector of numbers represents the probability of the next most likely token. That next most likely token are the next few characters that spit out of ChatGPT.

Let’s visualize these steps. The first two are straightforward:

Note that tokenization doesn’t necessarily mean splitting text into words; tokens can be subsets of words as well. 

Embeddings  are at the heart of large language models (LLM), and we create them from tokens in the next step:

An embedding is a multi-dimensional representation of a token. We explicitly train  some of our models  to explicitly allow the capture of semantic meanings and relationships between words or phrases. For example, the embedding for “dog” and “puppy” are closer together in several dimensions than “dog” and “computer” are. These multi-dimensional embeddings help machines understand human language more efficiently.

Model weights  are used to calculate a weighted embedding matrix, which is used to predict the next likely token. For this step, we need to use OpenAI’s weight matrix, which consists of hundreds of billions of weights, and multiply it by a matrix we construct from the embeddings. This is a compute-intensive multiplication.

Sampling a prediction  is done after we do billions of multiplications. The final vector represents the probability of the next most likely token. Sampling is when we choose the next most likely token and send it back to the user. Each word that spits out of ChatGPT is this same process repeated over and over again many times per second.

Pretraining and inference

How do we generate this complex set of model weights, whose values encode most of human knowledge? We do it through a process called  pretraining . The goal is to build a model that can predict the next token (which you can think of as a word), for all words on the internet. 

During pretraining, the weights are gradually updated via  gradient descent , which is is a mathematical optimization method. An analogy of gradient descent is a hiker stuck up a mountain, who’s trying to get down. However, they don’t have a full view of the mountain due to heavy fog which limits their view to a small area around them. Gradient descent would mean to look at the steepness of the incline from the hiker’s current position, and proceed in the direction of the steepest descent. We can assume steepness is not obvious from simple observation, but luckily, this hiker has an instrument to measure steepness. However, it takes time to do a single measurement and they want to get down before sunset. So, this hiker needs to decide how frequently to stop and measure the steepness, so they still can get down before sunset.

Once we have our model we can run  inference  on it, which is when we prompt the model with text. For example, the prompt could be: “write a guest post for the Pragmatic Engineer.” This prompt then asks the model to  predict the next most likely token (word) . How ChatGPT works isn’t magic, and is worth understanding.  Like most people, my first reaction to trying out ChatGPT was that it felt  magical . I typed in questions and got answers that felt like they could have come from a human! ChatGPT works impressively well with human language, and has access to more information than any one human could handle. It’s also good at programming-related questions, and there was a point when I questioned if ChatGPT could be  more capable  than humans, even in areas like programming, where humans have done better, until now?

For a sense of ChatGPT’s limitations, you need to understand how it works. ChatGPT and other LLMs do not “think” and “understand” like humans. ChatGPT does, however, generate words based on what the next most likely word should be, looking at the input, and everything generated so far.

In an excellent  deepdive about how ChatGPT works , Stephen Wolfram – creator of the expert search engine  WolframAlph a – summarizes ChatGPT:

“The basic concept of ChatGPT is at some level rather simple. Start from a huge sample of human-created text from the web, books, etc. Then train a neural net to generate text that’s “like this”. And in particular, make it able to start from a “prompt” and then continue with text that’s “like what it’s been trained with”. As we’ve seen, the actual neural net in ChatGPT is made up of very simple elements—though billions of them. And the basic operation of the neural net is also very simple, consisting essentially of passing input derived from the text it’s generated so far “once through its elements” (without any loops, etc.) for every new word (or part of a word) that it generates. But the remarkable—and unexpected—thing is that this process can produce text that’s successfully “like” what’s out there on the web, in books, etc. (...) The specific engineering of ChatGPT has made it quite compelling. But ultimately (at least until it can use outside tools) ChatGPT is “merely” pulling out some “coherent thread of text” from the “statistics of conventional wisdom” that it’s accumulated. But it’s amazing how human-like the results are.”

Since then, ChatGPT has continued to evolve, including its most recent development: access to its latest GPT-4 Turbo model for paid users.

ChatGPT and generative AI aren't a novelty anymore, but keeping track of what they can do can be a challenge as new abilities arrive. Most notably, OpenAI now provides easier access to anyone who wants to use it. It also lets anyone write custom AI apps called GPTs and share them on its own app store, while on a smaller scale ChatGPT can now speak its responses to you. OpenAI has been leading the generative AI charge , but it's hotly pursued by Microsoft, Google and startups far and wide.

What is ChatGPT?

ChatGPT is an online chatbot that responds to "prompts" -- text requests that you type. ChatGPT has countless uses . You can request relationship advice, a summarized history of punk rock or an explanation of the ocean's tides. It's particularly good at writing software, and it can also handle some other technical tasks, like creating 3D models .

ChatGPT is called a generative AI because it generates these responses on its own. But it can also display more overtly creative output like screenplays, poetry, jokes and student essays. That's one of the abilities that really caught people's attention.

Much of AI has been focused on specific tasks, but ChatGPT is a general-purpose tool. This puts it more into a category like a search engine.

That breadth makes it powerful but also hard to fully control. OpenAI has many mechanisms in place to try to screen out abuse and other problems, but there's an active cat-and-mouse game afoot by researchers and others who try to get ChatGPT to do things like offer bomb-making recipes.

ChatGPT really blew people's minds when it began passing tests. For example, AnsibleHealth researchers reported in 2023 that " ChatGPT performed at or near the passing threshold " for the United States Medical Licensing Exam, suggesting that AI chatbots "may have the potential to assist with medical education, and potentially, clinical decision-making."

We're a long way from fully fledged doctor-bots you can trust, but the computing industry is investing billions of dollars to solve the problems and expand AI into new domains like visual data too. OpenAI is among those at the vanguard. So strap in, because the AI journey is going to be a sometimes terrifying, sometimes exciting thrill.

What's ChatGPT's origin?

Artificial intelligence algorithms had been ticking away for years before ChatGPT arrived. These systems were a big departure from traditional programming, which follows a rigid if-this-then-that approach. AI, in contrast, is trained to spot patterns in complex real-world data. AI has been busy for more than a decade screening out spam, identifying our friends in photos, recommending videos and translating our Alexa voice commands into computerese.

A Google technology called transformers helped propel AI to a new level, leading to a type of AI called a large language model, or LLM. These AIs are trained on enormous quantities of text, including material like books, blog posts, forum comments and news articles. The training process internalizes the relationships between words, letting chatbots process input text and then generate what it believes to be appropriate output text. 

A second phase of building an LLM is called reinforcement learning through human feedback, or RLHF. That's when people review the chatbot's responses and steer it toward good answers or away from bad ones. That significantly alters the tool's behavior and is one important mechanism for trying to stop abuse.

OpenAI's LLM is called GPT, which stands for "generative pretrained transformer." Training a new model is expensive and time consuming, typically taking weeks and requiring a data center packed with thousands of expensive AI acceleration processors. OpenAI's latest LLM is called GPT-4 Turbo . Other LLMs include Google's Gemini (formerly called Bard), Anthropic's Claude and Meta's Llama .

ChatGPT is an interface that lets you easily prompt GPT for responses. When it arrived as a free tool in November 2022, its use exploded far beyond what OpenAI expected.

When OpenAI launched ChatGPT, the company didn't even see it as a product. It was supposed to be a mere "research preview," a test that could draw some feedback from a broader audience, said ChatGPT product leader Nick Turley. Instead, it went viral, and OpenAI scrambled to just keep the service up and running under the demand.

"It was surreal," Turley said. "There was something about that release that just struck a nerve with folks in a way that we certainly did not expect. I remember distinctly coming back the day after we launched and looking at dashboards and thinking, something's broken, this couldn't be real, because we really didn't make a very big deal out of this launch."

ChatGPT, a name only engineers could love, was launched as a research project in November 2022, but quickly caught on as a consumer product.

How do I use ChatGPT?

The ChatGPT website is the most obvious method. Open it up, select the LLM version you want from the drop-down menu in the upper left corner, and type in a query.

As of April 1, OpenAI is allowing consumers to use ChatGPT without first signing up for an account. According to a blog post , the move was meant to make the tool more accessible. OpenAI also said in the post that as part of the move, it's introducing added content safeguards, blocking prompts in a wider range of categories.

However, users with accounts will be able to do more with the tool, such as save and review their history, share conversations and tap into features like voice conversations and custom instructions.

OpenAI in 2023 released a ChatGPT app for iPhones and for Android phones . In February, ChatGPT for Apple Vision Pro arrived , too, adding the chatbot's abilities to the "spatial computing" headset. Be careful to look for the genuine article, because other developers can create their own chatbot apps that link to OpenAI's GPT.

In January, OpenAI opened its GPT Store , a collection of custom AI apps that focus ChatGPT's all-purpose design to specific jobs. A lot more on that later, but in addition to finding them through the store you can invoke them with the @ symbol in a prompt, the way you might tag a friend on Instagram.

Microsoft uses GPT for its Bing search engine, which means you can also try out ChatGPT there.

ChatGPT is sprouting up in various hardware devices, including Volkswagen EVs , Humane's voice-controlled AI pin and the squarish Rabbit R1 device .

How much does ChatGPT cost?

It's free, though you have to set up an account to take advantage of all of its features.

For more capability, there's also a subscription called ChatGPT Plus that costs $20 per month that offers a variety of advantages: It responds faster, particularly during busy times when the free version is slow or sometimes tells you to try again later. It also offers access to newer AI models, including GPT-4 Turbo . OpenAI said it has improved capabilities in writing, math, logical reasoning and coding in this model.

The free ChatGPT uses the older GPT-3.5, which doesn't do as well on OpenAI's benchmark tests but which is faster to respond. The newest variation, GPT-4 Turbo, arrived in late 2023 with more up-to-date responses and an ability to ingest and output larger blocks of text.

ChatGPT is growing beyond its language roots. With ChatGPT Plus, you can upload images, for example, to ask what type of mushroom is in a photo.

Perhaps most importantly, ChatGPT Plus lets you use GPTs.

What are these GPTs?

GPTs are custom versions of ChatGPT from OpenAI, its business partners and thousands of third-party developers who created their own GPTs.

Sometimes when people encounter ChatGPT, they don't know where to start. OpenAI calls it the "empty box problem." Discovering that led the company to find a way to narrow down the choices, Turley said.

"People really benefit from the packaging of a use case -- here's a very specific thing that I can do with ChatGPT," like travel planning, cooking help or an interactive, step-by-step tool to build a website, Turley said.

Think of GPTs as OpenAI trying to make the general-purpose power of ChatGPT more refined the same way smartphones have a wealth of specific tools. (And think of GPTs as OpenAI's attempt to take control over how we find, use and pay for these apps, much like Apple has a commanding role over iPhones through its App Store.)

What GPTs are available now?

OpenAI's GPT store now offers millions of GPTs , though as with smartphone apps, you'll probably not be interested in most of them. A range of GPT custom apps are available, including AllTrails personal trail recommendations , a Khan Academy programming tutor , a Canva design tool , a book recommender , a fitness trainer , the laundry buddy clothes washing label decoder, a music theory instructor , a haiku writer and the Pearl for Pets for vet advice bot .

One person excited by GPTs is Daniel Kivatinos, co-founder of financial services company JustPaid . His team is building a GPT designed to take a spreadsheet of financial data as input and then let executives ask questions. How fast is a startup going through the money investors gave it? Why did that employee just file a $6,000 travel expense?

JustPaid hopes that GPTs will eventually be powerful enough to accept connections to bank accounts and financial software, which would mean a more powerful tool. For now, the developers are focusing on guardrails to avoid problems like hallucinations -- those answers that sound plausible but are actually wrong -- or making sure the GPT is answering based on the users' data, not on some general information in its AI model, Kivatinos said.

Anyone can create a GPT, at least in principle. OpenAI's GPT editor walks you through the process with a series of prompts. Just like the regular ChatGPT, your ability to craft the right prompt will generate better results.

Another notable difference from regular ChatGPT: GPTs let you upload extra data that's relevant to your particular GPT, like a collection of essays or a writing style guide.

Some of the GPTs draw on OpenAI's Dall-E tool for turning text into images, which can be useful and entertaining. For example, there is a coloring book picture creator , a logo generator and a tool that turns text prompts into diagrams like company org charts. OpenAI calls Dall-E a GPT.

How up to date is ChatGPT?

Not very, and that can be a problem. For example, a Bing search using ChatGPT to process results said OpenAI hadn't yet released its ChatGPT Android app. Search results from traditional search engines can help to "ground" AI results, and indeed that's part of the Microsoft-OpenAI partnership that can tweak ChatGPT Plus results.

GPT-4 Turbo, announced in November, is trained on data up through April 2023. But it's nothing like a search engine whose bots crawl news sites many times a day for the latest information.

Can you trust ChatGPT responses?

No. Well, sometimes, but you need to be wary.

Large language models work by stringing words together, one after another, based on what's probable each step of the way. But it turns out that LLM's generative AI works better and sounds more natural with a little spice of randomness added to the word selection recipe. That's the basic statistical nature that underlies the criticism that LLMs are mere "stochastic parrots" rather than sophisticated systems that in some way understand the world's complexity.

The result of this system, combined with the steering influence of the human training, is an AI that produces results that sound plausible but that aren't necessarily true. ChatGPT does better with information that's well represented in training data and undisputed -- for instance, red traffic signals mean stop, Plato was a philosopher who wrote the Allegory of the Cave , an Alaskan earthquake in 1964 was the largest in US history at magnitude 9.2.

We humans interact with AI chatbots by writing prompts -- questions or statements that seek an answer from the information stored in the chatbot's underlying large language model. 

When facts are more sparsely documented, controversial or off the beaten track of human knowledge, LLMs don't work as well. Unfortunately, they sometimes produce incorrect answers with a convincing, authoritative voice. That's what tripped up a lawyer who used ChatGPT to bolster his legal case only to be reprimanded when it emerged he used ChatGPT fabricated some cases that appeared to support his arguments. "I did not comprehend that ChatGPT could fabricate cases ," he said, according to The New York Times.

Such fabrications are called hallucinations in the AI business.

That means when you're using ChatGPT, it's best to double check facts elsewhere.

But there are plenty of creative uses for ChatGPT that don't require strictly factual results.

Want to use ChatGPT to draft a cover letter for a job hunt or give you ideas for a themed birthday party? No problem. Looking for hotel suggestions in Bangladesh? ChatGPT can give useful travel itineraries , but confirm the results before booking anything.

Is the hallucination problem getting better?

Yes, but we haven't seen a breakthrough.

"Hallucinations are a fundamental limitation of the way that these models work today," Turley said. LLMs just predict the next word in a response, over and over, "which means that they return things that are likely to be true, which is not always the same as things that are true," Turley said.

But OpenAI has been making gradual progress. "With nearly every model update, we've gotten a little bit better on making the model both more factual and more self aware about what it does and doesn't know," Turley said. "If you compare ChatGPT now to the original ChatGPT, it's much better at saying, 'I don't know that' or 'I can't help you with that' versus making something up."

Hallucinations are so much a part of the zeitgeist that Dictionary.com touted it as a new word it added to its dictionary in 2023.

Can you use ChatGPT for wicked purposes?

You can try, but lots of it will violate OpenAI's terms of use , and the company tries to block it too. The company prohibits use that involves sexual or violent material, racist caricatures, and personal information like Social Security numbers or addresses.

OpenAI works hard to prevent harmful uses. Indeed, its basic sales pitch is trying to bring the benefits of AI to the world without the drawbacks. But it acknowledges the difficulties, for example in its GPT-4 "system card" that documents its safety work.

"GPT-4 can generate potentially harmful content, such as advice on planning attacks or hate speech. It can represent various societal biases and worldviews that may not be representative of the user's intent, or of widely shared values. It can also generate code that is compromised or vulnerable," the system card says. It also can be used to try to identify individuals and could help lower the cost of cyberattacks.

Through a process called red teaming, in which experts try to find unsafe uses of its AI and bypass protections, OpenAI identified lots of problems and tried to nip them in the bud before GPT-4 launched. For example, a prompt to generate jokes mocking a Muslim boyfriend in a wheelchair was diverted so its response said, "I cannot provide jokes that may offend someone based on their religion, disability or any other personal factors. However, I'd be happy to help you come up with some light-hearted and friendly jokes that can bring laughter to the event without hurting anyone's feelings."

Researchers are still probing LLM limits. For example, Italian researchers discovered they could use ChatGPT to fabricate fake but convincing medical research data . And Google DeepMind researchers found that telling ChatGPT to repeat the same word forever eventually caused a glitch that made the chatbot blurt out training data verbatim. That's a big no-no, and OpenAI barred the approach .

LLMs are still new. Expect more problems and more patches.

And there are plenty of uses for ChatGPT that might be allowed but ill-advised. The website of Philadelphia's sheriff published more than 30 bogus news stories generated with ChatGPT .

What about ChatGPT and cheating in school?

ChatGPT is well suited to short essays on just about anything you might encounter in high school or college, to the chagrin of many educators who fear students will type in prompts instead of thinking for themselves.

Microsoft CEO Satya Nadella touted his company's partnership with OpenAI at a November 2023 event for OpenAI developers. Microsoft uses OpenAI's GPT large language model for its Bing search engine, Office productivity tools and GitHub Copilot programming assistant.

ChatGPT also can solve some math problems, explain physics phenomena, write chemistry lab reports and handle all kinds of other work students are supposed to handle on their own. Companies that sell anti-plagiarism software have pivoted to flagging text they believe an AI generated.

But not everyone is opposed, seeing it more like a tool akin to Google search and Wikipedia articles that can help students.

"There was a time when using calculators on exams was a huge no-no," said Alexis Abramson, dean of Dartmouth's Thayer School of Engineering. "It's really important that our students learn how to use these tools, because 90% of them are going into jobs where they're going to be expected to use these tools. They're going to walk in the office and people will expect them, being age 22 and technologically savvy, to be able to use these tools."

ChatGPT also can help kids get past writer's block and can help kids who aren't as good at writing, perhaps because English isn't their first language, she said.

So for Abramson, using ChatGPT to write a first draft or polish their grammar is fine. But she asks her students to disclose that fact.

"Anytime you use it, I would like you to include what you did when you turn in your assignment," she said. "It's unavoidable that students will use ChatGPT, so why don't we figure out a way to help them use it responsibly?"

Is ChatGPT coming for my job?

The threat to employment is real as managers seek to replace expensive humans with cheaper automated processes. We've seen this movie before: elevator operators were replaced by buttons, bookkeepers were replaced by accounting software, welders were replaced by robots. 

ChatGPT has all sorts of potential to blitz white-collar jobs. Paralegals summarizing documents, marketers writing promotional materials, tax advisers interpreting IRS rules, even therapists offering relationship advice.

But so far, in part because of problems with things like hallucinations, AI companies present their bots as assistants and "copilots," not replacements.

And so far, sentiment is more positive than negative about chatbots, according to a survey by consulting firm PwC. Of 53,912 people surveyed around the world, 52% expressed at least one good expectation about the arrival of AI, for example that AI would increase their productivity. That compares with 35% who had at least one negative thing to say, for example that AI will replace them or require skills they're not confident they can learn.

How will ChatGPT affect programmers?

Software development is a particular area where people have found ChatGPT and its rivals useful. Trained on millions of lines of code, it internalized enough information to build websites and mobile apps. It can help programmers frame up bigger projects or fill in details.

One of the biggest fans is Microsoft's GitHub , a site where developers can host projects and invite collaboration. Nearly a third of people maintaining GitHub projects use its GPT-based assistant, called Copilot, and 92% of US developers say they're using AI tools .

"We call it the industrial revolution of software development," said Github Chief Product Officer Inbal Shani. "We see it lowering the barrier for entry. People who are not developers today can write software and develop applications using Copilot."

It's the next step in making programming more accessible, she said. Programmers used to have to understand bits and bytes, then higher-level languages gradually eased the difficulties. "Now you can write coding the way you talk to people," she said.

And AI programming aids still have a lot to prove. Researchers from Stanford and the University of California-San Diego found in a  study of 47 programmers  that those with access to an OpenAI programming help " wrote significantly less secure code  than those without access."

And they raise a variation of the cheating problem that some teachers are worried about: copying software that shouldn't be copied, which can lead to copyright problems. That's why Copyleaks, a maker of plagiarism detection software, offers a tool called the  Codeleaks Source Code AI Detector  designed to spot AI-generated code from ChatGPT, Google Gemini and GitHub Copilot. AIs could inadvertently copy code from other sources, and the latest version is designed to spot copied code based on its semantic structures, not just verbatim software.

At least in the next five years, Shani doesn't see AI tools like Copilot as taking humans out of programming.

"I don't think that it will replace the human in the loop. There's some capabilities that we as humanity have -- the creative thinking, the innovation, the ability to think beyond how a machine thinks in terms of putting things together in a creative way. That's something that the machine can still not do."

About 1 in 5 U.S. teens who’ve heard of ChatGPT have used it for schoolwork

Roughly one-in-five teenagers who have heard of ChatGPT say they have used it to help them do their schoolwork, according to a new Pew Research Center survey of U.S. teens ages 13 to 17. With a majority of teens having heard of ChatGPT, that amounts to 13% of all U.S. teens who have used the generative artificial intelligence (AI) chatbot in their schoolwork.

Teens in higher grade levels are particularly likely to have used the chatbot to help them with schoolwork. About one-quarter of 11th and 12th graders who have heard of ChatGPT say they have done this. This share drops to 17% among 9th and 10th graders and 12% among 7th and 8th graders.

There is no significant difference between teen boys and girls who have used ChatGPT in this way.

The introduction of ChatGPT last year has led to much discussion about its role in schools , especially whether schools should integrate the new technology into the classroom or ban it .

Pew Research Center conducted this analysis to understand American teens’ use and understanding of ChatGPT in the school setting.

The Center conducted an online survey of 1,453 U.S. teens from Sept. 26 to Oct. 23, 2023, via Ipsos. Ipsos recruited the teens via their parents, who were part of its KnowledgePanel . The KnowledgePanel is a probability-based web panel recruited primarily through national, random sampling of residential addresses. The survey was weighted to be representative of U.S. teens ages 13 to 17 who live with their parents by age, gender, race and ethnicity, household income, and other categories.

This research was reviewed and approved by an external institutional review board (IRB), Advarra, an independent committee of experts specializing in helping to protect the rights of research participants.

Here are the  questions used for this analysis , along with responses, and its  methodology .

Teens’ awareness of ChatGPT

Overall, two-thirds of U.S. teens say they have heard of ChatGPT, including 23% who have heard a lot about it. But awareness varies by race and ethnicity, as well as by household income:

  • 72% of White teens say they’ve heard at least a little about ChatGPT, compared with 63% of Hispanic teens and 56% of Black teens.
  • 75% of teens living in households that make $75,000 or more annually have heard of ChatGPT. Much smaller shares in households with incomes between $30,000 and $74,999 (58%) and less than $30,000 (41%) say the same.

Teens who are more aware of ChatGPT are more likely to use it for schoolwork. Roughly a third of teens who have heard a lot about ChatGPT (36%) have used it for schoolwork, far higher than the 10% among those who have heard a little about it.

When do teens think it’s OK for students to use ChatGPT?

For teens, whether it is – or is not – acceptable for students to use ChatGPT depends on what it is being used for.

There is a fair amount of support for using the chatbot to explore a topic. Roughly seven-in-ten teens who have heard of ChatGPT say it’s acceptable to use when they are researching something new, while 13% say it is not acceptable.

However, there is much less support for using ChatGPT to do the work itself. Just one-in-five teens who have heard of ChatGPT say it’s acceptable to use it to write essays, while 57% say it is not acceptable. And 39% say it’s acceptable to use ChatGPT to solve math problems, while a similar share of teens (36%) say it’s not acceptable.

Some teens are uncertain about whether it’s acceptable to use ChatGPT for these tasks. Between 18% and 24% say they aren’t sure whether these are acceptable use cases for ChatGPT.

Those who have heard a lot about ChatGPT are more likely than those who have only heard a little about it to say it’s acceptable to use the chatbot to research topics, solve math problems and write essays. For instance, 54% of teens who have heard a lot about ChatGPT say it’s acceptable to use it to solve math problems, compared with 32% among those who have heard a little about it.
