Debate: Student mental health matters – the heightened need for school‐based mental health in the era of COVID‐19

The COVID‐19 pandemic has highlighted existing gaps in school‐based mental health services and created new and urgent needs to address student mental health. Evidence from early in the pandemic already suggests that preexisting educational and mental health disparities have increased under the stress of the current health crisis. School mental health professionals are essential to help address anxiety, to promote social adjustment in the 'new normal', and to address trauma, grief, and loss. Schools will also need to creatively support teachers during this unprecedented time. Such efforts will require adequate funding and advocacy for the inclusion of school‐based mental health supports within governmental COVID‐19 aid packages.

Child and adolescent psychiatrists are taught from day 1 of training to always place the child’s best interest first. However, children can only do well when their caregivers and communities are similarly well supported. In considering the complex challenges of the COVID‐19 crisis, the best interests of children can only be met by simultaneously considering the best interests of parents, families, teachers, and school staff.

As schools open, in whatever shape or form, mental health practitioners who work in schools will be busier than ever. Even before this COVID‐19 apocalypse began, schools were already ill equipped to meet students’ mental health needs. It has been estimated that more than seven million children in the United States have a mental health disorder; less than half of those diagnosed actually receive treatment, and of those who do receive treatment, over 75% receive this treatment at schools (McKay, Lynn, & Bannon, 2005 ; Merikangas et al., 2011 ). Whether schools open, stay closed, or work semivirtually, the existing challenges will multiply and vary according to the specific solutions developed. Just as hospitals faced a crisis when the pandemic began, now school administrators, families, students, and school staff face similar distress, uncertainty, and confusion about how to return to school given the new realities of COVID‐19. The solutions will necessarily vary by community, level of COVID infection, school size, and resources. Surprisingly, in all that has been written about returning to school, very little has been targeted toward meeting the mental health needs of students, teachers, and other school staff.

Mental health contributes to overall health, and students must be healthy enough to learn. We know our children should return to school, but to do that they need a safe, secure, and supportive learning environment with professionals who also feel safe and cared about. If the stress and illness burden is not too great, we hope that the resilience of youth will prevail. However, those children already burdened by mental health challenges may suffer more. Mental health and educational disparities will widen the pandemic’s negative impact in marginalized and under‐resourced communities. Evidence from two U.S. national surveys conducted by the Edweek Research Center in Spring of 2020 already confirm vast disparities, with schools located in lower socioeconomic communities less able to provide access to technology, to reach all students, or to provide 'live' distance learning instruction (Herold B, April 2020 ). Fairness and equity require that those bearing a disproportionate impact of the pandemic be provided sufficient access to equipment, services, and technology, and mental health support to address systemic cultural disadvantages in education and mental health worsened by COVID‐19.

Decisions related to school reopening must be data‐driven, relying on the best available medical understanding and public health guidance. In addition, different populations, such as children receiving special education, children in foster care, or non‐English‐language learners, may require approaches that differ from the general approach. Children at different developmental stages will have specific needs. What is good for some students and their families will not necessarily be good for others.

Multiple potential mental health consequences arise from school closures related to COVID‐19. Lost learning opportunities during school closures result in possible regression of educational and emotional coping skills, particularly in children with special education needs. For some children, the pandemic may produce heightened fear, anxiety, and depression, leading to further difficulty in emotion regulation and behavior, learning, and interpersonal relationships. The fears of school staff will also need to be recognized and discussed, as these will impact the students in their care.

School mental health professionals will be essential in helping to shape messages to students and families about school re‐entry, to address anxiety, to promote social adjustment to the 'new normal', and to address other unanticipated mental health concerns that are sure to arise during this national trauma experience. Teachers and school personnel should receive training on how to talk with and support children during a pandemic. Intentional strategies to help students adapt to new school environments should be developed. Schools will also need to creatively support teachers in their efforts during this unprecedented time. Addressing the grief of those who have suffered loss will also be critical. Fortunately, many great resources to assist in this effort have already been developed. For example, the National Center for School Mental Health ( http://www.schoolmentalhealth.org/COVID‐19/ ) and the Mental Health Technology Transfer Center Network ( https://mhttcnetwork.org/centers/global‐mhttc/responding‐covid‐19‐school‐mental‐health‐resources ) have both developed school mental health resource pages for dealing with COVID‐19.

It is hard to imagine how children can return to school safely without providing additional (and substantial) financial support. Added to the costs of the physical needs created by the pandemic will be the increased costs necessary to bolster a full continuum of educational AND mental health supports. Funding of school‐based mental health supports will be necessary for prevention of staff burnout and trauma effects, as well as early identification and intervention of mental health problems in the increased numbers of students at high risk for adverse outcomes.

Child mental health organizations should be advocating to ensure that mental health programs, specifically children’s mental health, are included in governmental aid packages. Solidarity with other mental health organizations is essential to ensure that such provisions are included in any governmental COVID‐19 relief packages. Provisions should include increasing funding for existing and new mental health programs. The American Academy of Child and Adolescent Psychiatry (AACAP) Schools Committee, which cowrote this editorial, is especially concerned with ensuring that the best information on school mental health is available as it is developed. As child and adolescent psychiatrists, mental health is definitely 'our lane'. Every student, every family, and every school staff member will have a COVID‐19 story. We need to do our best to hear them all and respond when help is needed.

But some researchers argue that this theory isn't well supported by existing evidence and that it repeats a "moral panic" argument made many times in the past about video games, rap lyrics, television and even radio, back in its early days.

To understand both sides of the debate, I talked in detail to three researchers: one who argues that teens' use of tech is a big problem, one who thinks the danger is exaggerated and an expert in research methodology who suggests the connection may not be so simple. Very concerned about smartphones Jean Twenge, a professor of psychology at San Diego State University, may be the researcher most associated with the idea that smartphones are dangerous to teens. She's the author of the book iGen, whose 27-word subtitle states her thesis: Why Today's Super-Connected Kids Are Growing Up Less Rebellious, More Tolerant, Less Happy — and Completely Unprepared for Adulthood — and What That Means for the Rest of Us .

"At first, when I saw these trends in loneliness and unhappiness and depression starting to spike around 2011 or 2012, I really had no idea what could possibly be causing that. It was a real mystery," she tells NPR. Then, she says, she took note of Pew research that showed 2012 was the first year that most cell phone owners had switched to smartphones. Not only do these two trend lines seem to coincide in time, but Twenge also notes that young people who report spending the most time on smartphones — five to seven hours a day — are twice as likely to report being depressed as those who use their phones for one to two hours a day.

Twenge isn't claiming to have proved that smartphones cause depression. The data sets she works with — essentially large surveys — don't allow for that.

"It is impossible to do a random controlled trial on generations because you can't randomly assign people to be born at different times. So we cannot prove causation given that limitation," she explains. "So we have to go on the data that we have — which is obviously not going to be a true experiment — but it adds up to a lot of evidence pointing toward technology possibly playing a role in this increase in mental health issues."

Evidence — with caveats Given that all teens use media, I asked Twenge, why would the worsening trend in mental health be more pronounced in girls than in boys?

She responded with one possible explanation: Social media, which girls tend to spend more time on, may be the culprit.

"Social media invites comparison," Twenge says. "It's not in real time. It invites anxiety over the likes and responses that you're going to get."

Given that adults use media even more than teenagers, why does this trend crop up in teens? Twenge says it's because their brains are still vulnerable and developing. Plus, they haven't had as much time to make social connections in real life as older people have, so they are even more dependent on their phones for social validation.

Twenge even thinks that the availability of smartphones could help explain the rise in suicide rates among the youngest girls. "They have more access to information online — potentially harmful information about how to harm yourself."

My final question for Twenge: She, personally, made a very similar argument about young people before smartphones existed. She previously published a book, Generation Me, that looked at similar data sets and labeled the millennial generation as "miserable," "narcissistic" and "anxious." That book came out in 2006; the iPhone was introduced in 2007. Is she putting old wine in new bottles?

Twenge says that comparing then with now, mental health trends are even more negative for what she calls iGen and, in retrospect, "more of a mixed bag" for millennials.

Not worth the time

Parents are concerned. Detox programs have sprung up to treat teen tech addiction. But some researchers are skeptical of the hypothesis that smartphones cause problems.

One team has published three papers that analyzed the same data Twenge is looking at — over 350,000 participants in three nationwide surveys in the U.S. and the United Kingdom. Amy Orben, the lead author of each paper and a psychologist at Oxford University, says the team found that the actual negative relationship between teens' mental health and technology use is tiny. "A teenagers' technology use can only explain less than 1% of variation in well-being," Orben says. "It's so small that it's surpassed by whether a teenager wears glasses to school," or rides a bicycle, or eats potatoes — all comparisons made by Orben and her Oxford co-author Andrew K. Przybylski.

How can this be? Well, smartphone use is almost ubiquitous among teenagers today, while only a small minority report mental health problems. So, knowing that a teenager uses a smartphone, even for many hours a day, won't reliably predict that the teenager will become depressed. It tells you far, far less than factors like genetics or the presence of childhood trauma, for example.

Orben has been researching the history of people making dire claims about young people and new forms of media. For example, she says, "In the 1940s, people were already talking about 'radio addiction.' One study found that fully 57% of children ages 6 to 16 were severely 'addicted to radio programs and needed them like an alcoholic does their drink.' "

She thinks the negative trends in mental health could be explained by a wide range of factors: economic anxiety or political upheaval, to name two. And, she adds, there's a chance that young people today may simply be more open in surveys when asked about mental health challenges. "A lot of teenagers are a lot more OK to say they're not OK." Ironically, this openness may in fact be partly due to social media.

Twenge responds that a forthcoming paper she has written, currently under review, will challenge the conclusions of Orben's team. She says that just because the impact of smartphone use appears small, that doesn't mean it's insignificant, especially since, unlike genetics, it may be controllable.

Statistically problematic As a sort of referee on this debate, I called up Katherine Keyes, an epidemiologist at Columbia University's Mailman School of Public Health. Her focus is on explaining population-wide trends, particularly in adolescent mental health.

She too is a critic of Twenge's work, saying it has a tendency to "skew the data" by zooming in on screen use to the exclusion of other factors in the lives of adolescents.

And, she says, there are lots of numbers that don't necessarily fit Twenge's theory.

Not all the news is bad when it comes to teens. High school graduation rates are up, for example. Drug and alcohol use is down, as are car crashes and teen pregnancy.

We're not seeing the same negative trends in every country, even in those where teens are just as glued to their screens as they are in the United States.

There isn't a linear relationship between screen use and mental health. On most surveys, teens who use their phones up to two hours a day appear healthier than those who don't use them at all. This doesn't count other reasons for technology use, such as homework or listening to music.

In the case of heavy users, Keyes says, smartphone use may be more a symptom than a cause of mental health problems. Or there may be a third variable that is driving both — like a lack of parental support or health issues. The explanation that Keyes finds most compelling is that there is a "bidirectional" relationship among teens, screens and mental health. In other words, as argued in this paper by Candice Odgers in the journal Nature, teens who are already struggling may be more drawn to screens and more likely to form unhealthy relationships with media, for example by seeking out information on self-harm or encountering cyberbullies. The time they spend online might in turn make them feel worse.

Twenge agrees with the general idea that "social media invites comparisons and anxiety."

What's a parent to do? Although their conclusions are different, no researcher I've spoken with thinks it's a great idea to let teens scroll through TikTok or play Fortnite all day and night.

Twenge, Orben and Keyes are supportive of similar common-sense rules, like making sure teens don't have their phones in their bedrooms late at night and trying to ensure that their lives are balanced with outdoor exercise, school and face-to-face time with friends and family.

So why should the average parent worry about this scientific controversy? Because, Keyes says, when parents simply demonize phones, "there's less of a communications channel" about what teens are encountering online. A parent's opportunity to mentor or support positive uses of media is replaced by "confrontation on a day-to-day basis." Well-meaning parents, wrongly believing the phone to be as risky as a cigarette or a beer, may actually be making their children's lives harder by fighting with them about it.

Impact of COVID-19 on the mental health of US college students

Background/aim

In the beginning of 2020, the novel Coronavirus disease (COVID-19) caused by the SARS-CoV-2 virus, became a public health emergency in the U.S. and rapidly escalated into a global pandemic. Because the SARS-CoV-2 virus is highly contagious, physical distancing was enforced and indoor public spaces, including schools and educational institutions, were abruptly closed and evacuated to ensure civilian safety. Accordingly, educational institutions rapidly transitioned to remote learning. We investigated the impacts of the COVID-19 pandemic on domestic U.S. college students, ages 18–24 years.

Through Pollfish®’s survey research platform, we collected data from 200 domestic U.S. college students in this age range (N = 200) regarding the physical, emotional, and social impacts of COVID-19 as well as key background information (e.g. whether or not they are first-generation or if they identify with the LGBTQIA+ community).

Our results indicate that students closer to graduating faced increases in anxiety (60.8%), feeling of loneliness (54.1%), and depression (59.8%). Many reported worries for the health of loved ones most impacted their mental health status (20.0%), and the need to take care of family most affected current and future plans (31.8%). Almost one-half of students took to exercising and physical activity to take care of their mental health (46.7%). While a third did not have strained familial relationships (36.5%), almost one half did (45.7%). A majority found it harder to complete the semester at home (60.9%), especially among those who had strained relationships with family (34.1%). Seventy percent spent time during the pandemic watching television shows or movies. Significantly more men, first-generation, and low-income students gained beneficial opportunities in light of the pandemic, whereas their counterparts reported no impact. First-generation students were more likely to take a gap year or time off from school.

Conclusions

Although students found ways to take care of themselves and spent more time at home, the clear negative mental health impacts call for schools and federal regulations to accommodate, support, and make mental health care accessible to all students.

Introduction

The outbreak of the SARS-CoV-2 virus, originally from Wuhan, China, in December 2019 gave rise to an ongoing global public health crisis addressing the proliferation of the novel Coronavirus disease (COVID-19). The SARS-CoV-2 virus directly targets the respiratory system in humans with characteristic symptoms of cough, fever, sore throat, dyspnea, and fatigue [ 1 ]. As the virus continued spreading over 2020, more findings on the epidemiological characteristics of SARS-CoV-2 have been divulged to the public. A study showed that the COVID-19 disease presents varying lengths of incubation periods, typically an average of 5.2 days, and it is estimated to have a slightly higher basic reproduction number (2.24–3.58) than that of the original SARS-CoV virus [ 1 ]. According to the National Health Commission of China, the virus can be transmitted through aerosols, droplets, contacts, respiratory aspirates, and feces, with both animals (e.g., bats, pangolins) and humans being veritable modes of transmission [ 2 ]. Accurate and accelerated testing is necessary to control this extremely contagious disease in cities, communities, and hospitals.

Due to the sudden outbreak of COVID-19, most universities across the United States were forced to send their students home early for the 2019–2020 academic year to prevent spread and protect students as well as surrounding communities. The sudden change in students’ learning environment, the quality of their education, and other circumstances caused students to face unique challenges, adversely impacting their mental health. The loss of internships, on-campus jobs, and other opportunities also contributed to the stress and declining mental health of students. According to a study done on a cohort of students attending Dartmouth College, there were noticeable differences in behavioral and mental health over the course of the pandemic thus far, with a higher number of self-reported cases of depression and anxiety around final exams [ 3 ].

Other minority communities also faced particular hardship in light of the pandemic. For example, greater proportions of the lesbian, gay, bisexual, transgender, and queer or questioning (LGBTQ) population do not have access to health insurance and struggle with poverty compared to the non-LGBTQ population [ 4 ]. Because of their health disparities and social disadvantages, the mental health of LGBTQ students is exacerbated due to the psychological trauma that can come with the COVID-19 pandemic. Furthermore, people of color and those in socioeconomically disadvantaged groups are more likely to be mentally overwhelmed due to the unequal burden of finances, illness, and death. In a study done at a hospital in northern California of COVID-19 patients, non-Hispanic African Americans were found to be 2.7 times more likely to be hospitalized compared to non-Hispanic white patients [ 5 ].

A study of college students from India [ 6 ] found that both anxiety and depression were prevalent in their cohort, with women being affected more. They also noted a disturbed sleeping pattern which aligns with both anxiety and depression. A study that used smartphone-based ecological momentary assessments of anxiety and optimism related to COVID-19 and other generic mental health variables 6 times daily [ 7 ] found widespread mental health impact, especially anxiety, in their cohort of 140 students. Yet another study of college students [ 8 ] found a significant decline in physical activity and mental health occurred as a result of the COVID-19 pandemic.

There are many variables that can further contribute to the mental health status of college students during the pandemic, including their identity, family life, and background. We aimed to investigate the emotional, physical and social impacts on domestic US college students ages 18–24 and determine whether these impacts were significant among particular groups.

Recruitment and data collection

Two hundred (N = 200) domestic U.S. college students ages 18–24 attending a 4-year university in person before the COVID-19 pandemic were surveyed through Pollfish®. Pollfish® is a survey research platform that uses organic sampling built on Random Device Engagement (RDE). [ 9 ] Using artificial intelligence (AI) to track unique respondent identification, RDE reaches users in their natural environments as they participate in their daily activities through any device. [ 10 ] Pollfish®’s partnerships with over 120,000 applications and more than 700 million global users allow for random recruitment of participants fitting the specific inclusion criteria via in-app incentives specific to each user’s real-time activity on their respective devices [ 9 ]. The advanced AI technology and algorithm prevents fraud from single users on multiple accounts (SUMA) and suspicious or illogical responses to specific questions [ 10 ]. Pollfish® uses weighting to match the univariate distributions of age, gender, and geographic region. All results reported use this weighting.

Two screening questions were used to determine survey eligibility. These questions inquired whether participants are male or female aged 18–24 years, and whether they attended a domestic four-year U.S. college or university. The survey then consisted of 14 multiple choice questions. For some of the questions, multiple selections amongst the multiple choices were allowed, so that percentage totals could exceed 100%. The first 3 questions of the 14 inquired about background, including the participants’ year in college, whether they are the first in their family to attend college, and if they identify with the LGBTQIA+ community. The subsequent questions honed in on the physical, emotional, and social impacts of the COVID-19 pandemic. The final question was an open-ended one designed to capture the students’ verbatim feelings.

Statistical analysis

Data were analyzed using JMP Pro 14.1 for Windows [ 11 ]. Participants with a household income less than $50,000 annually were considered “low-income”. For comparing 2 × 2 contingency tables, Fisher’s two-tailed exact test was used. For comparing ordinal data, Wilcoxon’s rank-sum test was used. Ninety-five percent confidence intervals (CI) for odds ratios are Wald-based. All results used weighting generated by Pollfish® to match the univariate distributions of age and gender.

Ethics committee approval

This study (# 2020-966) was considered exempt by our institutional review board manager HCA Centralized Algorithms for Research Rules on IRB Exemptions (CARRIE).

A total of 200 people responded, of whom 50.6% were female (after adding weighting). Twenty three and 3/10% were first-year students, 39.9% were second-year students, 17.8% were third-year, 12.6% were fourth-year (seniors), and seven and one half percent were taking additional semesters (fifth or higher year). Fifty eight percent were first-generation students, and 36% considered themselves to be “a member of the LGBTQIA+ community.” The distributions of responses to each question are summarized in the Appendix, and each question is explored in detail in this section.

How has COVID-19 impacted your mental health?

For this question, multiple responses were allowed. Increased anxiety, depression, and feeling of loneliness were found in 60.8%, 54.1%, and 59.8% of the weighted population, respectively (Fig.  1 ). More than eighty percent (83.8%) reported an increase in at least one of these three symptoms. On the other hand, decreased anxiety, depression, and feelings of loneliness were respectively found in only 9.1%, 5.3%, and 4.6% of the population. For 10.7%, their mental health was unaffected. Using Fisher’s exact test, we found no significant differences in the prevalence of having at least one increased mental health symptom across first-generation status, gender, or LGBTQIA+ status. A general trend of decreased prevalence of symptoms as students drew closer to graduation was noticed. Using Fisher’s exact test, academic year and prevalence of mental health symptoms are not independent, with p  < 0.0001.

If you are not feeling at ease, what contributes MOST to your mental health status?

Twenty percent of respondents said that worries about the health of loved ones was their primary concern, 19.2% were most concerned about school/continuing education, 19.0% had anxiety about lack of proactivity, 15.9% were worried about finances, 10.8% were worried about future job offers, and 5.0% feared contracting the virus. Only 6.8% said that they were feeling at ease. Interestingly, there was no significant association between the proportion of respondents who were most worried about finances and income level (Fig.  2 ).

Was it easier or harder to complete this semester away from campus?

Almost sixty-one percent of students found it harder to complete the semester away from campus, while 32.7% found it easier and 6.4% reported no change. There was no significant difference across age, gender, income level, LGBTQIA+ status, or first-generation status in this response.

How has COVID-19 affected your physical health?

Fifty percent of respondents indicated that they gained weight due to increased eating, while 20.2% reported that they felt/looked better due to exercise or dieting, 13.3% lost weight due to a lack of appetite, and 16.6% reported no changes. Among the students who reported an increased feeling of loneliness (59.8%), 57% reported that they gained weight, compared with just 39.4% of those who did not experience symptoms. Using Fisher’s exact test, this was significant with a p -value of 0.0301. The odds ratio (OR) is 2.04 (95% CI 1.15–3.62).

How has COVID-19 impacted your current and future plans?

COVID-19 disrupted the lives of most students, with only 26.4% stating the pandemic has not impacted their current or future plans. 27.1% lost an internship or job offer, 22.9% are taking a gap year or time off from school, and 31.8% need to support family. Only 16.6% say that the pandemic has given them other beneficial opportunities.

The groups of people most likely to have other beneficial opportunities due to the pandemic were men (OR 3.18, 95% CI 1.41–7.22, p = 0.0039), first-generation students (OR 2.27, 95% CI 0.99–5.19, p  = 0.0329), and low-income students (OR 3.05, 95% CI 1.35–6.99, p  = 0.0042). First-generation students were significantly more likely to take a gap year/time off from school (OR 2.42, 95% CI 1.17–5.02, p  = 0.0251).

On the other hand, the groups that were significantly more likely to report no impact on future plans were women (OR 2.05, 95% CI 1.07–3.92, p  = 0.0159), non-first-generation students (OR 3.12, 95% CI 1.62–5.97, p  = 0.0003), and non-low-income students (OR 3.77, 95% CI 1.89–7.53, p  < 0.0001). Given that these three groups correspond exactly to those which were least likely to say they gained beneficial opportunities, we investigated the rate at which gender, first-generation status, and low-income status affects having either a beneficial opportunity or no change in future plans. When performing this analysis, we found no significant differences across any of the three groups. Combined with the other evidence, this suggests that the key difference between demographic groups lies in the rate at which they gained beneficial opportunities in light of the pandemic.

How has COVID-19 impacted your relationships with your family?

29.4% of students had improved relationships with family, while 34.1% had strained relationships with family and 36.5% had no impact on relationships with family. The people who had strained relationships were significantly more likely to consider it harder to complete the semester at home (OR 2.59, 95% CI 1.36–4.94, p  = 0.0036) compared to those who had improved relationships or no change.

27.8% of students had improved relationships with friends, while 45.7% had strained relationships with friends and 26.5% had no impact on relationships with friends.

How have you specifically taken care of your mental health amidst COVID-19?

29.0% of students engaged in mindfulness activities (meditation, yoga, journaling, etc.). 46.7% were exercising or engaging in physical activity, 22.0% were using a health app, 17.7% were obtaining mental health care from a professional, and 30.3% were not taking any specific actions to take care of their mental health. Of the people who were exercising, 35.9% gained weight while 32.0% said they felt/looked better due to exercise or dieting. In contrast, of those who were not exercising, 62.2% gained weight and only 9.8% said they felt or looked better. Using Fisher’s exact test, this effect was significant with p  < 0.0001.

At what point was your concern about COVID-19 heightened?

29.8% of students had their concern first heightened when college campuses sent students home, whereas 29.0% were first alarmed by states beginning lockdown guidelines. For 10.5%, they became more concerned when a friend or relative was diagnosed with COVID-19. For just 6.4%, the turning point was that friends or relatives were taking prevention measures seriously. Only 4.1% of the population indicated that they were not concerned about COVID-19. The distribution of responses was roughly equal across demographic groups.

How are you spending most of your time during the pandemic?

71.0% were watching TV shows or movies, 30.5% were reading books, 39.6% were exercising, 34.9% were learning new skills or picking up new hobbies, 33.6% were cooking or baking, 29.5% were working or interning, and 8.0% said they were not doing very much at all (Fig.  3 ). Note that the 39.6% figure of those exercising does not conflict with the 46.7% figure above, as some of the respondents may not be devoting very much time to exercise, and do not consider it a major use of their time during the pandemic.

Is there anything else you would like to share about the effect of COVID-19 on your mental health?

As this was a free-form text box, we used JMP’s text analysis suite to identify keywords. However, neither keyword identification nor singular value decomposition provided very much insight into the sentiments expressed. Given that this was an optional question, there was not enough information for numerical patterns to emerge. Instead, we have included a few quotes that demonstrate the range of ways that the COVID-19 pandemic has affected people’s mental health (not ordered by frequency).

The COVID-19 pandemic has brought severe educational and social repercussions, including the closure of college campuses as well as the introduction of online learning and social distancing at universities for the foreseeable future. College students were forced to abandon the social advantages of the so-called “college experience” (i.e. social gatherings, group studying, and in-person classes and meetings) to return home mid-semester. As reported by our survey, this triggered a decline in mental health measured by self-reported increased anxiety, depression, and loneliness. Consistent with these results, reports indicate that people of student status and ages 18–24 are at higher risk of anxiety and depression [ 12 ]. The stress associated with this abrupt social change, the disruptive factors that may exist at one’s home, and the fear of potentially contracting or spreading COVID-19 likely contributed to the development of trauma or stress-related disorders [ 13 ]. This accounts for the worsening mental health of domestic U.S. college students as shown by this survey, with significantly greater prevalence of symptoms among underclassmen that decreases with students closer to undergraduate completion. In line with the impacts we will discuss, this is likely due to the challenge of transitioning to college amidst a pandemic, the limited opportunities to form solid friendships, as well as a lack of human contact and in-person support.

Based on our survey’s results, the mental health status of the respondents has been most affected by their worries about the health of their loved ones amidst the COVID-19 pandemic. On top of that, many respondents feel uneasy about their plans for their continuing education, anxious about their lack of productivity at home, concerned about their finances and losing job and internship prospects, and worried about contracting the virus. These factors likely made it harder for students to complete the Spring 2020 semester at home, as many respondents have indicated. Indeed, most respondents indicated that their concern about the ongoing pandemic largely began when college campuses began to shut down or when states issued lockdown guidelines shortly after. These events evidently served as a turning point in the mental health of the respondents. As the pandemic ensued, the respondents reported that their lives were disrupted in significant ways. For instance, many reported to have lost an internship or job offer. Others reported the need to take a gap year. In addition, many respondents indicated that they now have the added responsibility of supporting their family in some way. Of the respondents who claimed that the COVID-19 pandemic strained family relationships, most admitted that it has been harder to work from home than at school. These results are summarized in Fig.  4 .

Ways in which COVID-19 has affected the mental health of college students

More than one third of first-generation students experienced increased anxiety and a quarter found it harder to complete the semester at home. Accordingly, significantly more first-generation students reported taking a gap year or time off from school. First generation students were more likely able to take advantage of opportunities due to the pandemic, indicating access to support systems that may have allowed these students to take time off from school in the first place. Men and low-income students were also more likely to benefit from opportunities due to the pandemic while women, non first-generation, and non low-income students were more likely to report no impacts. However, there were no significant differences across these three demographic statuses, indicating that the main differences were solely in whether or not they gained beneficial opportunities due to the pandemic. Taken together, it appears that first generation and low-income students likely sought out more support during the pandemic, which has helped them overcome pre-pandemic anxiety regarding communicating with campus faculty and staff and utilizing support services [ 14 ]. In regards to male students who benefitted from these opportunities, they likely took time off from school as the number of men who have enrolled in college this Fall 2020 declined by seven times compared to the number of women enrolled [ 15 ], potentially due to the increased mental health issues experienced by all. The key difference is that men are less likely to seek help for mental health difficulties compared to women even pre-pandemic [ 16 ].

On the other hand, there were mixed experiences (albeit statistically insignificant) for LGBTQIA+ students with regard to anxiety, with a quarter reported increased levels and a similar number reporting decreased levels of anxiety. There were similarly mixed experiences in completing the semester from home. This may be due to LGBTQIA+ students accessing necessary resources as sixty-two percent of U.S. colleges and universities have LGBTQIA+ support groups [ 17 ] that likely serve as a valuable resource for these students throughout the pandemic.

Presumably due to increased stress factors at home and distance barriers, most respondents indicated that the COVID-19 pandemic strained relationships with their friends. While communications with friends declined, most respondents reported to be spending most of their free time watching TV because of widespread state and local lockdown orders. According to a health survey in England from 2012, watching TV for two hours or more on a daily basis is associated with mental health disorders, measured by a poor performance on the General Health Questionnaire and Warwick-Edinburgh Mental Well-being Scale [ 18 ]. On the other hand, nearly 40% of respondents indicated that they spent time exercising. Physical exercise has been proven to alleviate anxiety and depression, while increasing one’s mood and cognitive function [ 19 ]. Roughly half of the respondents claimed that they used exercise to take care of their mental health. Of those people, most reported feeling better. However, over 30% of respondents admitted to not specifically taking care of their mental health. Most people reported that they gained weight, particularly those who reported increased loneliness. The isolation and lack of contact may have led affected students to seek comfort food or decrease their daily activity levels, whether it may be training on a sports team or walking to classes and other commitments.

Similar to our findings that the COVID-19 pandemic has led to an overall reduction in the mental health of US college students, a study in China reported that about 40% of female adolescents have suffered from depression. The study cited distance learning, concern about the pandemic, and a decrease in physical exercise and sleep as independent factors causing this increase in depression [ 20 ]. Undoubtedly, the pandemic has caused many to experience feelings of loneliness and isolation. Furthermore, it has caused many individuals’ pre-existing mental health conditions to deteriorate without access to the appropriate services [ 21 ].

These services are offered on most college campuses but are more difficult to access in the world of online learning. According to a study in the New York City metropolitan area, only about half of the surveyed colleges offered information about remote counseling on their website and less than 60% of college counseling websites even offered directions for students experiencing a mental health emergency [ 22 ]. Furthermore, many news outlets report that out-of-state students have been unable to receive mental health services from their schools due to state-dependent licensures—some of which require psychologists to apply for a temporary license or receive special permission to practice [ 23 ]. Although some policies have been relaxed, students still face geographic, time, and financial barriers to access these necessary mental health services [ 23 , 24 , 25 ]. Additionally, the flexibility extended for telemedicine providers due to the pandemic being a public health emergency began to expire in Fall 2020, and navigating these policies is time-consuming and confusing for healthcare providers [ 25 ]. All of these obstacles have contributed to out-of-state students losing their mental health support completely in the face of the clear decline in mental health we have found, as supported by recent similar studies [ 26 , 27 , 28 , 29 ].

To combat the mental health challenges experienced by many US college students amidst the COVID-19 pandemic, colleges and universities across the country could take steps to expand access to virtual mental health resources and professional guidance. Some institutions have taken to a 24/7 crisis support line as well as virtual resources and emergency funding provided by the CARES Act distributed through the Higher Education Emergency Relief Fund [ 30 ]. Some universities were able to refund costs from room and board, which likely contributed to easing financial distress [ 31 ]. At the federal level, the PSYPACT agreement was passed—allowing interstate practice within the fourteen states it has been enacted in [ 32 ]ーand the TREAT Act that would allow healthcare professionals to render services anywhere during the COVID-19 pandemic was proposed but has yet to be implemented [ 33 ].

Limitations

The survey results are limited by the fact that only students who are registered with Pollfish® as publishers have the opportunity to participate in this survey. Potentially due to the monetary incentive given by Pollfish® to the respondents of their surveys, first generation and low income students are slightly overrepresented. In this survey, 61.0% of students are first generation, 35.5% being low income which is higher than the national average of 56.0% first generation students [ 34 ]. However, a study by Haenz et al. at the University of California, Los Angeles reports that roughly 50.0% of first generation students are low-income [ 35 ], which may be accounted for by the additional 10.5% of students who preferred not to reveal their income status in our study. Additionally, there are more students who identify as a member of the LGBTQIA+ community (36.5%) compared to the national average of 18.2% [ 36 ]. Our slightly disproportionate sample indicates that our findings may not be fully representative of all U.S. college students.

Future avenues of investigation include looking into specific experiences of these groups as well as other factors, such as the experiences of international students, of associate degree program students, community college students, or students completing online degrees and areas of improvement for the specific support systems in place at these educational institutions for these students since the pandemic.

The outbreak of COVID-19 has taken a universal toll on almost all aspects of life. As cases rapidly increased with great incidence, dense areas and indoor public spaces were closed and physical distancing as well as other preventative measures were enforced. These safety measures led to abrupt closures of schools and educational institutions, and a rushed transition to remote learning. Many reported worries for the health of loved ones most impacted their mental health, and the need to take care of family most affected current and future plans. Given that most students’ concerns were heightened when college campuses sent students home as well as the detrimental effects of the COVID-19 pandemic on students’ mental health, it is crucial for colleges as well as federal regulations to provide the appropriate accommodations and access to mental health care to ensure well-being and safety are prioritized as much asーif not more thanーeducation.

Mental Health in Schools Essay Example

Schools overlook their students' mental health, students that struggle with mental health sometimes struggle with being able to learn and focus in school. With the lack of education about mental health students could think that they are not normal and struggle with being confident in themselves; statistics and studies show that 18.8% of teenagers in public schools all across America have attempted or seriously thought about suicide. Public high schools are not properly equipped with the resources and staff to help students with their mental health and preventing suicide. 

One of America's only resources for mental health in schools is the guidance counselors and there are not enough counselors compared to the amount of students. For instance, a public highschool with 3000 students and 3 guidance counselors, each counselor would be giving 1000 students resources. On top of that, they have to be able to notice the changes in their students' actions and behaviors. “In the worst case scenario, a student on the brink of suicide or another mental health disorder can be denied a counselling session because their counselor was too busy with other students” (O'Connor). The lack of counselors in the school system is one of the main reasons students have no safe person or space to talk about their feelings. “Counselors are very grateful for their jobs and work very hard, but sometimes it feels like a band-aid on an arterial wound,”says one source (JONES). The amount of school shootings that occur around the United States severely affects students' mental health, and when they have no support system at home the only place they know to go is their school.  When schools do not have enough counselors it could potentially lead to more risk for suicide, feeling worthless and alone.  For example, in an impoverished highschool with only one counselor all of these students would only be able to see that counselor maybe once a month. Counselors also have very many other tasks they do everyday, like making schedules, graduation support, etc. Adding a school therapist or mental health resource room for students could potentially help this problem. Students need a safe space or person that they can go to when they need support. Teachers all across the country need to have the proper resources and training to be able to notice changes in students behavior and empathy. Additionally more counselors could help with the awareness of the problem. 

Furthermore, students should be able to have access to mental health days to take a mental break or go to therapy. Education systems in America frown upon this as it's considered “not necessary” with our 2020 pandemic occurring staff believing that these students have had enough days off and are falling behind with their graduation path. “A 2020 report from the Centers for Disease Control and Prevention found that the proportion of mental health-related emergency department visits for children aged 5–11 and 12–17 increased by about 24% and 31%, respectively, compared to 2019 (Blackmer and Shenker)”. Along with the pandemic teenagers had to stay home and quarantine away from their friends and couldn’t travel, with this occurring in early 2020 and is still currently happening, students are still struggling with feeling alone and having no support system at home and at school. When your mind is constantly going it could lead to exhaustion which could potentially alter your thinking skills and make you overall more depressed and could induce self harm. Allowing mental health days in all middle/high schools  could help prevent the body and mind from self harm with the stress and exhaustion that occurs from school. Your mind and body need a break from staying up until 3am working on homework or a big assignment due the next school day. When your mind and body gets exhausted you tend to have physical pain as well. “Some public schools have proposed that high schoolers should be allowed to take up to five mental health days off per semester to improve academic performance.” (“Should High Schoolers Have Mental Health Days in School? - Solstice RTC”). Allowing mental health days could show significant improvement in motivation and the students overall health. When students do take their allowed mental health days they could come back to school with more motivation and overall become a better student because they aren’t so stressed out about their big exam anymore and it gives them an opportunity to take a break. 

Teaching mental health in schools could bring awareness to other students and staff about the concerns and struggles that their classmates/students are battling with. Allowing mental health lessons to be taught in schools allows teachers to be able to recognize the signs of depression and anxiety. Without being taught the realities of mental health difficulties, children won’t understand the impact that ‘banter’ (whether it be insults, bullying or spreading of lies and rumors) could have. The only way this will change is if we educate them, just like physical health is taught; (Hughes). Schools all around America have health classes to teach signs of drug use, eating healthy, and staying fit but no mental health. Students could struggle majorly understanding why their thoughts and why they feel different from their friends and family and having no realization that they are struggling with mental health because they have never been taught about it. Students with undiagnosed anxiety tend to get extremely stressed out and overwhelmed on exams. With that being said the stress from the exam on top of everything else going on in their life could cause them to fail the exam as well as not understand why they failed after studying for hours. Furthermore, “Nervousness and anxiety are perfectly normal reactions to stress. For some people, this fear can become so intense that it interferes with their ability to perform well on a test” (“The Causes of Test Anxiety and Academic Stress”).

Some parents and faculty could argue that students use their mental health as an excuse for being lazy. Students that struggle with mental health severely have serious difficulties with learning and being able to manage their anxiety. When teachers say that their students are using their mental health as an excuse, it could lead to more anxiety when asking for an extension on a paper or an exam. Parents and teachers that don't struggle with mental health dismiss when their student/child comes to them with a concern. Lack of education on the topic can lead to this, not allowing mental health days as an excuse could be more harmful than letting your child struggle and not understand why in class.  “There's the danger that we take too much care and when they hit the real world that same kind of support isn't there," says David Cozzens, dean of students and associate vice president of student affairs at the University of Wyoming in Laramie (Petersen). When teachers say it's an excuse just to miss school and not do their work it can end up causing more anxiety on the student and make them not wanna come at all and potentially drop out of school completely. 

Overall, mental health is one of the biggest issues in public high schools today and it is extremely overlooked by teachers and not taking exams seriously. Teachers can show their support in many different ways to show students that it is a real thing. Mental health days could improve the struggle tremendously and teachers and guidance counselors should have more education on the topic all around America.

Mental health means keeping our minds healthy. Mankind generally is more focused on keeping their physical body healthy. People tend to ignore the state of their minds. Human superiority over other animals lies in his superior mind. Man has been able to control life due to his highly developed brain. So, it becomes very important for a man to keep both his body and mind fit and healthy. Both physical and mental health are equally important for better performance and results.

Importance of Mental Health 

An emotionally fit and stable person always feels vibrant and truly alive and can easily manage emotionally difficult situations. To be emotionally strong, one has to be physically fit too. Although mental health is a personal issue, what affects one person may or may not affect another; yet, several key elements lead to mental health issues.

Many emotional factors have a significant effect on our fitness level like depression, aggression, negative thinking, frustration, and fear, etc. A physically fit person is always in a good mood and can easily cope up with situations of distress and depression resulting in regular training contributing to a good physical fitness standard. 

Mental fitness implies a state of psychological well-being. It denotes having a positive sense of how we feel, think, and act, which improves one’s ability to enjoy life. It contributes to one’s inner ability to be self-determined. It is a proactive, positive term and forsakes negative thoughts that may come to mind. The way we physically fall sick, we can also fall sick mentally. Mental illness is the instability of one’s health, which includes changes in emotion, thinking, and behaviour. It could also arise due to genetic factors, biochemical imbalances, child abuse or trauma, social disadvantage, poor physical health condition, etc. Mental illness is curable. Regular fitness exercises like morning walks, yoga, and meditation have proved to be great medicine for curing mental health. Besides this, it is imperative to have a good diet and enough sleep. A person needs 7 to 9 hours of sleep every night on average. When someone is tired yet still can't sleep, it's a symptom that their mental health is unstable. Overworking oneself can sometimes result in not just physical tiredness but also significant mental exhaustion. As a result, people get insomnia (the inability to fall asleep). Anxiety is another indicator. 

There are many symptoms of mental health issues that differ from person to person and among the different kinds of issues as well. For instance, panic attacks and racing thoughts are common side effects. As a result of this mental strain, a person may experience chest aches and breathing difficulties. Another sign of poor mental health is a lack of focus. It occurs when you have too much going on in your life at once, and you begin to make thoughtless mistakes, resulting in a loss of capacity to focus effectively. Another element is being on edge all of the time.

It's noticeable when you're quickly irritated by minor events or statements, become offended, and argue with your family, friends, or co-workers. It occurs as a result of a build-up of internal irritation. A sense of alienation from your loved ones might have a negative influence on your mental health. It makes you feel lonely and might even put you in a state of despair. You can prevent mental illness by taking care of yourself like calming your mind by listening to soft music, being more social, setting realistic goals for yourself, and taking care of your body. 

Surround yourself with individuals who understand your circumstances and respect you as the unique individual that you are. This practice will assist you in dealing with the sickness successfully.  Improve your mental health knowledge to receive the help you need to deal with the problem. To gain emotional support, connect with other people, family, and friends.  Always remember to be grateful in life.  Pursue a hobby or any other creative activity that you enjoy.

Many health experts have stated that mental, social, and emotional health is an important part of overall fitness. Physical fitness is a combination of physical, emotional, and mental fitness. Emotional fitness has been recognized as the state in which the mind is capable of staying away from negative thoughts and can focus on creative and constructive tasks. 

He should not overreact to situations. He should not get upset or disturbed by setbacks, which are parts of life. Those who do so are not emotionally fit though they may be physically strong and healthy. There are no gyms to set this right but yoga, meditation, and reading books, which tell us how to be emotionally strong, help to acquire emotional fitness. 

Stress and depression can lead to a variety of serious health problems, including suicide in extreme situations. Being mentally healthy extends your life by allowing you to experience more joy and happiness. Mental health also improves our ability to think clearly and boosts our self-esteem. We may also connect spiritually with ourselves and serve as role models for others. We'd also be able to serve people without being a mental drain on them. 

Mental sickness is becoming a growing issue in the 21st century. Not everyone receives the help that they need. Even though mental illness is common these days and can affect anyone, there is still a stigma attached to it. People are still reluctant to accept the illness of mind because of this stigma. They feel shame to acknowledge it and seek help from the doctors. It's important to remember that "mental health" and "mental sickness" are not interchangeable.

Mental health and mental illness are inextricably linked. Individuals with good mental health can develop mental illness, while those with no mental disease can have poor mental health. Mental illness does not imply that someone is insane, and it is not anything to be embarrassed by. Our society's perception of mental disease or disorder must shift. Mental health cannot be separated from physical health. They both are equally important for a person. 

Our society needs to change its perception of mental illness or disorder. People have to remove the stigma attached to this illness and educate themselves about it. Only about 20% of adolescents and children with diagnosable mental health issues receive the therapy they need. 

According to research conducted on adults, mental illness affects 19% of the adult population. Nearly one in every five children and adolescents on the globe has a mental illness. Depression, which affects 246 million people worldwide, is one of the leading causes of disability. If  mental illness is not treated at the correct time then the consequences can be grave.

One of the essential roles of school and education is to protect boys’ and girls' mental health as teenagers are at a high risk of mental health issues. It can also impair the proper growth and development of various emotional and social skills in teenagers. Many factors can cause such problems in children. Feelings of inferiority and insecurity are the two key factors that have the greatest impact. As a result, they lose their independence and confidence, which can be avoided by encouraging the children to believe in themselves at all times. 

To make people more aware of mental health, 10th October is observed as World Mental Health. The object of this day is to spread awareness about mental health issues around the world and make all efforts in the support of mental health.

The mind is one of the most powerful organs in the body, regulating the functioning of all other organs. When our minds are unstable, they affect the whole functioning of our bodies. Being both physically and emotionally fit is the key to success in all aspects of life. People should be aware of the consequences of mental illness and must give utmost importance to keeping the mind healthy like the way the physical body is kept healthy. Mental and physical health cannot be separated from each other. And only when both are balanced can we call a person perfectly healthy and well. So, it is crucial for everyone to work towards achieving a balance between mental and physical wellbeing and get the necessary help when either of them falters.

That means that the 18-25 cohort exhibits the highest rates of anxiety and depressive symptoms, and they’re also extremely vulnerable to developing parallel addictive behaviors.

“Those with social support reduce their risk of insomnia by 33%, their risk of anxiety by 47% and their risk of depression by 50%. That’s dramatic.”

But the data is only one side of the story, and Rosenthal prefaced her talk by noting, “I do want you to know we’re going to get to a positive note of the solutions by the end.” And it may not surprise you to learn that community-building — strengthening our real-world bonds to others — plays a crucial role in improving our individual mental health. (Put down that phone, while you’re at it.)

A Career Built on Studying Behavioral Health

Rosenthal has built an impressive career on her research into the multi-factor forces driving mental-health trends in young adults.

In addition to teaching and serving as the director of JWU’s Center for Student Research and Interdisciplinary Collaboration, she serves as an evaluator for the Rhode Island Department of Health and as the lead epidemiologist for the State Epidemiological Outcomes Workgroup (SEOW). Under the aegis of SEOW, she administers the RI Young Adult Survey (RIYAS), which focuses on the behavioral health of 18-25-year-olds.

Assessing the Pandemic Spike

Depression rates among young adults have doubled in the past 10 years. But the pandemic sent these rates soaring — Rosenthal calls the spike “extreme” — due to a polycrisis of factors, including social isolation, disrupted academic studies, and unprecedented loss. During the 2020-21 academic year, noted Rosenthal, 60% of college-age students reported at least one mental health disorder, and 75% experienced psychological distress. 1 in 5 young adults in Rhode Island reported losing a loved one to Covid-19.

Once you start taking into account the social upheaval of recent years — from gun violence to racial discrimination, climate change anxiety and political turmoil — and you have a massive amount of collective trauma having a ripple effect on already fragile mental health states.

Rosenthal provided a top-level overview of two recent studies. The Healthy Mind study surveyed more than 76,000 students from 400 institutions during the 2022-23 academic year. In October 2022, Rosenthal and her co-researchers used an NIH grant to survey 586 JWU students assessing their levels of depression, anxiety, insomnia and social support.

While the Healthy Mind study clocked depression rates at 41%, the JWU case study reported 53% — that’s “higher than what we saw in the national study, but actually comparable to what we saw in the Rhode Island state study,” explained Rosenthal. “And also we have a huge representation of sexual gender minority students, which is likely to be driving some of these higher rates for us as well.” (The JWU study demographics broke down as 15.1% cisgender male, 47.4% cisgender female and 37.5% sexual or gender minority.)

So, what are the solutions for combatting these numbers? For Rosenthal, who has done multiple studies correlating social media use and depression, building up social support networks is “critically important”: “Those with social support reduce their risk of insomnia by 33%, their risk of anxiety by 47% and their risk of depression by 50%. That’s dramatic.”

“Gratitude is really powerful. I often tell my students to break up anxiety with gratitude.”

Mindfulness, Quiet Hours & Other Mental Health Boosts

Rosenthal opened the conversation to students and faculty in the audience to share their ideas for boosting our community connections and strengthening overall well-being.

Sarah and Jasmine, two undergraduate students in the Public Health program , shared how much JWU’s 3-credit Mindfulness for Health & Wellbeing class helped them. (This course is currently an elective, but there is talk of making it requirement.)

The course provides students with the opportunity to learn the principles of mindfulness, develop their own mindfulness mediation practice, and apply principles of mindfulness to daily life.

Prior to taking the class, Jasmine had been struggling with time management. Having never meditated before, it took her some time to acclimate to the practice. But now, she says, “I feel like this class has definitely not only forced me to try new things, but I’ve made it a part of my daily routine. It’s definitely helped me!”

Initially, Sarah noted that she “had a really hard time sitting down with myself and going through [the process].” Gradually, with the guidance of Professor Jennifer Swanberg, “I've been able to develop my own practice that I find beneficial. There are those times where I still feel frustration, but now I feel more prepared to deal with it.”

In the Occupational Therapy department, faculty celebrate Grati-Tuesdays, where they keep a running list of what they are thankful for. “Gratitude is really powerful,” noted Assistant Professor Kathryn Burke. “I often tell my students to break up anxiety with gratitude. Thinking about something that you’re grateful for can sometimes help get your brain out of that downward spiral that everything is terrible.”

Other suggestions included:

  • A peer mentorship program to help increase social support
  • Moving 11:59pm assignment deadlines to 8pm
  • Minimizing the number of early morning classes
  • Enforcing quiet hours in residence halls
  • Expanding the number of safe community spaces (like the Bridge for Diversity, Equity and Social Justice ) where students can share with their peers

“Creating a sense of belonging is crucially important,” concluded Rosenthal. “We need to have safe spaces to hold people when the rest of the world feels unsafe.”

JWU has a wealth of confidential mental health resources, including counseling services in Providence and Charlotte . Individual counseling sessions and consultations are available by appointment, as well as resources for mental health screenings, education and crisis intervention.
