It is said that well begun is half done. Choosing a thesis topic and submitting a research protocol is an essential step in the life cycle of a postgraduate resident. National Medical Commission of India mandates that all postgraduate trainees must submit at least one original research work (dissertation), one oral paper, one poster, and one publication to be eligible for final year examination. It is the duty of the faculty to ensure that trainees take active interest and submit their theses on time. However, their journey is often marred by multiple challenges and hurdles. The literature was searched from year 2000 onwards till 2011 using Pubmed, ResearchGate, MEDLINE, and the Education Resources Information Centre databases with terms related to residency training, selecting thesis topic, challenges or hurdles, and conversion of thesis into journal article. Existing literature on the subject matter is sparse. Current article advocates promotion of ethical and original research during postgraduation and proposes a checklist for residents before submission of their proposals.

INTRODUCTION

Residency is an extremely important period in the life cycle of a modern medical graduate. During this period, a resident learns to practice and acquire proficiency in a subject under guidance of a teacher. Along with acquiring new skills, it is also expected that they learn to critically analyze clinical scenarios and reach a rational conclusion. They are also expected to formulate and conduct original research which is submitted in form of a dissertation or thesis. Research work by a postgraduate should eventually translate into a scientific publication in a peer-reviewed medical journal, which helps in dissemination of thesis findings to the community and scientists. It is essential toward furthering medical knowledge, clinical practice, and the progression of science.[ 1 ] The National Medical Commission has stated the aims of completing this task as “Writing the thesis is aimed at contributing to the development of a spirit of inquiry, besides exposing the candidate to the techniques of research, critical analysis, acquaintance with the latest advances in medical science and the manner of identifying and consulting available literature.”[ 2 ]

CHOOSING THE TOPIC

The most intriguing question while conducting research is “How do I choose the right topic and will I be able to find the right answer?” Starting off with fire in the belly gives the best chance of seeing one’s work through. So, it is important to choose something that entices one’s mind and promises a gratifying result. Existing literature on the topic suggests that the journey of choosing the right topic is often marred by multiple challenges and dilemmas at various stages of this tumultuous journey. There are constraints of time, availability of resources, and support network.[ 3 ]

Therefore, students must remain open to suggestions from within and outside their minds. It is also important to allow the research area to simmer inside their mind for some time so that they can analyze various facets of the chosen area. It is at deeper layer of learning where higher order thinking skills such as analysis, synthesis, and evaluation reside. This then justifies the longer period it takes to arrive at a meaningful thesis title as it represents the highest order of cognitive reasoning referred to as “create” stage.[ 2 ] Short of this, novice researchers operate at lower order and remain “copy-paste” type of researchers.[ 4 ]

Area of interest

A journey into an area of one’s interest is bound to be fulfilling. It is a good idea to review one’s past works and experiences, which may be intriguing. Starting with an open and fertile mind promises novel ideas and helps to sustain long-term interest and enthusiasm.[ 6 ] Tendency to merely replicate similar studies should be avoided as they fail to ignite the zest for newer information.[ 7 ] Think about why you got into your field of study. From general to specific

A dissertation topic in medicine needs to be captivating and must intrigue the reader to look closer into the research work.[ 8 ] At the outset, it is a good strategy to just define a broad area and a dissertation topic need not be very specific or restrictive. The defined general area must be studied thoroughly and all its facets analyzed in detail. Look for gaps in knowledge which offer an avenue for research. For example, while studying factors responsible for relapse in alcohol dependence, doing a research on employment status of the spouse may be a good idea as it may not have been studied as extensively as other factors. It is needless to say that the student must first be familiar with the disease and all the variables which define its long-term trajectory. Medical science is an evolving field . There are factors of significance that can crop up during course of the study. Therefore, some scope for minor modifications must be kept for unexpected spinoffs. Most of the institutional review board permit minor revision of the protocols though they adhere to their own standards to safeguard interests of the patients. Authors conducted a survey and found that out of 184 submitted, 96 (52%) received requests for minor revision of research protocols. The acceptance resulted in further refinements in research methodology and outcomes.[ 9 ] Therefore, while submitting any protocol, some scope for minor change with probable reasons must be endorsed so that there are no complications while submitting final draft. After discussion with the guide, a suitable title can be given to the research proposal. Selection of the title should be such that it reflects the gist of the whole research and must attract attention of the reader. The title has a long shelf life and may be the first (and many a times, also the only) part of an article that readers see or read. Based on their understanding of the title, readers decide if the article is relevant to them or not.[ 8 ]

Do not bite more than you can chew

The average time allotted for completion of the MD/MS/DNB thesis is 2 years. It may be further reduced due to administrative delays like allotment of thesis guides and selection of topic. It is safe to assume that it takes around 1 month to finalize and submit the protocol and 2 months to write, print, and submit the complete thesis. That leaves just around year and nine months for actual and adequate data collection. All these facts must be kept in mind to ensure genuineness of data.[ 10 ]

A bird in hand is worth two in the bush

Modern medical science thrives on multispecialty approach, and it is not uncommon that students may end up with a research topic involving more than one department or more than one facility of the institution. Studies conducted during Covid pandemic are perfect examples owing to multiple facets of the illness in terms of prevention, pathophysiology, and long-term sequele.[ 11 ] A realistic check for the available resources in terms of infrastructure, availability of study materials, and support from affiliated departments must be done before finalizing the research topic. It is highly unlikely that your thesis is the first or the last research work in a particular area. Negotiating with other department/institution to regularly avail their facilities is often challenging. It is because of the difference in timing, priorities, work culture, and administrative barriers. One way to deal with it is to have a co-guide from that facility/institution. Dissertation reviewers have noticed that students often select topics that become unmanageable during course of their study. It can lead to development of stress and uncertainty about findings at the time of analysis. It was found that institutional support in terms of guidance, access to other departments, and statistical guidance improved overall performance of students and led to timely submission of thesis for publication in journals.[ 12 ]

Avoid controversy

Getting into controversy during initial years of residency is bound to raise stress levels and may dissuade the worker from continuing the research work. Field of medicine is fast evolving on the wheels of technology. Moral and ethical boundaries are slowly getting blurred. Many a times, laws are not revised and many laws are land specific. Therefore, it is a sound practice to familiarize oneself with existing laws and to take care that they are not violated. Central Drug Standard and Control Organization is the regulatory authority responsible for clinical trial oversight, approval, and inspections in India. It functions under Director General of Health Servicesand part of Ministry of Health and Family Welfare. The information is available online on their website and it is updated regularly. It is a good idea to visit the website and familiarize oneself about the existing laws before undertaking any research work. The website also gives information about National Ethical Guidelines for biomedical and health research involving human participants. Similarly, informed consent needs to be spelt out quite clearly and should be devoid of incomplete information or concealment of vital health related information. It is now mandatory that all research proposals be vetted by institutional ethical committee prior to submission to the university.[ 13 - 15 ]

Conformation with national health policy

Young medical professionals can contribute immensely by their research designs and valuable inputs in ratifying existing health-related measures or to suggest further refinements. This concept must always be kept in mind while formulating any research designs. Researchers of today are planners for tomorrow and their work is reflection of their goal toward health of the nation. In a comprehensive report, it was found that merely 0.5% of the 4230 thesis citations were quoted in policy decision.[ 16 ] The figures may be even lower for this country. The figures are abysmal compared to the magnitude of the research undertaken in centers of higher learning. The success of National Iodine Deficiency Disorder Control Program in India owes credit to sound scientific inquiries beginning in 1956. Despite stiff opposition and cultural bias, the program gained strength and helped in significantly reducing burden of iodine deficiency disorders.[ 17 ] The findings led to significant policy change and legislation supporting sale of only iodized salt in the country.

Scope for publication

Any research work is considered futile if it does not reach the stage of publication in a reputed journal. A genuine research must eventually translate into a research article. It has become increasingly difficult to translate thesis into a scientific publication in an indexed journal due to stringent standards and peer review. In a retrospective analysis of 85 theses, it was found that the conversion rate to peer-reviewed publication was 32.5%. The most common reasons for not publishing were a lack of originality and poor design. The authors further encouraged publication of full length articles as it helped residents in long term.[ 18 ] Originality of research, sound methodology, and analysis of data besides cogency in manuscript writing have been defining factors that promote acceptance of an article in a reputed journal.[ 19 ] Lure of quick publishing in a predatory journal can be damaging in the long run. Young and inexperienced authors publishing in a predatory journal must be aware of the damage of their reputation, of inadequate peer-review processes and that these journals might get closed any time for variety of reasons. Such publishing harms the scientific community in the long run, and hence such an approach is best avoided.[ 20 ] It is prudent practice to check whether an intended journal is predatory or not from the https://predatoryjournals.com/journals/or Beall’s list (https://beallslist.net/). Similarly, increasing the score by “salami” publication is unethical and should be avoided.

Familiarization with research methodology

Now imagine trying to assemble without the manual. It may be extremely difficult if not impossible to assemble the plane and is surely bound to take much longer. Research methodology is exactly like a manual for research. A major confounding factor in medical research is student’s conceptual understanding and comfort level with research methodology.[ 21 ] Findings indicate that there were noticeable differences in perspectives regarding what constitutes research methodology and its utility at least during the first year of residency.[ 21 ] Familiarizing with basic research methods is now mandated for all the medical postgraduates before they submit their research proposals, and free certificate online courses are available on their website. Writing a thesis during MD/MS and DNB courses, without having a correct research methodology planning, is practically impossible. Some of the prominent causes of rejection of submitted manuscripts are poor methodology, small sample size, and poor statistical analysis.[ 22 ] Furthermore, postgraduate students choose research methodology based on a number of factors such as familiarity with a method, methodological orientation of the primary supervisor, the domain of study, and the nature of research problems pursued. Participants reported key challenges that they faced in understanding research methodology include framing research questions, understanding the theory or literature and its role in shaping research outcomes, and difficulties in performing data analysis.[ 23 ]

Motivation level of the researcher

Dr APJ Abdul Kalam, former president of India, quoted that “Dream is not that you see in sleep but it is something that doesn’t let you sleep.” No research work will reach its logical conclusion till the time a researcher has strong motivation to pursue it. Another factor that defines sustained interest in thesis topic is motivation. As described by David Langford, there exists a continuum from extrinsic to intrinsic motivation.[ 24 ] Extrinsic motivation basically refers to a situation wherein the students are ordered (to study). As we move along this continuum, the quality of learning improves consistently with the maturing of the relationship between teacher and student. The culmination of the relationship occurs when the teacher becomes an enabler while the student becomes an active self-learner (intrinsic motivation). The process involves a definitive element of mentorship. In traditional Indian context, Gurukul envisages a firm and enduring relationship between “Guru” (teacher) and “Shishya” (student). Vedas in ancient times were combined with prepared commentaries in the form of “Upanisads.” The term upanisad refers to “Sitting down near a teacher in order to learn.” Though many students have inherent intrinsic motivation, a dynamic “Guru” can really shape the “Shishya.” Though the concept is old, it still remains relevant in modern times because learning medical practice is both art and science and best habits are still passed on to the next generation by trained and experienced teachers.[ 25 ]

WHAT CAN HELP POSTGRADUATE THESIS SELECTION?

The authors of this article put their minds together to devise a questionnaire that can act as a checklist for the residents before they actually submit their draft proposal for submission [ Table 1 ]. The checklist contains 10 questions and the responses can be marked from strongly disagree to strongly agree on a five-point likert scale. The checklist can be self-administered and the responses will give an insight into the lacunae. The residents can gradually work on these lacunae so that they feel at ease during the fantastic journey of scientific research and publication.

Choosing thesis topic and submitting the protocols is an important milestone in the career of a postgraduate resident. However, its importance cannot be undermined from the fact that it is usually the first scientific pursuit of a medical graduate. Challenges and hurdles are expected but can be overcome with sustained and systematic effort. Required Formatting and Components of the MD Thesis

In general, one topic is appropriate for the thesis, but it is recognized that some students may have performed several projects in parallel under the supervision of their mentor. If the student elects to include more than one study in the thesis, it is recommended that an attempt be made to integrate the topics into one coherent presentation. In rare cases where this goal cannot be achieved, it may be acceptable to divide the results section into different portions (or “chapters”). However, to be consistent with Yale formatting requirements, a thesis may not contain more than one abstract, introduction, statement of purpose, methods, results, and discussion section. It is not acceptable to submit a published or submitted manuscript in lieu of the thesis requirement. The formal thesis is presented as a digital document (PDF) during the graduation year. It must adhere to the following formatting and content requirements. These aspects of research are critical in making the work sound, error-free, and impactful for communities of interest. Recognizing that students may have uncertainly regarding how these guidelines relate to their thesis, OSR will be happy to answer any questions that may arise. Additionally, students may find it useful to consult the Equator reporting guidelines to enhance the quality and transparency of their theses. The Yale MD Thesis is a doctoral thesis that assesses research competencies as the basis for the conferring of a doctoral degree. Therefore, a minimum requirement of 30 pages (exclusive of title page, abstract, acknowledgements, table of contents, figure legends, references) is considered the minimum acceptable length.

  • Font: 10-12-point font should be used.
  • Line spacing: Body paragraphs and tables should consist of double-spaced text. Single spaced text may be used within block quotations, footnotes, and bibliography.
  • Margins: 1.5-inch margins on the left with one-inch margins on the remaining three sides. These margins apply to text, full-page images and illustrations, and tables.
  • Figures: Illustrative information, schematics, and representative data should be included as figures. Each panel in a figure should be labeled. Legends should describe each panel in detail in a 9-point font or greater and positioned below the figure to which they refer. Figure legends do not count towards the 30-page minimum.
  • Page Numbers: Each page in the thesis should be numbered except the title page, abstract, acknowledgements, and table of contents. The numbering should start at page 1 of the first page of the instruction be placed either at the top or bottom center, or at the top or bottom right-hand corner, at least 1/2 inches from any edge.
  • Minimal page requirement: Most Yale MD theses average 40-80 pages of text. A minimum of 30 pages of text excluding title page, abstract, acknowledgements, table of contents, figures, legends, and references is required. Tables may count towards the minimum page requirement.

Required Components

  • Title page: Title should not exceed 100 characters including spaces between words. The title page is not included in the 30-page minimum.
  • Abstract page: As described below. The abstract is not included in the 30-page minimum.
  • Acknowledgements: Personal and faculty acknowledgements, grant support, departmental support, etc. The acknowledgements page is not included in the 30 page minimum.
  • Table of Contents: with page numbers for each section. The table of contents is not included in the 30-page minimum.
  • Introduction: A thorough, complete, detailed, critical review of the literature that contextualizes and cites the work of previous investigators. This section should describe the state of the existing knowledge, provide rationale for the study, identify knowledge gaps, and frame the contribution of the thesis to medicine. The introduction is included in the 30-page minimum.
  • Statement of purpose: Specific hypothesis if appropriate, and specific aims of the thesis. The statement of purpose is included in the towards the 30-page minimum.
  • Student Contributions: Describe in detail exactly which procedures, methods and experiments were conducted by you and which procedures, methods and experiments, generation of data, or production of reagents, were performed by other members of the study team. It is not sufficient to state that this information may be mentioned elsewhere. It must be summarized here. It is recognized that students may often be completing a portion of a larger work. A statement detailing precisely what was done by the student and what was done by others does not detract from the thesis but is necessary for academic honesty.
  • Ethics Statement: Include information regarding the ethical and responsible conduct of research.
  • Human Subjects Research: If relevant, include explicit information regarding Institutional Review Board approval and informed consent. If the study had a waiver of consent, this exception must be clearly stated. Information regarding inclusion of historically vulnerable populations as research participants should be included here.
  • Laboratory Animals: For studies involving laboratory animals, include an explicit statement regarding study approval from the Institutional Animal Care and Use Committee. Include information regarding the species, strain, sex, and age of laboratory animals in this section as well.
  • Methods Description: Provide information regarding the materials and methods used in the study. Each method should consist of its subheading and paragraph and be described in detail that is sufficient to allow its replication by an investigator who did not participate in the study.
  • Statistical Methods: The last paragraph in this section should present the methods used to derive results. As needed, describe any data preprocessing such as transformation and normalization. Describe how outliers were defined and handled and present descriptive statistics as appropriate. The number of sampled units (ie, “n”) and significance (ie, “P”) should be reported for each statistical comparison. Continuous variables that are normally distributed may be presented as mean + standard deviation. Continuous variables that are asymmetrically distributed should be presented as median + interquartile range. All statistical tests should be clearly described and include information regarding testing level (alpha) and one- or two-sided comparisons. Corrections for multiple testing should be addressed and reported. Any novel or complex data algorithms should be clearly described and appropriately referenced.
  • Transparent reporting of results: All primary data related to the thesis topic should be presented. Important data should be enumerated in figures or tables. For ease of review, it is preferred that figures and tables be included in proximity to their callout in the text. Alternately, tables and figures can be presented separately after the discussion but, if possible, it is advantageous to the reader to include these components in the body of the results section, as occurs in research publications. The results section is included in the 30-page minimum.
  • Discussion: This section presents thorough and detailed interpretation and analysis of data, conclusions drawn, and framing of observations with the larger scientific literature. Limitations should be addressed, as should alternate interpretations and how the thesis may inform future studies in the field. Whenever relevant, a discussion of how the thesis may or already has meaningfully impact(ed) communities of interest should be included here. The discussion section is included in the 30-page minimum.
  • Challenges & Limitations : A brief discussion of methodologic, operational, and other challenges relevant to the research presented in the thesis. Please also include a brief discussion of how these challenges were addressed. Recognizing that all research projects have important limitations that readers should consider in interpreting the results, please include a brief discussion of the limitations relevant for your research. This section is included in the 30-page minimum.
  • Dissemination: Please include efforts made to share findings with the scientific community (through oral presentation, peer-reviewed publications, and other venues) and the larger community including patients. This section is included in the 30-page minimum.
  • Figure References and Legends: Figures must be cited sequentially in the text using Arabic numerals (for example, “Fig. 7”). Provide a short title (in the legend, not on the figure itself), explanation in sufficient detail to make the figure intelligible without reference to the text, and a key to any symbols used. Figures and legends are not included in the 30-page minimum.
  • Tables: All tables should be double-spaced, self-contained, and self-explanatory. Provide brief titles and use superscript capital letters starting from A and continuing in alphabetical order for footnotes. Tables and their legends are included in the 30-page minimum.
  • References: We strongly recommend the use of bibliography software such as Endnote for managing the references. References should be formatted according to New England Journal of Medicine Style. References are not included in the 30-page minimum.

It is acknowledged that theses in the area of medical humanities, ethics, history, and related fields may not obviously adhere to the above requirements. In this case, the thesis will likely replace the “hypothesis” with a “claim” based on evidence gleaned through literary, historical, and ethical research. The first paragraph of the Methods should still contain information about the student’s contribution. The subsequent paragraphs should describe the artistic, literary, or historical databases and methods used to gather the “evidence” presented in support of and contrary to the central claim. Abstract Page

Standardized format for the abstract of each MD thesis is required. This format must be followed for all abstracts published in the Yale Medicine Thesis Digital Library. These abstracts will not be reviewed for content. It is the responsibility of the student investigator and the faculty advisor to prepare the abstract. Faculty sponsors provide approval of the abstract when they approve the final version of the thesis. These abstract instructions are to be used for the digital library submission.

  • Abstracts should be formatted with 1.5-inch margins on the left and 1-inch margins on the remaining three sides. All text, including title page, must be double-spaced.
  • Abstracts may be no more than 800 words in length, not including title and author information.
  • Titles should be brief, clear, and carefully chosen. The title should not exceed 100 characters including spaces between words. Capitalize the entire title, using no abbreviations.
  • Authors’ names are to be written in full, omitting degrees. The student author's name shall be first. If the faculty sponsor also qualifies as an author, their name should be last. If the faculty member has been only a sponsor, his or her name should appear in parentheses after the name(s) of other authors as follows: "(Sponsored by...)". Other collaborators should be listed after the student's name and before the faculty sponsor's name. Immediately following the faculty sponsor's name, designate section (if any), departmental affiliation, institution, city, and state (Yale University School of Medicine, New Haven, CT) (see examples in the Thesis Guide ).
  • For thesis work performed at another institution, designate the senior author's departmental and institutional affiliation. In parenthesis, indicate the Yale faculty sponsor and institutional affiliation with the phrase: "Sponsored by..." (see examples).
  • A statement of the hypothesis or goals and specific aims of the study.
  • A statement of the methods used.
  • A summary of the results presented in sufficient detail to support the conclusions. Include actual values with statistics, if appropriate.
  • A statement of the conclusions reached.
  • Do not use subtitles, e.g., methods, results.
  • Do not include graphs, references to other publications, or acknowledgement of any research grant support. A single short table of results can be used if appropriate.
  • Abbreviations may be used in text only if defined initially by placing them in parentheses after the full word (or phrase) first appears in the text. Abbreviations may not be introduced in the title.
  • Non-proprietary (generic) names are required the first time a drug is mentioned, written in small letters. Proprietary names are always capitalized, e.g., acetazolamide (Diamox).
  • Completed abstracts must be approved by faculty advisor.

Thesis Assessment

The thesis assessment will assess student performance on the following domains using a 1-5 Likert Scale.

Likert Scale:

  • Not Acceptable

The vast majority of YSM students receive scores of 2-3 across these domains. Scores of 1 are considered truly outstanding. Scores of 5 are highly unusual and indicate the presence of critical deficiencies. Students receiving a score of “5” in any domain will be referred to OSR and the Progress Committee for remediation in order that they may stay on track to graduate.

Your MD Thesis represents an academic milestone. The protection of your thesis copyright exists from the time your work was created in digital form. Every article, book, or web page you used in conducting research and writing your thesis is also protected by copyright. When you download a single personal copy of a research article or cite a short passage, this scholarship is protected by the legal concept of fair use. A basic understanding of copyright protections and fair use is found at www.copyright.gov . Yale University also provides a guide to copyright protection and fair use: http://ogc.yale.edu/legal_reference/copyright.html .

Section 107 of the U.S. Copyright Law describes how to determine if a particular use of copyrighted material is fair. However, the distinction between what is fair use and what is infringement is not always clear or easily defined. Copying an image from an academic e-journal and citing the source does not substitute for obtaining permission to reproduce the image. Many publishers use www.copyright.com to grant reproduction rights of their articles to authors.

ProQuest/UMI and the Yale Medicine Thesis Digital Library may elect not to distribute your thesis in the absence of evidence that permission or reproduction rights have been secured. Providing evidence of permission or reproduction rights is a student author responsibility. Examples encountered in MD Thesis research that require documentation of reproduction rights include but are not limited to:

  • Sections of published survey instruments or questionnaires.
  • Complete journal articles or other complete scholarly works [Note: The publisher Elsevier does allow graduate student authors of a journal article prior to graduation to reproduce their article in a thesis].
  • Image, graphic, or pictorial works from publications where the author has transferred copyright to the publisher, a common occurrence.

The safest course to avoid a delay in thesis is to avoid using published images without obtaining permission. It is almost always possible to cite a source and expect that readers can find the figure, chart, or image in the published version of the referenced work. Research faculty who transfer copyright to a publisher of their article are no longer the copyright holder and are unable to grant permission for reproduction. To circumvent this issue, the student or mentor may be able to offer unpublished images from their group’s image collection.

When it is impracticable or prohibitively expensive to obtain permission through the publisher or the Copyright Clearance Center (www.copyright.com), students should avoid using that the material, unless they have obtained a written legal opinion that fair use would apply to the situation. Neither the Office of Student Research nor the Yale Library can supply legal advice on copyright and fair use. If there is any doubt, it is advisable to consult the Yale University Office of the Vice President and General Counsel at 432-4949. If a student is unable to obtain permission to use their previously published work in their MD thesis, they may place a permanent embargo on their thesis at the time it is uploaded into the Yale Medicine Digital Thesis Library (see below).

Upon receiving notification that the MD thesis has been approved as meeting graduation requirements, students should upload the thesis to the Yale Medicine Digital Thesis Library.

Yale Medicine Digital Thesis Library:

Starting with the YSM class of 2002, the Cushing/Whitney Medical Library and OSR have collaborated on the Yale Medicine Digital Thesis Library (YMTDL) project, publishing the digitized full text of medical student theses as a durable product of Yale student research efforts. Digital publication of theses ensures access for all scientists to a summary of such work, provides students with a formal citation for their thesis, and demonstrates the exceptional quality of student research and student-faculty cooperation at Yale. In 2006, the digital copy became a graduation requirement. Starting in 2012, alumni of the Yale School of Medicine were invited to participate in the YMTDL project by granting scanning and hosting permission to the Cushing/Whitney Medical Library, which digitized the Library’s print copy of their thesis or dissertation.

The Office of Student Research and Yale School of Medicine require that the MD thesis be submitted to the YMTDL. This submission should be accompanied by a completed “ Yale School of Medicine Digital Thesis Depositor’s Declaration Form. ”

Detailed instructions on how to submit a thesis via the ProQuest website will be provided to students at the time they receive notification of formal YSM thesis approval and feedback from the Office of Student Research.

The electronic thesis submission process in ProQuest is organized into three major categories: Publishing information, About my dissertation/thesis, and Submission & payment. The system will walk you through the submission process step-by-step.

A few things to note: the ETD Administrator software is a 3rd party product that YSM licenses from ProQuest. Because this vendor is unaffiliated with Yale, OSR and Yale University Libraries do not have control over its policies or processes. The ProQuest Dissertations and Theses Global database is a subscription resource that collects dissertations and theses from multiple countries and a range of academic specialties. This collection is then made available to subscribers. When students choose to make their thesis publicly available, the full-text will appear in this database and users will be able to read, save, and download the text.

EliScholar is a digital platform for scholarly publishing provided by the Yale University Library (YUL). While it is supported by third party software, EliScholar is maintained by YUL and offers more flexibility in uploading and managing theses. The option selected for thesis release in the ETD Administrator system will apply to both ProQuest Dissertations and Theses Global and EliScholar. All theses are available to the Yale community (individuals with a NetID and password and users physically present at a library facility on campus) upon publication. A limited release of approved theses to the awarding institution’s user community is common practice.

Starting with the Yale School of Medicine (YSM) graduating class of 2002, the Cushing/Whitney Medical Library and YSM Office of Student Research have collaborated on the Yale Medicine Thesis Digital Library (YMTDL) project, publishing the digitized full text of medical student theses on the web as a valuable byproduct of Yale student research efforts. The digital thesis deposit has been a graduation requirement since 2006. Starting in 2012, alumni of the Yale School of Medicine were invited to participate in the YMTDL project by granting scanning and hosting permission to the Cushing/Whitney Medical Library, which digitized the Library’s print copy of their thesis or dissertation. A grant from the Arcadia Fund in 2017 provided the means for digitizing over 1,000 additional theses. The Potential of Nanoparticle-Mediated Macrophage Polarization for Solid Tumor Therapy: Evidence Synthesis and Temporal Monitoring

Cancer persists as a significant public health challenge despite treatment advances.  Colorectal cancer is especially a concern because its incidence and mortality are increasing in  young individuals. Solid tumors, such as colorectal cancer, are challenging to treat with  immunotherapies due to their immunosuppressive microenvironment preventing T cell infiltration.  Tumor-associated macrophages (TAMs) play a pivotal role in supporting this immunosuppression  by adopting an anti-inflammatory and pro-tumoral phenotype within the tumor microenvironment.  Leveraging nanoparticles to repolarize TAMs towards a proinflammatory and tumoricidal  phenotype holds promise for solid tumor therapy. However, clinical translation of cancer nanomedicines has been slow. This dissertation aims to address this through quantitative review  of the intersection between the colorectal cancer nanomedicine and macrophage polarization,  and by proposing a novel method of temporally tracking macrophage polarization using  bioluminescent reporter cells.  

Using eLDA topic modeling, the dissertation identifies six major topics in the intersection  of cancer medicine and macrophage polarization, providing insights into nanoparticle design  choices and therapeutic strategies across various cancer types. A scoping review and meta analysis of colorectal cancer nanomedicine over two decades reveal evolving nanoparticle design  strategies and their impact on macrophage polarization. We also demonstrate how a  nanoparticle’s ability to increase macrophages’ ratio of M1 to M2 polarization is correlated with  their efficacy at reducing tumor growth and increasing survival. This dissertation also includes a  technology assessment of how evidence synthesis of preclinical studies informs open science  policy.   

To better temporally track macrophage polarization, the dissertation introduces a method  utilizing THP-1 reporter cells with bioluminescently labeled polarization-relevant transcription  factors. We demonstrated how these reporter cells enable time-resolved activation curves for  tumor-associated macrophages, revealing unique NFκB activation profiles dependent on cancer  type that we linked to the tumor microenvironments immunogenicity. Furthermore, monitoring  monocyte to macrophage differentiation with this method highlights the importance of selecting appropriate differentiation protocols for the intended use. These examples demonstrate the  potential use of this bioluminescent platform to monitor macrophage polarization in response to  immunomodulatory treatments, like macrophage-targeted cancer nanomedicine.

Personalized oxygenation could improve outcomes for patients on ventilators

Supplemental oxygen is among the most widely prescribed therapies in the world, with an estimated 13 to 20 million patients worldwide requiring oxygen delivery by mechanical ventilation each year. Mechanical ventilation — a form of life support — is a technology that moves breathable air into and out of the lungs, acting like a bellows. Ventilators have moved far beyond the “iron lung” machines some people might picture; now, apparatuses have progressed to sophisticated, compact digital machines that deliver oxygen through a small plastic tube that goes down the throat.

Despite technological advancements, the correct amount of oxygen to deliver to each patient has remained a guessing game. Clinicians prescribe oxygen levels by using devices that record SpO 2 saturation, which measures the amount of oxygen in a patient’s blood. However, prior research was unable to establish whether a higher or lower SpO 2 target is better for patients.

“The standard of care is to maintain oxygen saturation between 88 and 100; within that range, doctors have had to choose an oxygen level for ventilation without having high-quality data to inform their decision-making,” said Kevin Buell, MBBS, a pulmonary and critical care fellow at the University of Chicago Medicine. “Whether we like it or not, making that decision for each patient exposes them to the potential benefits or harms of the chosen oxygen level.”

To take the guesswork out of ventilation, Buell and a group of other researchers used a machine learning model to study whether the effects of different oxygen levels depend on individual patients’ characteristics. The results, published in JAMA , suggest that personalized oxygenation targets could reduce mortality — which could have far-reaching impacts on critical care.

Previously, some research groups conducted randomized trials to investigate whether higher or lower oxygen levels are better for patients overall, but most produced no clear answer. Buell and his collaborators hypothesized that instead of indicating that oxygen levels don’t affect patient outcomes, the neutral results might indicate that the treatment outcomes for different oxygen levels varied by patient and simply averaged to zero effect in randomized trials.

As personalized medicine continues gaining traction, there is a growing interest in using machine learning to make predictions for individual patients. In the context of mechanical ventilation, these models could potentially use specific patient characteristics to predict an ideal oxygen level for each patient. These characteristics included age, sex, heart rate, body temperature and reason for being admitted to an Intensive Care Unit (ICU).

“We set out to create an evidence-based, personalized prediction of who would benefit from a lower or higher oxygen target when they go on a ventilator,” said Buell, a joint first author on the study.

Those previous randomized trials didn’t go to waste — Buell and his collaborators used data from those studies to design and train their machine learning model. After the model was developed using trial data collected in the U.S., the collaborators applied it to data from patients across the world in Australia and New Zealand. For patients who received oxygenation that fell within the target range the machine learning model predicted to be beneficial for them, mortality could have decreased by 6.4% overall.

It’s impossible to generalize predictions based on a single characteristic — for example, not all patients with brain injuries will benefit from lower oxygen saturation even though the data skew in that direction — which is why clinicians need a tool like the researchers’ machine learning model to piece together the mosaic of each patient’s needs. However, Buell pointed out that although the algorithm itself is complicated, the variables healthcare teams would input are all familiar clinical variables, making it easy for anyone to implement this kind of tool in the future.

At UChicago Medicine, healthcare teams can already use algorithms directly integrated into the electronic health record (EHR) system to inform other areas of clinical decision-making. Buell hopes mechanical ventilation can one day function the same way. For hospitals that might not have the resources to integrate machine learning into an EHR, he even envisions creating a web-based application that would allow clinicians to type in patient characteristics and obtain a prediction that way — like an online calculator. A lot of validation, testing and refinement needs to happen before clinical implementation can become a reality, but the end goal makes that future research well worth the investment.

In an editorial that accompanied the article’s publication, renowned critical care expert Derek Angus, MD, wrote: “If the results are true and generalizable, then the consequences are staggering. If one could instantly assign every patient into their appropriate group of predicted benefit or harm and assign their oxygen target accordingly, the intervention would theoretically yield the greatest single improvement in lives saved from critical illness in the history of the field.”

The study, “ Individualized Treatment Effects of Oxygen Targets in Mechanically Ventilated Critically Ill Adults ,” was published in the Journal of the American Medical Association in March 2024. While the 10 papers presented below admittedly represent an arbitrary choice (Graphical Abstract), there are thousands of other ones equally worthy of mention.

Over the past year, there have been many outstanding contributions to the field of cardio-oncology, and the scope and depth of them are exemplified herein. The first to mention is the update by the International Late Effects of Childhood Cancer Guideline Harmonization Group on tailored cardiomyopathy surveillance recommendations for healthcare providers and childhood and young adult cancer patients, who are at least 2 years from the completion of cancer therapy, which included anthracyclines and/or radiation therapy (RT) with heart exposure.

Gene variants foretell the biology of future breast cancers in Stanford Medicine study

In a finding that vastly expands the understanding of tumor evolution, researchers discover genetic biomarkers that can predict the breast cancer subtype a patient is likely to develop.

Stanford Medicine researchers found that inherited gene sequences can predict what type of breast cancer a patient is likely to develop, along with how aggressive that cancer may be.   Emily Moskal

A Stanford Medicine study of thousands of breast cancers has found that the gene sequences we inherit at conception are powerful predictors of the breast cancer type we might develop decades later and how deadly it might be.

The study challenges the dogma that most cancers arise as the result of random mutations that accumulate during our lifetimes. Instead, it points to the active involvement of gene sequences we inherit from our parents — what’s known as your germline genome — in determining whether cells bearing potential cancer-causing mutations are recognized and eliminated by the immune system or skitter under the radar to become nascent cancers. 

“Apart from a few highly penetrant genes that confer significant cancer risk, the role of hereditary factors remains poorly understood, and most malignancies are assumed to result from random errors during cell division or bad luck,” said Christina Curtis , PhD, the RZ Cao Professor of Medicine and a professor of genetics and of biomedical data science. “This would imply that tumor initiation is random, but that is not what we observe. Rather, we find that the path to tumor development is constrained by hereditary factors and immunity. This new result unearths a new class of biomarkers to forecast tumor progression and an entirely new way of understanding breast cancer origins.”

Curtis is the senior author of the study, which will be published May 31 in Science . Postdoctoral scholar Kathleen Houlahan , PhD, is the lead author of the research.

“Back in 2015, we had posited that some tumors are ‘born to be bad’ — meaning that their malignant and even metastatic potential is determined early in the disease course,” Curtis said. “We and others have since corroborated this finding across multiple tumors, but these findings cast a whole new light on just how early this happens.”

A new take on cancer’s origin

The study, which gives a nuanced and powerful new understanding of the interplay between newly arisen cancer cells and the immune system, is likely to help researchers and clinicians better predict and combat breast tumors.

Currently, only a few high-profile cancer-associated mutations in genes are regularly used to predict cancers, but these account for a small minority of cases. Those include BRCA1 and BRCA2, which occur in about one of every 500 women and confer an increased risk of breast or ovarian cancer, and rarer mutations in a gene called TP53 that causes a disease called Li Fraumeni syndrome, which predisposes to childhood and adult-onset tumors.

The findings suggest there are tens or hundreds of additional gene variants — identifiable in healthy people — that through interactions with the immune system pull the strings that determine why some people remain cancer-free throughout their lives.

“Our findings not only explain which subtype of breast cancer an individual is likely to develop,” Houlahan said, “but they also hint at how aggressive and prone to metastasizing that subtype will be. Beyond that, we speculate that these inherited variants may influence a person’s risk of developing breast cancer. However, future studies will be needed to examine this.”  

The genes we inherit from our parents are known as our germline genome. They’re mirrors of our parents’ genetic makeup, and they can vary among people in small ways that give some of us blue eyes, brown hair or type O blood. Some inherited genes include mutations that confer increased cancer risk from the get-go, such as BRCA1, BRCA2 and TP53.

In contrast, most cancer-associated genes are part of what’s known as our somatic genome. As we live our lives, our cells divide and die in the tens of millions. Each time the DNA in a cell is copied, mistakes happen and mutations can accumulate. DNA in tumors is often compared with the germline genomes in blood or normal tissues in an individual to pinpoint which changes likely led to the cell’s cancerous transformation.

Classifying breast cancers

In 2012, Curtis began a deep dive — assisted by machine learning — into the types of somatic mutations that occur in thousands of breast cancers. She was eventually able to categorize the disease into 11 subtypes with varying prognoses and risk of recurrence, finding that four of the 11 groups were significantly more likely to recur even 10 or 20 years after diagnosis — critical information for clinicians making treatment decisions and discussing long-term prognoses with their patients.

Prior studies had shown that people with inherited BRCA1 mutations tend to develop a subtype of breast cancer known as triple negative breast cancer. This correlation implies some behind-the-scenes shenanigans by the germline genome that affects what subtype of breast cancer someone might develop.

“We wanted to understand how inherited DNA might sculpt how a tumor evolves,” Houlahan said. To do so, they took a close look at the immune system.

It’s a quirk of biology that even healthy cells routinely decorate their outer membranes with small chunks of the proteins they have bobbing in their cytoplasm — an outward display that reflects their inner style.

The foundations for this display are what’s known as HLA proteins, and they are highly variable among individuals. Like fashion police, immune cells called T cells prowl the body looking for any suspicious or overly flashy bling (called epitopes) that might signal something is amiss inside the cell. A cell infected with a virus will display bits of viral proteins; a sick or cancerous cell will adorn itself with abnormal proteins. These faux pas trigger the T cells to destroy the offenders.

Houlahan and Curtis decided to focus on oncogenes, normal genes that, when mutated, can free a cell from regulatory pathways meant to keep it on the straight and narrow. Often, these mutations take the form of multiple copies of the normal gene, arranged nose to tail along the DNA — the result of a kind of genomic stutter called amplification. Amplifications in specific oncogenes drive different cancer pathways and were used to differentiate one breast cancer subtype from another in Curtis’ original studies.

The importance of bling

The researchers wondered whether highly recognizable epitopes would be more likely to attract T cells’ attention than other, more modest displays (think golf-ball-sized, dangly turquoise earrings versus a simple silver stud). If so, a cell that had inherited a flashy version of an oncogene might be less able to pull off its amplification without alerting the immune system than a cell with a more modest version of the same gene. (One pair of overly gaudy turquoise earrings can be excused; five pairs might cause a patrolling fashionista T cell to switch from tutting to terminating.)

The researchers studied nearly 6,000 breast tumors spanning various stages of disease to learn whether the subtype of each tumor correlated with the patients’ germline oncogene sequences. They found that people who had inherited an oncogene with a high germline epitope burden (read: lots of bling) — and an HLA type that can display that epitope prominently — were significantly less likely to develop breast cancer subtypes in which that oncogene is amplified.

There was a surprise, though. The researchers found that cancers with a large germline epitope burden that manage to escape the roving immune cells early in their development tended to be more aggressive and have a poorer prognosis than their more subdued peers.

“At the early, pre-invasive stage, a high germline epitope burden is protective against cancer,” Houlahan said. “But once it’s been forced to wrestle with the immune system and come up with mechanisms to overcome it, tumors with high germline epitope burden are more aggressive and prone to metastasis. The pattern flips during tumor progression.”

“Basically, there is a tug of war between tumor and immune cells,” Curtis said. “In the preinvasive setting, the nascent tumor may initially be more susceptible to immune surveillance and destruction. Indeed, many tumors are likely eliminated in this manner and go unnoticed. However, the immune system does not always win. Some tumor cells may not be eliminated and those that persist develop ways to evade immune recognition and destruction. Our findings shed light on this opaque process and may inform the optimal timing of therapeutic intervention, as well as how to make an immunologically cold tumor become hot, rendering it more sensitive to therapy.”

The researchers envision a future when the germline genome is used to further stratify the 11 breast cancer subtypes identified by Curtis to guide treatment decisions and improve prognoses and monitoring for recurrence. The study’s findings may also give additional clues in the hunt for personalized cancer immunotherapies and may enable clinicians to one day predict a healthy person’s risk of developing an invasive breast cancer from a simple blood sample.

“We started with a bold hypothesis,” Curtis said. “The field had not thought about tumor origins and evolution in this way. We’re examining other cancers through this new lens of hereditary and acquired factors and tumor-immune co-evolution.”

“Rosetta Stone” of cell signaling could expedite precision cancer medicine

A newly complete database of human protein kinases and their preferred binding sites provides a powerful new platform to investigate cell signaling pathways.

Culminating 25 years of research, MIT, Harvard University, and Yale University scientists and collaborators have unveiled a comprehensive atlas of human tyrosine kinases — enzymes that regulate a wide variety of cellular activities — and their binding sites.

The addition of tyrosine kinases to a previously published dataset from the same group now completes a free, publicly available atlas of all human kinases and their specific binding sites on proteins, which together orchestrate fundamental cell processes such as growth, cell division, and metabolism.

Now, researchers can use data from mass spectrometry, a common laboratory technique, to identify the kinases involved in normal and dysregulated cell signaling in human tissue, such as during inflammation or cancer progression.

“I am most excited about being able to apply this to individual patients’ tumors and learn about the signaling states of cancer and heterogeneity of that signaling,” says Michael Yaffe, who is the David H. Koch Professor of Science at MIT, the director of the MIT Center for Precision Cancer Medicine, a member of MIT’s Koch Institute for Integrative Cancer Research, and a senior author of the new study. “This could reveal new druggable targets or novel combination therapies.”

The study, published in Nature , is the product of a long-standing collaboration with senior authors Lewis Cantley at Harvard Medical School and Dana-Farber Cancer Institute, Benjamin Turk at Yale School of Medicine, and Jared Johnson at Weill Cornell Medical College.

The paper’s lead authors are Tomer Yaron-Barir at Columbia University Irving Medical Center, and MIT’s Brian Joughin, with contributions from Kontstantin Krismer, Mina Takegami, and Pau Creixell.

Kinase kingdom

Human cells are governed by a network of diverse protein kinases that alter the properties of other proteins by adding or removing chemical compounds called phosphate groups. Phosphate groups are small but powerful: When attached to proteins, they can turn proteins on or off, or even dramatically change their function. Identifying which of the almost 400 human kinases phosphorylate a specific protein at a particular site on the protein was traditionally a lengthy, laborious process.

Beginning in the mid 1990s, the Cantley laboratory developed a method using a library of small peptides to identify the optimal amino acid sequence — called a motif, similar to a scannable barcode — that a kinase targets on its substrate proteins for the addition of a phosphate group. Over the ensuing years, Yaffe, Turk, and Johnson, all of whom spent time as postdocs in the Cantley lab, made seminal advancements in the technique, increasing its throughput, accuracy, and utility.

Johnson led a massive experimental effort exposing batches of kinases to these peptide libraries and observed which kinases phosphorylated which subsets of peptides. In a corresponding Nature  paper published in January 2023, the team mapped more than 300 serine/threonine kinases, the other main type of protein kinase, to their motifs. In the current paper, they complete the human “kinome” by successfully mapping 93 tyrosine kinases to their corresponding motifs.

Next, by creating and using advanced computational tools, Yaron-Barir, Krismer, Joughin, Takegami, and Yaffe tested whether the results were predictive of real proteins, and whether the results might reveal unknown signaling events in normal and cancer cells. By analyzing phosphoproteomic data from mass spectrometry to reveal phosphorylation patterns in cells, their atlas accurately predicted tyrosine kinase activity in previously studied cell signaling pathways.

For example, using recently published phosphoproteomic data of human lung cancer cells treated with two targeted drugs, the atlas identified that treatment with erlotinib, a known inhibitor of the protein EGFR, downregulated sites matching a motif for EGFR. Treatment with afatinib, a known HER2 inhibitor, downregulated sites matching the HER2 motif. Unexpectedly, afatinib treatment also upregulated the motif for the tyrosine kinase MET, a finding that helps explain patient data linking MET activity to afatinib drug resistance.

Actionable results

There are two key ways researchers can use the new atlas. First, for a protein of interest that is being phosphorylated, the atlas can be used to narrow down hundreds of kinases to a short list of candidates likely to be involved. “The predictions that come from using this will still need to be validated experimentally, but it’s a huge step forward in making clear predictions that can be tested,” says Yaffe.

Second, the atlas makes phosphoproteomic data more useful and actionable. In the past, researchers might gather phosphoproteomic data from a tissue sample, but it was difficult to know what that data was saying or how to best use it to guide next steps in research. Now, that data can be used to predict which kinases are upregulated or downregulated and therefore which cellular signaling pathways are active or not.

“We now have a new tool now to interpret those large datasets, a Rosetta Stone for phosphoproteomics,” says Yaffe. “It is going to be particularly helpful for turning this type of disease data into actionable items.”

In the context of cancer, phosophoproteomic data from a patient’s tumor biopsy could be used to help doctors quickly identify which kinases and cell signaling pathways are involved in cancer expansion or drug resistance, then use that knowledge to target those pathways with appropriate drug therapy or combination therapy.

Yaffe’s lab and their colleagues at the National Institutes of Health are now using the atlas to seek out new insights into difficult cancers, including appendiceal cancer and neuroendocrine tumors. While many cancers have been shown to have a strong genetic component, such as the genes BRCA1 and BRCA2 in breast cancer, other cancers are not associated with any known genetic cause. “We’re using this atlas to interrogate these tumors that don’t seem to have a clear genetic driver to see if we can identify kinases that are driving cancer progression,” he says.

Biological insights

In addition to completing the human kinase atlas, the team made two biological discoveries in their recent study. First, they identified three main classes of phosphorylation motifs, or barcodes, for tyrosine kinases. The first class is motifs that map to multiple kinases, suggesting that numerous signaling pathways converge to phosphorylate a protein boasting that motif. The second class is motifs with a one-to-one match between motif and kinase, in which only a specific kinase will activate a protein with that motif. This came as a partial surprise, as tyrosine kinases have been thought to have minimal specificity by some in the field.

The final class includes motifs for which there is no clear match to one of the 78 classical tyrosine kinases. This class includes motifs that match to 15 atypical tyrosine kinases known to also phosphorylate serine or threonine residues. “This means that there’s a subset of kinases that we didn’t recognize that are actually playing an important role,” says Yaffe. It also indicates there may be other mechanisms besides motifs alone that affect how a kinase interacts with a protein.

The team also discovered that tyrosine kinase motifs are tightly conserved between humans and the worm species C. elegans,  despite the species being separated by more than 600 million years of evolution. In other words, a worm kinase and its human homologue are phosphorylating essentially the same motif. That sequence preservation suggests that tyrosine kinases are highly critical to signaling pathways in all multicellular organisms, and any small change would be harmful to an organism.

10 Things to Know about CDER’s Pharmaceutical Quality Functions

Pharmaceutical quality provides the confidence that a patient’s next dose of medicine is safe, effective, and available when it’s needed. When CDER receives a prescription drug application for approval, we evaluate whether the drug can be made to consistently meet quality standards. Pharmaceutical quality is about more than just the quality of the drug itself. It includes the manufacturing facilities and the processes used to manufacture the drug. Some of CDER’s pharmaceutical quality functions occur before a prescription drug is approved and reaches patients. CDER also monitors drugs on the market and the facilities making them to ensure continued quality.

Here are some things to know about CDER’s pharmaceutical quality functions:

  • Before FDA approval, prescription drug manufacturers must prove their drugs are safe, effective, and meet quality standards. FDA’s quality standards are the same for all drug manufacturers, no matter where they are located (foreign or domestic) or whether they manufacture generic or brand-name drugs.
  • CDER works to develop national and international policies that establish quality standards for drug manufacturers. For example, we publish guidance documents to help drug manufacturers understand regulatory requirements and best practices so they can produce high-quality drugs, and we work with industry organizations and international regulators to advance pharmaceutical quality policies. CDER’s research informs policy development by providing the latest scientific information related to pharmaceutical quality.
  • During our review of a drug application, CDER examines how the manufacturer will ensure the identity, strength, consistency, and purity of the drug. We do this in part by evaluating the manufacturer’s facility and manufacturing process to determine whether the drug can be manufactured in compliance with U.S. law and regulations. 
  • FDA performs a risk-based assessment of facilities listed in a drug application, which includes the facilities used to manufacture a drug, laboratories, and packaging facilities. FDA uses inspections as one tool to help ensure facility and process quality. During the drug approval process, CDER uses a risk-based approach to determine which drug manufacturing facilities need to be inspected prior to approving the drug (for example, an inspection is typically conducted if the drug will be the first product made at a newly built facility). A drug will not be approved if investigators discover a significant quality issue during an inspection prior to approval. Critical manufacturing problems need to be corrected before CDER can approve a drug.
  • CDER maintains an inventory of all facilities engaged in commercial manufacture of US marketed drugs (which we call CDER’s Site Catalog). Most facility inspections are surveillance inspections, which assess whether the facility is operating with processes in place to assure quality and is in compliance with statutory and regulatory requirements. CDER uses a risk-based model to prioritize surveillance inspections among facilities subject to inspection. Risk factors include the compliance history of the facility, recalls or quality complaints linked to the facility, and the type of drugs manufactured at the facility such as sterile injectable drugs.
  • FDA also inspects manufacturers who make non-application drugs (drugs marketed without an FDA-approved application), such as many over-the-counter drugs, to ensure the quality and safety of drugs marketed or sold in the U.S.
  • CDER may request documents from a facility or request to remotely interact with a facility to evaluate compliance with required manufacturing practices. CDER also uses inspection information from regulatory partners in other countries. These alternative tools to inspection allow CDER to obtain information about manufacturing facilities without physically visiting the facility.
  • We take action when there is a safety or quality concern. We may recommend a company recall its drug from the market or add a company to an “import alert” to help stop potentially unsafe products from entering the U.S. CDER may issue a “warning letter” advising a company of their manufacturing violations.
  • CDER maintains an inventory of all drugs marketed in the U.S. (which we call CDER’s Product Catalog) and we sample and test drugs from this list, based on risk, to monitor for quality issues. With approximately 150,000 drugs in CDER’s Product Catalog, we select products for sampling and testing based on data received from FDA investigators, manufacturers, distributors, and wholesalers, as well as directly from consumers and patients. Decades of sampling and testing have shown that the vast majority of legally marketed drugs meet quality standards.
