The Transfer Center team includes specially trained registered nurses and Transfer Center coordinators who are available 24 hours a day, seven days a week to coordinate transfers into UW Medicine hospitals from the entire WWAMI region (Washington, Wyoming, Alaska, Montana and Idaho) and beyond.

Step 1 - Send face sheet 

We need this demographic information to create an MRN and help our physicians with their decision to accept a patient as quickly as possible.

Step 2 - Send images 

Send images via PACS, Life Image, or secure email.

  • Images can also be sent via secure email (i.e., burns, hand, ophthalmology, complex wounds) 
  • File format should be .jpeg
  • Nothing over 10 megabytes

Step 3 - Call the Transfer Center 

When you call, one of our Transfer Center coordinators will complete an intake and remind you to fax a face sheet and send images as needed. We can help facilitate the transfer of clinical documentation to ensure continuity of care. Our transfer team can also help you coordinate air medical transport via Airlift Northwest . All calls to the Transfer Center are recorded for quality purposes. During the phone call with our coordinator, be prepared to provide the following information:

  • Patient's name and date of birth 
  • Referring physician's name and phone number 
  • Referring facility name and phone number 
  • Diagnosis, acuity and current clinical condition 
  • Requested treatment and specialty service 
  • Information on whether you are requesting a consult or transfer 
  • A patient who has an emergency medical condition (EMC) for which you are requesting transfer because your facility does not have the capacity or capabilities to provide the necessary stabilizing treatment for the EMC. 
  • A patient who has an EMC and they (or their legally responsible medical decision-maker) are requesting a transfer to another facility. 
  • An "inpatient" status patient at your facility (i.e., is currently admitted for inpatient care).

Step 4 - Clinical triage and discussion

Our Transfer Center team will connect the referring provider with the appropriate UW Medicine physician to discuss the patient's condition. Decisions to transfer are evaluated based on the required level of care, capacity, approval of the accepting physician and other factors. All calls to the Transfer Center are recorded for quality purposes.

Step 5 - Physical transfer of the patient

If accepted, our team will advise you when to set up transport. Please do not send a patient to UW Medicine until directed by our Transfer Center team. When transferring a patient, be sure to send the following physical copies:

  • Printed medical history and physical (H&P)
  • Printed discharge summary
  • Send only if images were not electronically sent

EMERGENCY TRANSFER

When air transport is necessary, we recommend the referring hospital use Airlift Northwest, but ultimately, we must abide by the transferring physician’s decision, as it is that person’s responsibility to decide what transport mode and service will be used.

Requests for air medical transport can be initiated by the UW Medicine Transfer Center and relayed to the Airlift Northwest Communications Center 800.426.2430 for immediate transport upon acceptance.

Through the Patient Transfer and Access Center, providers can now request a transfer from their acute care hospital to Mass General Brigham hospitals—all with a single phone call.

Ensuring easy access to the right care, at the right time, in the right place

With a single phone call, the Mass General Brigham Patient Transfer and Access Center (PTAC) works with providers to facilitate efficient acute care hospital transfers to either Brigham and Women’s Hospital, Massachusetts General Hospital, or one of our acute care community hospitals. An integrated and centralized hub, the PTAC is staffed all day, every day by a knowledgeable, experienced, and coordinated team of access, bed, and case management specialists who will provide a seamless patient transfer.

Our team will assess the patient’s health, condition, and needs and use real-time, systemwide capacity data to match them to the most appropriate level and location for inpatient care. On a case-by-case basis, our team may explore the option to transfer the patient back to their original hospital for recovery, once care is complete.

How to transfer a patient

For providers looking to transfer a patient, please call us at 1-855-468-6642 (1-855 GO TO MGB).

When you call the Mass General Brigham PTAC, our staff will:

  • Ask you to confirm your role and where you are calling from
  • Ask for the patient’s information, including their name, date of birth, medical record number, as well as diagnosis, diagnostic studies and lab results to assess what type of transfer is needed
  • Facilitate a phone call with a clinician

At the University of Chicago Medicine Transfer Center, specially trained registered nurses are available 24 hours a day, seven days a week to coordinate acute inter-facility transfers. We serve as a single point of contact for urgent and emergent transfer requests, thereby helping to expedite the admissions process.

To request to transfer an inpatient to UChicago Medicine, call the Transfer Center at 1-855-834-4782.

When requesting a patient transfer, the referring physician will need to supply the following information:

  • Contact information for the current attending physician
  • Current clinical condition
  • Current location of the patient
  • Medical service(s) required
  • Patient demographics, including full name and date of birth

How to Transfer a Patient

The nurse specialist who takes your call will initiate the acceptance process, which includes bed selection and the arrangement of transportation for critical transports. Learn more about the University of Chicago Aeromedical Network (UCAN).

During the process, a nurse specialist also will arrange for the referring physician to speak directly to a UChicago Medicine physician. Please note: Requests for transfer will be evaluated based on the patient’s clinical condition and the availability of resources.

Follow-Up Services

We aim to keep you informed of your patient’s status throughout the transfer process. Our nurse specialists will provide:

  • Updates on bed availability
  • Contact information for the accepting physician
  • The patient's nursing unit
  • The patient's arrival time

Nurses at the UChicago Medicine Transfer Center are available at any time, should questions arise during the transfer process.

UChicago CareLink: Eligible physicians may use CareLink to review their patients' medical information, place referral orders and communicate with UChicago Medicine providers.

Westchester Medical Center

Accessible 24-Hour Service

With just one phone call, Westchester Medical Center's full-service 24-hour One-Call Transfer Center makes the referral process easier and improves communication with referring physicians and institutions. When you call our toll-free number, you will have immediate access to the Transfer Center and to one of our on-site attending specialists at Westchester Medical Center.

To Transfer a Patient:

  • Call 1-866-GO TO WMC.
  • Have your patient's information ready, including name, condition, the referring physician's name and the hospital from which the patient is being referred.
  • You will be kept on the line while the Westchester Medical Center attending physician is paged.
  • The Transfer Center staff will facilitate a consult between you and the Medical Center's attending physician to discuss your patient's condition.

Once your patient is accepted to Westchester Medical Center, the Transfer Center will:

  • Assist with either ground or medevac transportation at your request.
  • Coordinate nursing unit and bed arrangements for your patient.
  • Facilitate the transmission of necessary paperwork from the referring hospital to Westchester Medical Center, so that only one "report" is required.
  • Notify you once the patient arrives at our hospital. The Transfer Center is available for both urgent and same-day admission transfers. This includes all transfers to our University Hospital, Maria Fareri Children's Hospital ,  Joel A. Halpern Trauma Center , and our adult and pediatric emergency departments.

If at any time you have a question or concern about the One-Call Transfer Center or any other associated transfer or transport issues, please call us at (914) 493-7000 and ask for the on-call Transfer Center coordinator.

To Refer a Patient:

To refer a patient to our Outpatient APS Gynecologic Oncology and Robotic Gynecologic Surgery, fax referral and clinical documentation stating the reason for the referral to 914.493.2322. Our office staff will contact Patient Transfer to schedule with one of our attendings.

University Hospitals Transfer Referral Center provides immediate and comprehensive round-the-clock services for adult and pediatric patients and serves as a central point of entry for transfers into University Hospitals Cleveland Medical Center and other system hospitals.

With just one phone call, our team of transfer liaisons will expedite the process and give you immediate access to our critical care transport teams, board-certified specialists and five Beacon Award-winning intensive care units. Transfer Services

We offer a full range of services to hospitals, medical centers and emergency departments looking to transfer patients, including:

  • Basic or advanced life support transports
  • Critical Care Transport by ground ambulance, helicopter, or fixed-wing aircraft

Clinical Capabilities

We treat a wide variety of conditions at UH Cleveland Medical Center and our community facilities, including:

  • Segment elevation myocardial infarction (STEMI)
  • Ischemic or hemorrhagic strokes
  • Intracranial hemorrhages including Epidural hematoma, Subdural hematoma and Subarachnoid hemorrhage
  • Aortic syndromes including aneurysm, dissection, intramural thrombus and penetrating atherosclerotic aortic ulcer

Critical Care

Our critical care transport team consists of a mobile intensive care unit (ICU), an ICU-trained nursing staff and a variety of technical capabilities, including:

  • Balloon pumps
  • Extracorporeal membrane oxygenation (ECMO) for respiratory and cardiac support
  • Isolettes for premature babies

The more than 300 full-time physicians within the UT Southwestern Pediatric Group constitute one of the largest pediatric groups in Texas. They provide care at UT Southwestern, Children’s Health, Parkland Memorial Hospital, Scottish Rite for Children, and Texas Health Resources.

Our dedicated transplant intake coordinators simplify the process for both physicians and patients, starting from the initial point of contact.

Depending on the patient’s needs, the transplant intake coordinators take primary responsibility for obtaining the necessary medical records and images, coordinating the initial insurance and financial screenings, and arranging scheduling for the patient’s first appointment.

UT Southwestern’s Harold C. Simmons Comprehensive Cancer Center is the only NCI-designated Comprehensive Cancer Center in North Texas and one of just five in the entire Southwest.

In addition, Simmons Cancer Center is among only 32 U.S. cancer research centers to be designated by the NCI as a National Clinical Trials Network Lead Academic Participating Site.

Physicians at UT Southwestern’s Spine Center work as a team with experts in nerve, muscle, and bone conditions, as well as nurses and physical therapists who work closely with patients to manage symptoms and reduce the risk of recurrence.

With one call, referring physicians can connect with all the specialists they need for spine care, such as experts from neurosurgery, orthopaedic surgery, pain management, spine imaging, spine rehabilitation, and anesthesiology.

Long-Haul COVID-19

UT Southwestern’s COVID Recover program – with locations in Dallas and Frisco – offers a combination of physical therapy to help patients regain strength and mobility, occupational therapy to improve activities of daily living, and speech therapy for cognitive rehabilitation. A four-week virtual group therapy program is also offered to help patients address psychological, sleep, and other issues.

Neuropsychologists and licensed professional counselors are available at each site for those who need more individualized attention. If needed, consultants have been identified in pulmonology, cardiology, neurology, and psychiatry for additional care.

At UVA Health System, we are continually evaluating our procedures to ensure we are providing optimal access to care. Your satisfaction with the patient transfer process has been a key consideration this year, and after careful review, we have made improvements we hope will allow for more streamlined service and a reliable, timely response to transfer requests.

For those requesting to transfer patients to UVA, the process will appear unchanged, with the exception of the phone number required. Behind the scenes, however, we have established our own internal response team within our Transfer Center. These 12 new personnel are medics — not call center operators — who have up to 30 years of experience in the medical field. This means they are better prepared to appropriately triage patients based on information provided. They can more easily detect patients who are emergent and they know which clinicians to contact for consultation.

“We now have Medic Transfer Coordinators working the transfer service 24-7,” says Transfer Center Manager Sarah Souter. “By asking key clinical questions, they can better guide calls to the appropriate service line, and can be a better monitor of patient conditions if changes occur.”

In addition, a new note section in Epic will allow UVA to record and track this information over time so that when a patient arrives, UVA’s team is well aware of their current status. “Now we have real-time clinical information that allows us to place patients appropriately from the start,” Souter says.

Because the transfer team will be co-located with the UVA Bed Center, there will also be greater transparency in terms of bed availability. “This will allow us to better estimate when exactly a bed will be available and provide a clear ETA to referring hospitals,” says Souter.
