Setup requirements and cost estimation

The setup cost of the lab purely depends on the services offered and investment you make in the business. The lesser the investment, lesser will be the number of patients for whom the service is provided. If there is more or enough investment to spend adequately on infrastructure, space, staff, machinery and software tools the higher will be incoming and outgoing of patients in your lab. From this, there is a clear view that these five parameters like infrastructure, space, staff, equipment and software tools decides the entire setup planning and organisation.

Infrastructure

The setup has to give focus to the infrastructure of the lab since this may help the staff work efficiently without any disruption. The availability of accessibility of various services of the lab should be well communicated to the patients who visit the lab for tests. There should be a very clear layout planned for various rooms and treatment areas so that the planned activities of each department are done without any hindrance.

Facilities like clean restrooms, lift services, wheel chairs, and other basic elements of a hospital should be present in the pathology lab too. Apart from these, the level of cleanliness should always be kept high. Since the sector is related to medical activities, even a minor deviation from healthy surrounding will lead to serious problems for both the patients and the staff working there.

The staff selected to work in the lab should meet the set standard qualifications for respective designations. The procurement phase has to be expensive with rounds of screening, so that the resulting staff are efficient enough to handle the complex problems in the medical field.

The lab technicians are also appointed to have a thorough control and knowledge over the machines used in the lab. They should be provided with training and induction during the initial stages of their appointment. This is basically to reduce the errors they make and to make them adapt to the methods of the lab.

Uniformity in work and operations of the lab will be achieved if training is given. The appointed staff should be vaccinated with necessary medicines to safeguard them from the communicable diseases of the patients.

The staff appointed should frequently be assisted by a medical representative who will perform the activation of updating the staff with changing techniques. The medical representative will be informing you and the staff regularly about the dynamic conditions in medical field. New innovations, new medicines, and new techniques will be informed so that you make necessary changes in the methods of finding results.

Minimum space requirement should be to fit hundred patients and their respective attenders. Patients should not be left to merge and sit together or have a congested environment due to insufficient space. Maximum attention should be given for space so that the related factor infrastructure also turns out to be an efficient one. Space planning is necessary to place things right at its place. Clubbing of two three departments performing various activities should not happen since they may result in misplaced results or any other undesirable condition.

Software tools

It would be a great support if you can carry out the activities of the lab in a computerised manner. This will eliminate discrepancy in results and will be an efficient time-saving opportunity. Updating each patients file with the results, sending them reminders, having a record of their medical history will be possible with strong software tools and a staff with adequate knowledge in this field. Apart from serving the patients with software tools, there is a need for these tools in supporting the lab work too. Diagnosis will also require efficient systems and programming to arrive at the results.

The equipment required for the pathology lab has a very different range of items in it. I would recommend to purchase them all by having a checklist since you might miss one or two important items. At times, even if your lab is highly rated in other four factors like staff, space, infrastructure and software, you will be rated low if you are running short of equipment in your lab.

Having a diverse range of testing tools is the only way to make your lab more productive to the patients coming in. The basic expectation of any patient will be the availability of all the required services at one place. To ensure that, you will have to procure an install the necessary items. Of this, the most important part is the safety aspect of the purchased equipment.

All of it should be revised as and when required or even on the basis of a scheduled time. This will considerably maintain the working life of the machine and facilitates less expensive maintenance costs. Guidelines for ultrasound owners and owners of clinics, diagnostic centres, nursing homes and hospitals

These are guidelines that are required for ultrasound owners for registration and use of ultrasound machines and are applicable across India. A brief description of the violations and penalties has also been listed.

Registration and Use of Machines

  • The owner needs to register, at least 3 months in advance , all the places where a USG machine, whether a black and white or color Doppler machine, is to be used; this applies to medical/surgical nursing homes/intensive cardiac care unit and wherever a USG machine is used. This registration is compulsory and is regardless of the type of studies intended to be performed on that machine, whether pregnancy scans, abdominal scans, or only 2D echocardiograms. All centers should be registered.
  • All radiologists/sonologists/doctors using USG machines need to have their names entered in the PC-PNDT certificate or on a separate sheet that must be displayed along with the PC-PNDT registration certificate. These doctors should be qualified to perform ultrasound according to their degree/ diploma certificate.
  • No doctor other than the one registered in that particular center may use the machine installed there. Other qualified doctors may use the machine at that Centre only after notifying the appropriate authority (AA). In the case of a locum doctor, the doctor's name and personal details, state council registration number, and his/her degree certificate need to be sent to the AA/ward office, accompanied by a covering letter from the clinic owner requesting permission for the use of the machine by the locum on the specified dates.
  • The USG machine should not be shifted from one Centre to another. One center can have multiple USG machines, but the manufacturer's name and the makes and models of the machines should be noted in the PC-PNDT registration certificate (all ultrasound machines available at that center should be registered on the certificate or on a separate attached sheet).
  • Any changes in the center (e.g., change in machine or place of use) should be intimated to the AA 30 days in advance. Any change of the doctor operating the machine should be intimated to the AA within 30 days. A recent Gazette notification has stated that application for locum to adding a doctor's name to the Registration Certificate needs to be done 30 days in advance; this gazette has been challenged by IRIA in Delhi High Court, and interim relief has been given of 7 days.

Record Keeping and Information to be displayed at the Clinic

  • All records (including images) of all the patients are to be maintained for a minimum of 2 years. If any legal case against the Centre is pending then the concerned records should be maintained till the same case is disposed of.
  • The PC-PNDT registration certificate has to be displayed in original in the reception area and the USG room. If there are more than two rooms where sonography is performed, the original has to be displayed in both rooms as well as the reception area (request for more originals can be submitted to the AA/ward office).
  • There should be a board, prominently displayed, with the message in English and the local language that ‘sex selection and detection is not done in this Centre and is punishable under the PC-PNDT Act’. This should be displayed in the waiting area and the USG room. No specific size for the message is suggested.
  • A copy of the PC-PNDT Act should be maintained in the center and needs to be displayed in the waiting room/reception area as well as in the USG room.
  • Form ‘D’ is to be maintained by genetic counseling Centres only.
  • Form ‘E’ is to be maintained by genetic laboratories.
  • All columns in form ‘F’ should be filled up; no column should be left blank, write “No” or “Not Applicable” wherever required
  • Previous obstetric history of patients, with number of children and the sex of each child should be mentioned in form ‘F.’
  • Form ‘F’ is to be filled up in duplicate: one copy is to be given to the AA (i.e., Medical Officer of Health of the ward) and one copy has to be kept at the clinic (records to be kept for 2 years). In the case of online submission of form ‘F,’ either a printout of the form with the patient's signature or another form ‘F’ has to be maintained at the center (i.e., a hard copy has to be preserved at the clinic when form ‘F’ is submitted online.)
  • The name of the referring doctor and the indications and results of the USG examination must be mentioned in the form.
  • Form ‘F’ should be signed by the doctor conducting the sonographic examination and not by the owner of the center.
  • As per the Act, before the USG examination, a full explanation of the procedure must be given to the patient in her own language and a declaration of consent must be obtained, signed by both the patient and the doctor.
  • All blank areas should be labeled as ‘Not Applicable’ / ‘Nil’.
  • A monthly report should be sent to the AA by the 5 th of the month by all PC-PNDT-registered centers. (This is not needed if form ‘F’ is filled up online.).

  • Application form ‘A’ is to be submitted in duplicate, along with the documents of the USG machine and an undertaking, duly notarized, stating sex selection and sex detection will not be done at the center. The undertaking should also state that the center will display a message in English and the local language that sex selection and detection are not done at the center and that the same are punishable under the Act.

Miscellaneous

  • Advertisement of any sort is banned and is punishable under the Act with a fine of up to Rs.1 Lakh and/or imprisonment for 5 years.
  • The sex of the fetus should not be disclosed to the patient, relatives, referring doctor, or anyone else.

1. How important is Form F?

Form F filing is mandatory for all pregnant patients. It goes without saying that it should be complete and without errors. The authority should be able to contact the patient on basis on this form. All columns need to be filled and if any points are not applicable, please write NA or Not Applicable against that column, do not leave it blank. Forms should be maintained properly so that one can easily access it whenever asked for

2. How does one apply for PC- pndt0 registration?

All details have been given in guidelines. The new amendment (5 June) states that an intimation of one month prior should be given before starting the clinic, adding new machines.

3. How does one renew PCPNDT registration?

All details have been given in guidelines. One should apply ideally 3 months before expiry of the current registration and certainly before one month of expiry.

4. Do MRI machines need to be registered?

Yes, they need to be registered and all changes to be notified to the AA

5. Do CT scan machines need to be registered?

6. what is the current status of video recording devices in maharashtra and across the country.

Video recording devices have not been installed in Maharashtra except in Kolhapur district. They are being installed in Rajasthan and in some pockets of Punjab. There is no order for their implementation, but they are being installed at local level by collectors.

7. What does one do when officials come for inspection. What are the papers required during the inspection?

One should be ready with all paperwork and form F copies. All certificates should be displayed in reception and suite area as mentioned in the guidelines. One needs to be co operative.

8. Is there a restriction on portable ultrasound devices ?

Based on the dismissal of writ filed in high court by Maharashtra State Branch of IRIA, ultrasound machine cannot be used as a portable device. Though it's use is not explicitly prohibited in the PC PNDT act, there are clauses to suggest that one cannot transport it from place to place, even though those centres may be individually registered.

9. Is there a restriction on the number of places a Sonologist can visit ?

As per the recent gazette dated 5 June 2012, a sonologist can visit only 2 places in one district. This gazette has been challenged in Delhi High Court by IRIA and ad-interim stay has been obtained. It has also been challenged in Mumbai High Court by Dr Rajeev Zankar, and ad-interim stay has been given until next hearing in October.

10. Can one arrange for a locum sonologist when one is on leave or vacation?

One has to give one month notice for the same to local AA with a letter, and copy of certificates of that doctor. This has been reduced to 7 days in the ad-interim relief given by Delhi High Court in July 2012, until the next hearing or there-after. However, it is probable that the 2 place rule as mentioned in the previous FAQ may also apply to the locum doctor.

11. Are training courses authorised by the Govt of India?

Medical Council of India has been given guidelines by the Central Supervisory Board in their meeting held in Mumbai in Jan 2012, to create a 6 month Level I training course in Ultrasound for MBBS doctors. This process is still a work in progress.

12. What should one do when renewal papers are not processed?

If papers are not processed in 90 days, the centre is considered registered. One must however still inform the authorities once again once the 90 day period is over through a written letter stating the same.

13. What are the steps one needs to take to deseal their machines?

If a show cause notice has been issued by authorities and the machine sealed, one needs to release the machine through the courts only.

This case outlines the dilemma of V. Raja, president and CEO of GE Healthcare India, when the company's ultrasound machines were implicated in many cases of prenatal sex determination. Even in the 21st century, Indian society favored males and many in India saw females as a burden on their families. Studies had shown that fewer and fewer girls were being born, with potentially catastrophic results for future Indian society. The reason: Many women were relying on ultrasound machines to determine the gender of their fetus and, if it were a girl, having abortions. Raja knew that GE Healthcare and ultrasound machines were providing much better medical care for Indians, particularly those in rural communities, and that the company was following all the rules and regulations to prevent this type of abuse. But he also understood the social issues that were involved. Ultrasound machine sales had enormous potential to help maintain GE's market-leading position in India. But should the company step back from its aggressive sales strategy? How could Raja and the company alleviate the growing discontent among critics and the media against the practice of prenatal sex determination testing using GE's ultrasound machines? What additional efforts did GE need to make to prove its intentions of promoting prenatal care? What other efforts should the company make to stop the illicit prenatal gender determination and resulting abortions? How could he protect the as yet untarnished image of GE as a responsible corporation going forward?

This incentivizes the physician client to schedule efficiently and fill the ultrasound calendar up.
  • Under This Model You Will Service The Client Based On A Pay Per Study Fee. You will list your rates in your contract for each exam. You will bill the physician client the following month for service issued during the previous month. You can charge fair market value for these studies. Under this model, you get paid only when there is a patient to scan. This model is better for the Doctor because they don’t pay you per hour. They only pay you when there is a study to be performed. It is important to compare and contrast the pros and cons before engaging in this model. This incentivizes the physician client to add more patients because of fixed overhead (paying your fee). You have to consider your time as well in this model, because – if there are no patients, you do not get paid.
  • Under This Model You Will Service The Client That Has Their Own Ultrasound Equipment In House. You will provide the staffing services of the equipment,  scanning services, and any related administrative or other agreed upon tasks for the client. This model is great because you bring a special skill set to the practice that can really improve ultrasound and imaging operations. Rates under this model can vary but $75 – $120 / hr is not uncommon.

This article is going to give you everything you need to know about building an imaging center so you can come to an informed conclusion on whether or not this endeavor is right for you.

Assessing Imaging Center Profitability

Before you start creating a vision and a marketing plan for a  new imaging center, you’ll want to do some research. For your radiology center business plan, Imaging center profitability may be on a long-term upward trend, but with healthcare reform and economic changes, there are no guarantees. Is it worth the risk? The only way to answer this is to do the research specific to your situation and location.

Starting an imaging center is not cheap.

You’ll have to put in a lot of money upfront and it can take time to start making a profit. Will you need investors, a business partner, or some loans? Crunching the numbers should be your first step.

Ask Yourself a Lot of  Questions

These are dozens of questions you will want to ask yourself in order to determine whether starting an imaging center is a good choice for you. Questions will help guide your research and planning into the financial and marketing aspects of opening an imaging center.

Consider The Location You’re Thinking About Opening an Imaging Center In

The number one rule of all real estate is location, location. location. Sounds easy but there some specific considerations to take into account when we are talking about where to open an imaging center.

Locations near physician’s offices and near the hospital are ideal.

Even if a test is not able to be performed immediately or even in the next few days, the patient likely lives or works nearby or at least is familiar with the area.

Pick a location that’s easy to find

If the patient cannot easily locate the radiology imaging center his or her physician recommended, it is the patient’s prerogative to go to any location he or she chooses.  Missed appointments mean your radiology marketing budget is taking a hit.

Look and Feel of the Place

Additionally, you’ll want to consider the overall appearance of the center both inside and out. If the building is old and the rooms are cramped, it could steer potential patients and referring physicians away from you.

Parking and Public Transportation to your Imaging Center

There must be enough parking spaces and many patients will need handicapped spaces. If the building is not free-standing, you must be cognizant of the types of businesses above your planned imaging center.

Ideally, your imaging center will be located within easy reach of public transportation. Otherwise, you are limiting your patient pool to those who have a vehicle.

MRI Specific Considerations

The MRI machine should be placed in an RF-shielded (radio-frequency) room.

The MRI space consists of three nested components:

  • An outer shell for structural support
  • A middle metallic RF-shield
  • An interior layer made of finish materials

It takes extra space for these items and they must be constructed properly. It can not be too close to moving traffic as that may interfere with the machine’s capabilities.

Steer Clear of ALL METAL

If metal is allowed in the room, catastrophe will ensue. Many times a “foyer” is constructed at the entrance to the MRI room with metal detectors to ensure there are no mistakes.

Consider The Noise of the MRI

The machine is noisy when in use and will possibly be a nuisance to any upstairs or even downstairs neighbors. If your facility is not in a stand-alone building you should assess the possibility of upsetting your neighbors. You also want to take into account any area of your facility that may be sound sensitive and make sure it’s not near any noisy equipment.

Additional MRI Considerations

Depending on where you are, the MRI may have to be a minimum distance away from moving vehicles as well. This includes lawnmowers and trash collection trucks when they are in use. 

CT & X-Ray Machines

The x-ray room and the CT room will have to be lead lined. There will be questions about storage and elimination of any chemicals that may be needed for any specialty machines. Newer machines may not be as “hazardous” as older machines so a decision concerning what radiographic machines to purchase may have to be done prior to the final plans for the building.

Installing The Equipment

Remember, the machines to be installed are more often than not very large and very heavy. They will not fit through a doorway. Often the outside wall of the building will have to be removed for the installation, especially for the MRI. After installation, the wall, as well as any required lining, will have to re-installed. The ceiling of the room also will have to be lined.

Start an imaging center: After deliberating and choosing a suitable location for your center, the hard work begins. Even if the building is already built, even as an imaging center, work will have to be done to the premises.

The layout of the facility should be designed by a professional in the area of imaging centers. Dimensions of rooms may vary wildly depending on the footprint of the proposed or existing building.

An MRI requires definitive parameters that a CT exam room or a room for a plain x-ray machine will not need. You’ll need to think about the width of the hallways and the location of the different modalities, as well as the location of the front desk and even the bathrooms.

Think your plans are done? Not till Uncle Sam says so!

After getting the plans drawn and approved by all of the stakeholders involved in the project, the process is just beginning. The county will have its say on the construction or the remodeling of the buildings. There will be details from the building department of your county that may also have specifications for the build of the center. They will go over every aspect in detail. You may think you have thought of every contingency but this is rarely the case.

Where are the exits on the building? Where will the fire extinguishers be placed? Don’t forget about those required pesky water fountains. What about sprinkler heads? Oh no, this may interfere with the MRI machine. You may also have to educate the authorities on the requirements of the machines you will be installing. It can be a difficult and tedious process.

Keeping Your Patients Happy

The entrance to your facility is very important. You need to be sure it makes all patients feel welcome and at ease.  Keeping your patients happy also means the look of the interior is crucial. An interior decorator may be utilized to get the perfect “feel” of the imaging center.

Think Beyond The Building

A beautiful facility is a great start, but before you need to think ahead. How will you attract top-notch employees? How can you get 5-star online reviews ethically right out the gate? Established competitors have already considered these points and if you want to succeed you cannot overlook them.

From the start, you should utilize a reputation management system to gather patient experience data and build your positive online reviews. Here’s how to build a positive online reputation right from the start.

Ready to Get Started?

Now that you have your plans in place, the land or building is purchased or at least under contract or option, it is time to begin . You may have thought that getting to this point was difficult but more work, the hard work, is about to begin.

You may have a contractor or you may have decided to handle the renovations or building yourself. It is not impossible and can be done. By now you know your design inside out. Your contractor should too but it is still your money. If mistakes are made, you are the one who will have to live with them.

Go to the site daily and check on everything.

From the site preparation to the actual building of the imaging center, if the site work is off and needs to be fixed at a later date, you will have to pay for it. Since you’re already paying a pretty penny, you’re not going to want to tack on extra costs because of a misunderstanding. Mistakes can be fixed, but always at a cost, and most often you will have to pay the bill.

Building a great staff for your imaging center takes time and effort. Consider hiring a chief technologist and let him or her interview prospective employees. Don’t wait for your building to be completed to begin this step. Relocating, leaving another job, and upcoming travel can all play a part in when your new employees can start, not to mention training time.

How to make $1000 a day in your mobile ultrasound business.

One of the very first "must-do" things when you're looking at starting any business is determining a) how much it costs to start and run and b) how much revenue you can bring in!

It can get really confusing when you start trying to figure out how to put everything together, and so I want to break this down for you, and show you how to come up with your potential revenue and what that looks like.

First things first, you need to understand how the business is set up. In our Business Academy , we teach our students to initially start their business using a service-fee model. In this setup scenario, the physician office bills insurance directly (because they are already credentialed and in-network) and the mobile ultrasound company charges the physician client a fee to provide the services in their office. This way, you don't have to waste months of time and energy AND purchase your equipment and be forking over the monthly payments before you are even credentialed. Insurance credentialing is confusing and time consuming, and did I mention the insurance groups can tell you they don't need anymore providers? So, now that you have a little heads up on how the business model looks, let's chat about revenue potential....

The average amount a mobile ultrasound company can charge is about $125/hour. We suggest setting out blocks of time at your client offices so you aren't driving around all over creation doing one ultrasound here and two ultrasounds there...when they know you're coming in every week on Tuesdays for example, the office books all their ultrasound patients into that particular time slot, you show up and provide the services.

Let's do a little math here. AT $125/hour, a half day (4hr) time slot is $500 in revenue and a full day (8hr) time slot is $1000 in revenue. This is then the total gross revenue back to your mobile ultrasound company for providing the services in their office. It will take some time to build up those accounts to 5 days per week, but the average monthly revenue back at full capacity then with an average of 20 working days a month is $20,000 per month. With such low overhead, this is really the way to start this business.

Different areas of the country will differ as far as the amount to charge, some will be a bit less and others a bit more, but $125 is a great average to do your initial revenue analysis on. And the best part about this setup is that due to Stark and Anti-Kickback laws, physicians are not legally able to make any profit off of patients they are referring out to the hospital or imaging center. So you're bringing in an opportunity for the physician to make a revenue off of something he or she is already ordering, while your company does 99% of the work. AND it saves the patient a ton of money as opposed to going to the hospital for their scan, so it's not a only a win/win, it's a win/win/win for all parties involved!

BUT it has to be set up correctly! The laws governing this setup are strict and there are a lot of them. You need to ensure your service contract is set up to carve out all of the exceptions in the laws that pertain to providing mobile ultrasound services inside a physician practice.

Samsung Medison to acquire French AI ultrasound startup Sonio for $92.7M

Samsung Medison, a medical device unit of Samsung Electronics that specializes in developing diagnostic imaging devices, said on Wednesday it plans to acquire  Sonio , a Paris-based startup that makes AI-powered software for ultrasound workflows, for  about $92.7 million (KRW 126 billion) .

The French startup’s AI assistant is aimed at helping obstetricians and gynecologists with the evaluation and documentation of ultrasound exams. It has received regulatory clearance in the United States (FDA 510(k)) for Sonio Detect, a product that uses deep learning algorithms to improve the image quality of ultrasound scans in real time.

Samsung Medison said Sonio’s software would help it bring better AI-driven imaging workflows to the market. Samsung Electronics, which owns a 68.45% stake in the medical device unit, acquired Medison for $22 million in 2011.

Samsung said in a statement that following the acquisition, Sonio will remain an independent company and continue to grow commercially and offer products and services in France.

Co-founded by Cecile Brosset (CEO) and Rémi Besson (CSO) in 2020, Sonio most recently secured $14 million in a  Series A led by Cross Border Impact Ventures in August 2023 . The company has raised a total of $27.2 million, according to  Tracxn , and its investors include Elaia, Bpifrance French Tech Seed, OneRagtime, and a few angel investors.

“Through the acquisition of Sonio, Samsung Medison will continue to deliver upon our promise to improve the quality of people’s lives with technology,” said Yong Kwan Kim, CEO of Samsung Medison. “Collaboration with Sonio will bring together best-in-class ultrasound AI technology and reporting capabilities to bring a paradigm shift in the prenatal ultrasound exam.”

“Samsung Medison’s established global ultrasound business combined with Sonio’s advanced AI creates an exciting growth opportunity for both sides,” said Brosset, CEO of Sonio. “We have found in Samsung Medison an amazing, trusting partner to pursue and accelerate our roadmap and mission. In addition to close collaboration with Samsung Medison, as an independent company, Sonio will continue to advance medical reporting technology and diagnostic software globally, including for underserved areas in healthcare.”

NEW DELHI (Reuters) - Indian software firm Zoho is planning a foray into chipmaking and seeking incentives from the federal government, two sources with direct knowledge of the proposal said, with one of them pegging the investment plan at $700 million.

Zoho, established in 1996 and now headquartered in India's Tamil Nadu, offers software and related services on subscription to businesses in 150 countries, competing with the likes of Microsoft and Salesforce.

It is the latest company to seek financial sweeteners from the government to set up a chip fabrication plant.

Semiconductors are a key plank of India's business agenda, with a $10 billion package in place to boost the industry as it hopes to compete with countries such as Taiwan in a few years.

Zoho is proposing to manufacture compound semiconductors, which have specialized commercial applications and are made from alternatives to the more-commonly used silicon in chipmaking, the two sources said.

The proposal is being reviewed by the panel that drives India's chip initiatives at the IT ministry, they added. The ministry has sought more clarity from Zoho on the customers it intends to do business with, the second source said.

Zoho declined to comment, while the IT ministry did not immediately respond to a request.

The first source, who said the company had estimated an investment outlay of $700 million, said Zoho had also identified a tech partner to help set up the operation from scratch, without naming the firm.

In March, founder and CEO Sridhar Vembu said Zoho was planning a chip design project in the southern state of Tamil Nadu, without providing further details. Its plans to diversify into chip manufacturing have not previously been reported.

Zoho made annual revenue of over $1 billion in the financial year ended March 2023, according to media reports.

In February, India gave the go-ahead to construction of three semiconductor plants worth over $15 billion by firms including Tata Group and CG Power, with plans to manufacture and package chips for sectors including defence, automobiles and telecommunication.

India has estimated its semiconductor market will be worth $63 billion by 2026.

McDonald's is reportedly trying to win back customers with a $5 meal deal this summer. But some diners are already lampooning the offer.

The fast-food chain plans to start offering the deal for about a month beginning June 25 at its US restaurants, The Wall Street Journal reported Wednesday. The meal is set to come with customers' choice of a McDouble or McChicken, as well as a small fry, four McNuggets, and a small soft drink, the Journal said.

While the deal isn't supposed to start for over a month, some consumers already say it isn't worth it.
