Here, your editor can't upset the delicate balance of not intervening too much or too little. It's tricky, but a great way to think about it is to remember: editing is about asking questions, not giving answers .

Revision questions should point out:

Bad editing is usually very heavy-handed editing. Instead of helping you find your best voice and ideas, a bad editor changes your writing into their own vision.

You may be dealing with a bad editor if they:

  • Add material (examples, descriptions) that doesn't come from you
  • Use a thesaurus to make your college essay sound "more mature"
  • Add meaning or insight to the essay that doesn't come from you
  • Tell you what to say and how to say it
  • Write sentences, phrases, and paragraphs for you
  • Change your voice in the essay so it no longer sounds like it was written by a teenager

Colleges can tell the difference between a 17-year-old's writing and a 50-year-old's writing. Not only that, they have access to your SAT or ACT Writing section, so they can compare your essay to something else you wrote. Writing that's a little more polished is great and expected. But a totally different voice and style will raise questions.

Where's the Line Between Helpful Editing and Unethical Over-Editing?

Sometimes it's hard to tell whether your college essay editor is doing the right thing. Here are some guidelines for staying on the ethical side of the line.

  • An editor should say that the opening paragraph is kind of boring, and explain what exactly is making it drag. But it's overstepping for an editor to tell you exactly how to change it.
  • An editor should point out where your prose is unclear or vague. But it's completely inappropriate for the editor to rewrite that section of your essay.
  • An editor should let you know that a section is light on detail or description. But giving you similes and metaphors to beef up that description is a no-go.

Proofreading (also called copy-editing) is checking for errors in the last draft of a written work. It happens at the end of the process and is meant as the final polishing touch. Proofreading is meticulous and detail-oriented, focusing on small corrections. It sands off all the surface rough spots that could alienate the reader.

Because proofreading is usually concerned with making fixes on the word or sentence level, this is the only process where someone else can actually add to or take away things from your essay . This is because what they are adding or taking away tends to be one or two misplaced letters.

Laser focus. Proofreading is all about the tiny details, so the ability to really concentrate on finding small slip-ups is a must.

Excellent grammar and spelling skills. Proofreaders need to dot every "i" and cross every "t." Good proofreaders should correct spelling, punctuation, capitalization, and grammar. They should put foreign words in italics and surround quotations with quotation marks. They should check that you used the correct college's name, and that you adhered to any formatting requirements (name and date at the top of the page, uniform font and size, uniform spacing).

Limited interference. A proofreader needs to make sure that you followed any word limits. But if cuts need to be made to shorten the essay, that's your job and not the proofreader's.

Now let's talk about some potential people to approach for your college essay editing and proofreading needs. It's best to start close to home and slowly expand outward. Not only are your family and friends more invested in your success than strangers, but they also have a better handle on your interests and personality. This knowledge is key for judging whether your essay is expressing your true self.

Parents or Close Relatives

Your family may be full of potentially excellent editors! Parents are deeply committed to your well-being, and family members know you and your life well enough to offer details or incidents that can be included in your essay. On the other hand, the rewriting process necessarily involves criticism, which is sometimes hard to hear from someone very close to you.

A parent or close family member is a great choice for an editor if you can answer "yes" to the following questions. Is your parent or close relative a good writer or reader? Do you have a relationship where editing your essay won't create conflict? Are you able to constructively listen to criticism and suggestion from the parent?

One suggestion for defusing face-to-face discussions is to try working on the essay over email. Send your parent a draft, have them write you back some comments, and then you can pick which of their suggestions you want to use and which to discard.

Teachers or Tutors

Moreover, any teacher or tutor that has been working with you for some time, knows you very well and can vet the essay to make sure it "sounds like you."

If your teacher or tutor has some experience with what college essays are supposed to be like, ask them to be your editor. If not, then ask whether they have time to proofread your final draft.

Guidance or College Counselor at Your School

The best thing about asking your counselor to edit your work is that this is their job. This means that they have a very good sense of what colleges are looking for in an application essay.

At the same time, school counselors tend to have relationships with admissions officers in many colleges, which again gives them insight into what works and which college is focused on what aspect of the application.

Unfortunately, in many schools the guidance counselor tends to be way overextended. If your ratio is 300 students to 1 college counselor, you're unlikely to get that person's undivided attention and focus. It is still useful to ask them for general advice about your potential topics, but don't expect them to be able to stay with your essay from first draft to final version.

Friends, Siblings, or Classmates

Although they most likely don't have much experience with what colleges are hoping to see, your peers are excellent sources for checking that your essay is you .

Friends and siblings are perfect for the read-aloud edit. Read your essay to them so they can listen for words and phrases that are stilted, pompous, or phrases that just don't sound like you.

You can even trade essays and give helpful advice on each other's work.

If your editor hasn't worked with college admissions essays very much, no worries! Any astute and attentive reader can still greatly help with your process. But, as in all things, beginners do better with some preparation.

Then, as they read your essay, they can work through the following series of questions that will help them to guide you.

Introduction Questions

  • Is the first sentence a killer opening line? Why or why not?
  • Does the introduction hook the reader? Does it have a colorful, detailed, and interesting narrative? Or does it propose a compelling or surprising idea?
  • Can you feel the author's voice in the introduction, or is the tone dry, dull, or overly formal? Show the places where the voice comes through.

Essay Body Questions

  • Does the essay have a through-line? Is it built around a central argument, thought, idea, or focus? Can you put this idea into your own words?
  • How is the essay organized? By logical progression? Chronologically? Do you feel order when you read it, or are there moments where you are confused or lose the thread of the essay?
  • Does the essay have both narratives about the author's life and explanations and insight into what these stories reveal about the author's character, personality, goals, or dreams? If not, which is missing?
  • Does the essay flow? Are there smooth transitions/clever links between paragraphs? Between the narrative and moments of insight?

Reader Response Questions

  • Does the writer's personality come through? Do we know what the speaker cares about? Do we get a sense of "who he or she is"?
  • Where did you feel most connected to the essay? Which parts of the essay gave you a "you are there" sensation by invoking your senses? What moments could you picture in your head well?
  • Where are the details and examples vague and not specific enough?
  • Did you get an "a-ha!" feeling anywhere in the essay? Is there a moment of insight that connected all the dots for you? Is there a good reveal or "twist" anywhere in the essay?
  • What are the strengths of this essay? What needs the most improvement?

Should You Pay Money for Essay Editing?

One alternative to asking someone you know to help you with your college essay is the paid editor route. There are two different ways to pay for essay help: a private essay coach or a less personal editing service , like the many proliferating on the internet.

My advice is to think of these options as a last resort rather than your go-to first choice. I'll first go through the reasons why. Then, if you do decide to go with a paid editor, I'll help you decide between a coach and a service.

When to Consider a Paid Editor

In general, I think hiring someone to work on your essay makes a lot of sense if none of the people I discussed above are a possibility for you.

If you can't ask your parents. For example, if your parents aren't good writers, or if English isn't their first language. Or if you think getting your parents to help is going create unnecessary extra conflict in your relationship with them (applying to college is stressful as it is!)

If you can't ask your teacher or tutor. Maybe you don't have a trusted teacher or tutor that has time to look over your essay with focus. Or, for instance, your favorite humanities teacher has very limited experience with college essays and so won't know what admissions officers want to see.

If you can't ask your guidance counselor. This could be because your guidance counselor is way overwhelmed with other students.

If you can't share your essay with those who know you. It might be that your essay is on a very personal topic that you're unwilling to share with parents, teachers, or peers. Just make sure it doesn't fall into one of the bad-idea topics in our article on bad college essays .

If the cost isn't a consideration. Many of these services are quite expensive, and private coaches even more so. If you have finite resources, I'd say that hiring an SAT or ACT tutor (whether it's PrepScholar or someone else) is better way to spend your money . This is because there's no guarantee that a slightly better essay will sufficiently elevate the rest of your application, but a significantly higher SAT score will definitely raise your applicant profile much more.

Should You Hire an Essay Coach?

On the plus side, essay coaches have read dozens or even hundreds of college essays, so they have experience with the format. Also, because you'll be working closely with a specific person, it's more personal than sending your essay to a service, which will know even less about you.

But, on the minus side, you'll still be bouncing ideas off of someone who doesn't know that much about you . In general, if you can adequately get the help from someone you know, there is no advantage to paying someone to help you.

If you do decide to hire a coach, ask your school counselor, or older students that have used the service for recommendations. If you can't afford the coach's fees, ask whether they can work on a sliding scale —many do. And finally, beware those who guarantee admission to your school of choice—essay coaches don't have any special magic that can back up those promises.

Should You Send Your Essay to a Service?

On the plus side, essay editing services provide a similar product to essay coaches, and they cost significantly less . If you have some assurance that you'll be working with a good editor, the lack of face-to-face interaction won't prevent great results.

On the minus side, however, it can be difficult to gauge the quality of the service before working with them . If they are churning through many application essays without getting to know the students they are helping, you could end up with an over-edited essay that sounds just like everyone else's. In the worst case scenario, an unscrupulous service could send you back a plagiarized essay.

Getting recommendations from friends or a school counselor for reputable services is key to avoiding heavy-handed editing that writes essays for you or does too much to change your essay. Including a badly-edited essay like this in your application could cause problems if there are inconsistencies. For example, in interviews it might be clear you didn't write the essay, or the skill of the essay might not be reflected in your schoolwork and test scores.

Should You Buy an Essay Written by Someone Else?

Let me elaborate. There are super sketchy places on the internet where you can simply buy a pre-written essay. Don't do this!

For one thing, you'll be lying on an official, signed document. All college applications make you sign a statement saying something like this:

I certify that all information submitted in the admission process—including the application, the personal essay, any supplements, and any other supporting materials—is my own work, factually true, and honestly presented... I understand that I may be subject to a range of possible disciplinary actions, including admission revocation, expulsion, or revocation of course credit, grades, and degree, should the information I have certified be false. (From the Common Application )

For another thing, if your academic record doesn't match the essay's quality, the admissions officer will start thinking your whole application is riddled with lies.

Admission officers have full access to your writing portion of the SAT or ACT so that they can compare work that was done in proctored conditions with that done at home. They can tell if these were written by different people. Not only that, but there are now a number of search engines that faculty and admission officers can use to see if an essay contains strings of words that have appeared in other essays—you have no guarantee that the essay you bought wasn't also bought by 50 other students.

  • You should get college essay help with both editing and proofreading
  • A good editor will ask questions about your idea, logic, and structure, and will point out places where clarity is needed
  • A good editor will absolutely not answer these questions, give you their own ideas, or write the essay or parts of the essay for you
  • A good proofreader will find typos and check your formatting
  • All of them agree that getting light editing and proofreading is necessary
  • Parents, teachers, guidance or college counselor, and peers or siblings
  • If you can't ask any of those, you can pay for college essay help, but watch out for services or coaches who over-edit you work
  • Don't buy a pre-written essay! Colleges can tell, and it'll make your whole application sound false.

Helping others can help you feel better during the pandemic, a new study suggests that people who volunteer or support others during the pandemic tend to be happier..

In many ways, the coronavirus pandemic has demonstrated the extent to which we rely on others. This year, we’ve been forced to find new ways to stay connected, whether that’s signing up for virtual volunteering , organizing Zoom happy hours, or using resources like Nextdoor’s Help Map to obtain essential supplies.

How are these new ways of connecting impacting our well-being during the pandemic? According to a new research paper published in The Gerontologist , all the help that we’re giving and receiving may be serving to brighten our days and keep our relationships strong.

As shelter-in-place orders were issued in March, a team of researchers began asking participants to complete surveys each night for a week. In total, over 1,000 participants in the United States and Canada responded in the spring and summer. In the surveys, participants were asked if they had helped anyone that day—either as part of an organized volunteer activity or by providing help more informally (for example, by offering emotional support to a friend or bringing a neighbor groceries). In addition, participants also reported on their positive and negative emotions, indicated whether they had received support from anyone that day, and rated how they felt their relationships were going.

The researchers found that participants who helped others more often—whether through formal volunteering or providing more informal types of help—reported higher positive emotions, lower negative emotions, and more satisfaction with their relationships. In addition to these differences between people, the researchers also observed people’s well-being fluctuate over time: On days when participants helped others, they felt greater positive emotions and were happier with their relationships, compared to days when they didn’t help anyone else.

Additionally, providing emotional support (that is, providing a listening ear rather than trying to fix someone’s problem) had a unique benefit: On days when participants offered this kind of support, they reported lower negative emotions.

During the study, older participants (ages 60 and up) were the most likely to participate in formal volunteering activities, and they were the most likely to receive emotional support from others. Older participants also reported the highest levels of well-being, in terms of positive and negative emotions and satisfaction with their relationships. Volunteering and staying socially connected—albeit at a distance—may play a role in helping older adults stay well during the pandemic.

In fact, receiving help seemed to be beneficial for everyone, not just older people: On days when participants received support from others, they reported higher positive emotions and more happiness with their relationships.

While this might seem intuitive, it actually differs from previous research, which has found that receiving help from others can sometimes backfire. For example, receiving support we didn’t ask for can be an unpleasant experience, since it can make us feel like our competence is being called into question. Research also suggests that feeling incompetent or powerless as a result of receiving support is linked to negative consequences, such as having more symptoms of depression.

Why didn’t receiving support have adverse consequences in the present study? Nancy Sin, assistant professor at the University of British Columbia and lead author of the study, explains that one reason may have to do with the nature of the pandemic. Since all of us are going through a huge, collective stressor, reaching out for help is, in a sense, normalized.

Additionally, people may be more likely to receive the kind of helpful, effective support that they want right now. Participants in the study were especially likely to receive emotional support , and, when we’re facing an uncontrollable, unpredictable event—like COVID-19 is—being able to vent is sometimes more effective than having someone jump in to fix whatever’s wrong. It also helps that a lot of the support happening right now is reciprocal: In a conversation with a friend, we might find ourselves taking the role of both support provider and support recipient.

How to encourage kindness in ways that also boost happiness

Sin’s advice for people who are feeling lonely or disconnected right now? Seek out opportunities to connect with others, whether through formal volunteer organizations (many of which are offering virtual or socially distanced opportunities to help) or by simply reaching out to a friend you haven’t talked to in a while.

Another way to help out is to get others connected to the digital resources they need to set up Zoom calls or do virtual volunteering. While more and more older adults are connected to the internet, not all are (and socioeconomic inequalities can exacerbate this issue). Helping to bridge this digital gap will have a meaningful impact on people’s sense of connectedness right now.

Sin also suggests that the efforts we’re making now to cultivate our social networks can have long-reaching consequences. The volunteer networks, community groups, and mutual aid organizations we’ve built up while social distancing are resources that we can carry forward, even after the pandemic. She explains, “What I hope is that, by people becoming more active in helping other people, in maybe becoming more involved in their communities, that this will build resources that people can still rely on in the future even after the pandemic is over.”

Be a nonjudgmental listener.

You can help just by letting people know that you care.

  • Write a nice email or letter and casually mention a reason why you like the recipient. Reminisce about something fun you did together, or bring up one of their best qualities.
  • Include some of the recipient’s favorite snacks or home-baked goods in a care package, combined with little things that they might enjoy. For example, you might send a new sketch pad if they’re artistic.

Say “thank you” and express appreciation.

Helping people is a sign to them to show you care. I am sure everyone has helped someone else at least once, as it is a common thing. Further on in this essay I will state various times when I have helped someone.

Like many people, I have a sibling. He is younger than me so I help him many times. I help my brother with many things such as helping him with his homework, keeping him entertained when he is bored and ensuring of his safety. But sometimes there are significant events where he really needs my help such as when he gets bullied or badly injured and there is no one else around who can help him. Something like this happened at school. At lunch while I was enjoying my delicious sandwich my brother came up to me and said that his friends were running away from him. As an older sister I was alarmed and angry with those who upset my little brother. So, as most people would do, I took my brother's hand and he lead me to his friends. I told my brother's friends that this was an impolite thing to be doing and was unkind. As I was a senior in the school, my brother's friends did not argue and started to play with my brother.

Another instance when I have helped someone, was when I spotted a girl a little younger than I was who was injured and crying on the footpath. I approached her cautiously and bent down to ask her what had happened. She told me that she was going on a jog alone but she slipped and grazed her knees and hands and that now she couldn't get up. I straight away knew what to do. I asked her to calm down and not move so I could go and get my parents to assist her. Once I had gotten my parents and some bandages she told my parents what her parent's phone number was. We told the girl's parents where she was, and within five minutes they had come and picked her up.

You can help people whether you know them or not. Usually what I feel is that after you help someone you feel happy and glad about what you have just done. So go and help people in need because it will help you to feel happy too.

If someone you love is depressed, you may be experiencing any number of difficult emotions, including helplessness, frustration, anger, fear, guilt, and sadness. These feelings are all normal. It’s not easy dealing with a friend or family member’s depression. And if you neglect your own health, it can become overwhelming.

That said, your companionship and support can be crucial to your loved one’s recovery. You can help them to cope with depression symptoms, overcome negative thoughts, and regain their energy, optimism, and enjoyment of life. Start by learning all you can about depression and how to best talk about it with your friend or family member. But as you reach out, don’t forget to look after your own emotional health—you’ll need it to provide the full support your loved one needs.

Understanding depression in a friend or family member

Depression is a serious condition. Don’t underestimate the seriousness of depression. Depression drains a person’s energy, optimism, and motivation. Your depressed loved one can’t just “snap out of it” by sheer force of will.

The symptoms of depression aren’t personal. Depression makes it difficult for a person to connect on a deep emotional level with anyone, even the people they love the most. It’s also common for depressed people to say hurtful things and lash out in anger. Remember that this is the depression talking, not your loved one, so try not to take it personally.

Hiding the problem won’t make it go away.  It doesn’t help anyone involved if you try making excuses, covering up the problem, or lying for a friend or family member who is depressed. In fact, this may keep the depressed person from seeking treatment.

Your loved one isn’t lazy or unmotivated.  When you’re suffering from depression, just thinking about doing the things that may help you to feel better can seem exhausting or impossible to put into action. Have patience as you encourage your loved one to take the first small steps to recovery.

You can’t “fix” someone else’s depression. As much as you may want to, you can’t rescue someone from depression nor fix the problem for them. You’re not to blame for your loved one’s depression or responsible for their happiness (or lack thereof). While you can offer love and support, ultimately recovery is in the hands of the depressed person.

Family and friends are often the first line of defense in the fight against depression. That’s why it’s important to understand the signs and symptoms of depression . You may notice the problem in a depressed loved one before they do, and your influence and concern can motivate them to seek help.

Be concerned if your loved one: 

Doesn’t seem to care about anything anymore. Has lost interest in work, sex, hobbies, and other pleasurable activities. Has withdrawn from friends, family, and other social activities.

Expresses a bleak or negative outlook on life. Is uncharacteristically sad, irritable, short-tempered, critical, or moody; talks about feeling “helpless” or “hopeless.”

Frequently complains of aches and pains such as headaches, stomach problems, and back pain. Or complains of feeling tired and drained all the time.

Sleeps less than usual or oversleeps. Has become indecisive, forgetful, disorganized, and “out of it.”

Eats more or less than usual , and has recently gained or lost weight.

Drinks more or abuses drugs , including prescription sleeping pills and painkillers, as a way to self-medicate how they’re feeling.

Sometimes it is hard to know what to say when speaking to someone about depression. You might fear that if you bring up your worries the person will get angry, feel insulted, or ignore your concerns. You may be unsure what questions to ask or how to be supportive.

If you don’t know where to start, the following suggestions may help. But remember that being a compassionate listener is much more important than giving advice. You don’t have to try to “fix” your friend or family member; you just have to be a good listener. Often, the simple act of talking face to face can be an enormous help to someone suffering from depression. Encourage the depressed person to talk about their feelings, and be willing to listen without judgment.

Don’t expect a single conversation to be the end of it. Depressed people tend to withdraw from others and isolate themselves. You may need to express your concern and willingness to listen over and over again. Be gentle, yet persistent.

Starting the conversation

Finding a way to start a conversation about depression with your loved one is always the hardest part. You could try saying:

  • “I have been feeling concerned about you lately.”
  • “Recently, I have noticed some differences in you and wondered how you are doing.”
  • “I wanted to check in with you because you have seemed pretty down lately.”

Once you’re talking, you can ask questions such as:

  • “When did you begin feeling like this?”
  • “Did something happen that made you start feeling this way?”
  • “How can I best support you right now?”
  • “Have you thought about getting help?”

Remember, being supportive involves offering encouragement and hope. Very often, this is a matter of talking to the person in language that they will understand and can respond to while in a depressed state of mind.

What you CAN say that helps:

  • “You’re not alone. I’m here for you during this tough time.”
  • “It may be hard to believe right now, but the way you’re feeling will change.”
  • “Please tell me what I can do now to help you.”
  • “Even if I’m not able to understand exactly how you feel, I care about you and want to help.”
  • “You’re important to me. Your life is important to me.”
  • “When you want to give up, tell yourself you will hold on for just one more day, hour, or minute—whatever you can manage.”

What you should AVOID saying:

  • “This is all in your head”
  • “Everyone goes through tough times.”
  • “Try to look on the bright side.”
  • “Why do you want to die when you have so much to live for?”
  • “I can’t do anything about your situation.”
  • “Just snap out of it.”
  • “You should be feeling better by now.”

It may be hard to believe that the person you know and love would ever consider something as drastic as suicide, but a depressed person may not see any other way out. Depression clouds judgment and distorts thinking, causing a normally rational person to believe that death is the only way to end the pain they’re feeling.

Since suicide is a very real danger when someone is depressed, it’s important to know the warning signs:

  • Talking about suicide, dying, or harming oneself; a preoccupation with death
  • Expressing feelings of hopelessness or self-hate
  • Acting in dangerous or self-destructive ways
  • Getting affairs in order and saying goodbye
  • Seeking out pills, weapons, or other lethal objects
  • A sudden sense of calm after depression

If you think a friend or family member might be considering suicide, don’t wait, talk to them about your concerns. Many people feel uncomfortable bringing up the topic but it is one of the best things you can do for someone who is thinking about suicide . Talking openly about suicidal thoughts and feelings can save a person’s life, so speak up if you’re concerned and seek professional help immediately!

What to do in a crisis situation

If you believe your loved one is at an immediate risk for suicide, do NOT leave them alone.

In the U.S., dial 911 or call the 988 Suicide and Crisis Lifeline at 988.

In other countries, call your country’s emergency services number or visit IASP to find a suicide prevention helpline.

While you can’t control someone else’s recovery from depression, you can start by encouraging the depressed person to seek help. Getting a depressed person into treatment can be difficult. Depression saps energy and motivation, so even the act of making an appointment or finding a doctor can seem daunting to your loved one. Depression also involves negative ways of thinking. The depressed person may believe that the situation is hopeless and treatment pointless.

Because of these obstacles, getting your loved one to admit to the problem—and helping them see that it can be solved—is an essential step in depression recovery.

If your friend or family member resists getting help:

Suggest a general check-up with a physician. Your loved one may be less anxious about seeing a family doctor than a mental health professional. A regular doctor’s visit is actually a great option, since the doctor can rule out medical causes of depression. If the doctor diagnoses depression, they can refer your loved one to a psychiatrist or psychologist. Sometimes, this “professional” opinion makes all the difference.

Offer to help the depressed person find a doctor or therapist and go with them on the first visit. Finding the right treatment provider can be difficult, and is often a trial-and-error process. For a depressed person already low on energy, it is a huge help to have assistance making calls and looking into the options.

Encourage your loved one to make a thorough list of symptoms and ailments to discuss with the doctor. You can even bring up things that you have noticed as an outside observer, such as, “You seem to feel much worse in the mornings,” or “You always get stomach pains before work.”

Supporting your loved one’s treatment

One of the most important things you can do to help a friend or relative with depression is to give your unconditional love and support throughout the treatment process. This involves being compassionate and patient, which is not always easy when dealing with the negativity, hostility, and moodiness that go hand in hand with depression.

Provide whatever assistance the person needs (and is willing to accept). Help your loved one make and keep appointments, research treatment options, and stay on schedule with any treatment prescribed.

Have realistic expectations. It can be frustrating to watch a depressed friend or family member struggle, especially if progress is slow or stalled. Having patience is important. Even with optimal treatment, recovery from depression doesn’t happen overnight.

Lead by example. Encourage the person to lead a healthier, mood-boosting lifestyle by doing it yourself: maintain a positive outlook, eat better, avoid alcohol and drugs, exercise, and lean on others for support.

Encourage activity. Invite your loved one to join you in uplifting activities, like going to a funny movie or having dinner at a favorite restaurant. Exercise is especially helpful , so try to get your depressed loved one moving. Going on walks together is one of the easiest options. Be gently and lovingly persistent—don’t get discouraged or stop asking.

Pitch in when possible. Seemingly small tasks can be very hard for someone with depression to manage. Offer to help out with household responsibilities or chores, but only do what you can without getting burned out yourself!

There’s a natural impulse to want to fix the problems of people we care about, but you can’t control someone else’s depression. You can, however, control how well you take care of yourself. It’s just as important for you to stay healthy as it is for the depressed person to get treatment, so make your own well-being a priority.

Remember the advice of airline flight attendants: put on your own oxygen mask before you assist anyone else. In other words, make sure your own health and happiness are solid before you try to help someone who is depressed. You won’t do your friend or family member any good if you collapse under the pressure of trying to help. When your own needs are taken care of, you’ll have the energy you need to lend a helping hand.

Speak up for yourself. You may be hesitant to speak out when the depressed person in your life upsets you or lets you down. However, honest communication will actually help the relationship in the long run. If you’re suffering in silence and letting resentment build, your loved one will pick up on these negative emotions and feel even worse. Gently talk about how you’re feeling before pent-up emotions make it too hard to communicate with sensitivity.

Set boundaries. Of course you want to help, but you can only do so much. Your own health will suffer if you let your life be controlled by your loved one’s depression. You can’t be a caretaker round the clock without paying a psychological price. To avoid burnout and resentment, set clear limits on what you are willing and able to do. You are not your loved one’s therapist, so don’t take on that responsibility.

Stay on track with your own life. While some changes in your daily routine may be unavoidable while caring for your friend or relative, do your best to keep appointments and plans with friends. If your depressed loved one is unable to go on an outing or trip you had planned, ask a friend to join you instead.

Seek support. You are NOT betraying your depressed relative or friend by turning to others for support. Joining a support group, talking to a counselor or clergyman, or confiding in a trusted friend will help you get through this tough time. You don’t need to go into detail about your loved one’s depression or betray confidences; instead focus on your emotions and what you are feeling. Make sure you can be totally honest with the person you turn to—choose someone who will listen without interruption and without judging you.

I remember an incident when I was in 10th standard. I helped my needy friend who was my class fellow.

His name was Avneet Singh and belong to a poor family. He didn’t have enough money to buy books. And I used to receive a hundred rupees per month pocket money in those days. Usually, I spent that money on chocolates and  I love chocolates.

I decided to help him by giving all my pocket money to him. So, he could be able to buy books. I remember his face that how he became happy. Then, he bought books. Since then, we have been best friends. To be honest, I felt happy at that moment that feeling was extremely different which I never felt before.

I realized that some things can give more happiness than eating chocolates.

Well, it is a moral value to help someone in any difficult situation. And here I would like to talk about a time when I helped an unknown person. I describe it briefly.

About a couple of months ago, I was going to college. Actually, my college is located at some distance from my hometown. I go there via bus. Once a day, I took a bus at about 9:30 am to reach my destination. On this bus, I sit down on a seat and felt relaxed.

But suddenly, I felt something in my feet. I thought it is a paper or a small notebook of someone other. But when I stooped down to take it, I saw that it was a passport. I checked it and asked about it to all the people who were available on that bus. But everyone said no to this passport. that passport was of someone other.

I read the address on it and that address was of a village named “Malliana” which is nearby my hometown. I took it with me. Then, on the next day, I went to “Mariana” village and asked a villager about a home address which was available on that passport.

I easily found that home address. I went there and knocked on the door. A lady opened the door. I told her about the lost passport.

She felt immensely glad after listen to it from me. Because of that passport of her son. She gave respect to me and thanked me. I also felt highly elated by helping a human being.

All in all, that was the time when I helped an unknown person.

Although, I belong to an educated family consists of 5 members, my father. my mother, my two siblings and myself, We are all spending our lines with great enthusiasm.

Even though, I have remembered all the moral values given by my parents to me such as help others, respect to elders, love with the country and so on.

Generally, about two months ago, I had helped an old and feeble person. On one Sunday, I was sitting on my home’s roof in the evening. Then, I saw that an old person was wondering in my locality here and there.

Unfortunately, he was not looking well. It seemed that he was asking about an address or location.

Hence, I went outside and asked that person about the matter. He told me that he is looking for an address. At the same time, the weather was too hot because this happened during the summer season.

So, I took that old person to my house and gave water to him for drinking. He felt relax and cool. Then, I asked him about the address for which he was wandering. He told me everything.

Initially, after it, I went with him on my motorbike and dropped him at the same address which was that old person searching.

Eventually, I had helped him by dropping him at his destination. He blessed me a lot. I felt very much glad after seeing a smile on his face. I felt myself on the ninth cloud by happiness.

How to ask for and receive help, personal perspective: asking for help is hard, but it's already helping me heal..

It’s hard to ask for help, it may be even harder to need the help. But I do. I need it because tomorrow I'm having a big surgery, spinal fusion at eight levels. This surgery is both known and unknown to me. It’s my third fusion surgery, but my memories of the first two are vague. I remember they hurt a lot. The first was an emergency, so that was a terrorizing and traumatic blur. After the second, I couldn’t swallow solid foods for nearly six months; I remember that part more than anything else. Not being able to eat was awful—I really, really love to eat, especially fried foods.

Anyway, today I am striving to stay in a cautiously optimistic space. It is a wonder and a gift that this surgery is available to me. I have insurance that will pay for (most of) it and I have access to very talented doctors I come from a family of doctors to help me navigate those other doctors. If all goes well, I’ll regain the strength in my right hand and keep the strength in my left. I might even be able to hold my head up without the special backrest that I bring with me everywhere I go. I might be in less overall pain, possibly.

(I have to stop the happy thoughts there because if I think more about what my life would be like with less pain I get too excited. I’d have so much freaking energy. I could go grocery shopping without planning for recovery time. I could stand up to chit-chat at a party or wait in line at the bank. I could go dancing, maybe in high heels. I could sit in the horrible chairs to watch my kid get an award at the school assembly. Maybe, maybe, maybe)

I have thought about all of this before my previous spine surgeries, but because of my condition ( hypermobility-type Ehlers-Danlos Syndrome ), it has been my experience that fusion surgery, while keeping my spine stable, does not make my life much better. But maybe this time.

I am also trying to stay out of the dark place, where I think about pain and dependency and further disability and infection and death. But I wrote this at three o’clock in the morning; you can guess how well that’s going.

I have had so much to do but had a hard time concentrating—finish my taxes, update my advanced healthcare directive, wind down all my open work contracts, and figure out kid-care. But my mind kept wandering, and fighting the fear takes energy. In reality, I spent a lot of time staring into space.

Last week, my sister-in-law sat down with me to create a meal train website for people in my community to deliver food. So many people have offered to help. The minute they hear about the surgery they ask: What can I do? How can I help? I tend to thank them, nod and smile, then go about my business. But here I am, newly divorced , a consultant dependent on being able to work for my income, co- parenting two fabulous children. And I really, really, need the help.

These past few days I’ve been watching the sign-ups roll in on that site, and it makes my heart do funny things. At first, I was uncomfortable, and a little embarrassed. I had to walk away from my computer and stare into space. But then I reminded myself, this is love. This is my people showing up for me. I’m getting a little worried about where we will store all the food that’s coming in my tiny apartment refrigerator (note: this is a much more enjoyable worry than any of the ones listed above). I have help for driving my kid to soccer and frisbee, help to pay someone to come in and clean for a few months, and help with Jen-sitting to keep my spirits up. So much help. And love. And support.

It makes me feel held. Even now, in the darkest hour of the night, when I look at that website I don’t feel so alone. Many Patients Don’t Survive End-Stage Poverty

Dr. Ryan is an associate physician at the University of California, San Francisco, department of medicine.

He has an easy smile, blue eyes and a life-threatening bone infection in one arm. Grateful for treatment, he jokes with the medical intern each morning. A friend, a fellow doctor, is supervising the man’s care. We both work as internists at a public hospital in the medical safety net , a loose term for institutions that disproportionately serve patients on Medicaid or without insurance. You could describe the safety net in another way, too, as a place that holds up a mirror to our nation.

What is reflected can be difficult to face. It’s this: After learning that antibiotics aren’t eradicating his infection and amputation is the only chance for cure, the man withdraws, says barely a word to the intern. When she asks what he’s thinking, his reply is so tentative that she has to prompt him to repeat himself. Now with a clear voice, he tells her that if his arm must be amputated, he doesn’t want to live. She doesn’t understand what it’s like to survive on the streets, he continues. With a disability, he’ll be a target — robbed, assaulted. He’d rather die, unless, he says later, someone can find him a permanent apartment. In that case, he’ll proceed with the amputation.

The psychiatrists evaluate him. He’s not suicidal. His reasoning is logical. The social workers search for rooms, but in San Francisco far more people need long-term rehousing than the available units can accommodate. That the medical care the patient is receiving exceeds the cost of a year’s rent makes no practical difference. Eventually, the palliative care doctors see him. He transitions to hospice and dies.

A death certificate would say he died of sepsis from a bone infection, but my friend and I have a term for the illness that killed him: end-stage poverty. We needed to coin a phrase because so many of our patients die of the same thing.

Safety-net hospitals and clinics care for a population heavily skewed toward the poor, recent immigrants and people of color. The budgets of these places are forever tight . And anyone who works in them could tell you that illness in our patients isn’t just a biological phenomenon. It’s the manifestation of social inequality in people’s bodies.

Neglecting this fact can make otherwise meticulous care fail. That’s why, on one busy night, a medical student on my team is scouring websites and LinkedIn. She’s not shirking her duties. In fact, she’s one of the best students I’ve ever taught.

This week she’s caring for a retired low-wage worker with strokes and likely early dementia who was found sleeping in the street. He abandoned his rent-controlled apartment when electrolyte and kidney problems triggered a period of severe confusion that has since been resolved. Now, with little savings, he has nowhere to go. A respite center can receive patients like him when it has vacancies. The alternative is a shelter bed. He’s nearly 90 years old.

Medical textbooks usually don’t discuss fixing your patient’s housing. They seldom include making sure your patient has enough food and some way to get to a clinic. But textbooks miss what my med students don’t: that people die for lack of these basics.

People struggle to keep wounds clean. Their medications get stolen. They sicken from poor diet, undervaccination and repeated psychological trauma. Forced to focus on short-term survival and often lacking cellphones, they miss appointments for everything from Pap smears to chemotherapy. They fall ill in myriad ways — and fall through the cracks in just as many.

Early in his hospitalization, our retired patient mentions a daughter, from whom he’s been estranged for years. He doesn’t know any contact details, just her name. It’s a long shot, but we wonder if she can take him in.

The med student has one mission: find her.

I love reading about medical advances. I’m blown away that with a brain implant, a person who’s paralyzed can move a robotic arm and that surgeons recently transplanted a genetically modified pig kidney into a man on dialysis. This is the best of American innovation and cause for celebration. But breakthroughs like these won’t fix the fact that despite spending the highest percentage of its G.D.P. on health care among O.E.C.D. nations, the United States has a life expectancy years lower than comparable nations—the U.K. and Canada— and a rate of preventable death far higher .

The solution to that problem is messy, incremental, protean and inglorious. It requires massive investment in housing, addiction treatment, free and low-barrier health care and social services. It calls for just as much innovation in the social realm as in the biomedical, for acknowledgment that inequities — based on race, class, primary language and other categories — mediate how disease becomes embodied. If health care is interpreted in the truest sense of caring for people’s health, it must be a practice that extends well beyond the boundaries of hospitals and clinics.

Meanwhile, on the ground, we make do. Though the social workers are excellent and try valiantly, there are too few of them , both in my hospital and throughout a country that devalues and underfunds their profession. And so the medical student spends hours helping the family of a newly arrived Filipino immigrant navigate the health insurance system. Without her efforts, he wouldn’t get treatment for acute hepatitis C. Another patient, who is in her 20s, can’t afford rent after losing her job because of repeated hospitalizations for pancreatitis — but she can’t get the pancreatic operation she needs without a home in which to recuperate. I phone an eviction defense lawyer friend; the young woman eventually gets surgery.

Sorting out housing and insurance isn’t the best use of my skill set or that of the medical students and residents, but our efforts can be rewarding. The internet turned up the work email of the daughter of the retired man. Her house was a little cramped with his grandchildren, she said, but she would make room. The medical student came in beaming.

In these cases we succeeded; in many others we don’t. Safety-net hospitals can feel like the rapids foreshadowing a waterfall, the final common destination to which people facing inequities are swept by forces beyond their control. We try our hardest to fish them out, but sometimes we can’t do much more than toss them a life jacket or maybe a barrel and hope for the best.

I used to teach residents about the principles of internal medicine — sodium disturbances, delirium management, antibiotics. I still do, but these days I also teach about other topics — tapping community resources, thinking creatively about barriers and troubleshooting how our patients can continue to get better after leaving the supports of the hospital.

When we debrief, residents tell me how much they struggle with the moral dissonance of working in a system in which the best medicine they can provide often falls short. They’re right about how much it hurts, so I don’t know exactly what to say to them. Perhaps I never will.

The Menstrual Mood Disorder You’ve Never Heard About

E ver since she was a teenager, Tanya knew her mood swings were a problem. She had periodic bouts of anger and depression that left her wanting to die. Her life was a roller coaster of highs and lows but doctors couldn’t figure out what was wrong. Five years ago, she decided she’d had enough. She wanted to end her life.

Tanya managed to talk herself out of it and instead did some googling. She stumbled across a website about premenstrual dysphoric disorder (PMDD), a menstrual mood disorder, that impacts roughly 3 to 8% of menstruators. PMDD means having severe and debilitating mood swings right around one’s period. Around a third of people with PMDD have attempted suicide, and over  70% have regular suicidal ideation .

“It just hit me,” Tanya told me. “I went through my life and so many moments made sense.” She called her doctor the next day. He sent her to get tested for diabetes.

Tanya finally managed to get a PMDD diagnosis in 2019 at the age of 26, but she was haunted by the question: Why had she never heard of PMDD before and why was it so hard to get a diagnosis?

Although research shows suicidal ideation is linked to specific times during the menstrual cycle, today most people still haven’t heard of PMDD, including doctors. According to a 2022 survey of PMDD patients, over a third said their general practitioners had no knowledge of premenstrual disorders, and about 40% said their mental-health-care providers had no knowledge of premenstrual disorders. But PMDD has been around since periods have been around and we’ve known about it for nearly a century.

So why aren’t we getting people the help they need? The answer, as history shows, is more complicated than we think.

While the menstrual cycle has been notoriously understudied, the first papers on PMDD appeared in 1930s. In the 1950s British physician Dr. Katrina Dalton began studying PMDD —or “premenstrual syndrome” as she called it, and opened a clinic in London . Still, PMDD mostly flew under the radar until 1980, when Dalton served as a medical expert for three women accused of crime. During the trials Dalton showed that the women committed the crimes right before their period, and that their symptoms disappeared with treatment. She was able to get them a reduced sentence.

Overnight there was a media boom around PMS. The magazine Glamour even polled readers about the validity of using PMS as a legal defense: (24% were for, 71% against, and 5% unsure). As a result, the growing awareness around PMS bled over to the medical community. In 1984 psychiatrist Robert Spitzer proposed PMS should be added to the Diagnostic and Statistical Manual of Mental Disorders (DSM), the handbook used by doctors to diagnosis mental health problems. Spitzer was the chair of the committee revising the third version of the DSM, and he’d been getting several invites to conferences about PMS. Adding PMS to the DSM meant that the medical community was recognizing it as a formal disorder that could be diagnosed and treated.

However, experts were divided on where to draw the line between PMDD (the severe, debilitating mood swings) and PMS (garden variety grumpiness and irritability). When the Washington Post asked experts about the prevalence of PMS in 1984, estimates ranged from 3% to 90% of women . In a sense, both of these estimates were true: only a few people have severe symptoms, but up to 90% of people do have mild symptoms before their period. It became clear that experts were conflating PMDD and PMS and this was reflected in popular culture. Suddenly, anyone who had a period was seen as unstable. Products with sexist jokes abounded, such as greeting cards that read “What’s the difference between a woman with PMS and a terrorist? Answer: You can negotiate with a terrorist.”

Controversy erupted. A group of feminists spearheaded a campaign to stop PMDD’s inclusion in the DSM. They argued that including PMDD would be tantamount to calling menstruation a mental illness. Much of their reaction was rooted in the atmosphere of the ‘70s and ‘80s as women were fighting for gender equality on several fronts from work to reproductive rights. In the 1970s, for the first time, over half of women were working outside the home. The Supreme Court ruled to protect the right to abortion in Roe v. Wade. However, this equality was also precarious and under threat—conservatives defeated the Equal Rights Amendment which would have prohibited gender discrimination. For feminists, PMDD and the sexist jokes it generated seemed to be another attempt to discredit women and chip away at the progress feminists had made for women’s rights.

One of PMDD’s major critics, Dr. Joan Chrisler, attended a lecture in the 1970s, early on in her career on why there were no great women artists or novelists. The conclusion: women’s creativity declines when they are menstruating. This inspired Chrisler to devote her career to fighting menstrual stigma. In an era rife with sexism, adding PMDD to the DSM set off alarm bells.

To further complicate matters, in the early 2000s, drug manufacturer Eli Lilly stuck its finger in the pie. Eli Lilly’s patent on its blockbuster drug Prozac, which treated depression, was set to expire and it’s share prices had tanked. The company desperately needed a new market that would buy Prozac.

Eli Lilly landed on PMDD as their next likely market. Early studies already suggested that Prozac could treat PMDD, but the problem was that experts were still debating whether PMDD belonged in the DSM as an official diagnosis at all. Undaunted, Eli Lilly embarked on a massive campaign to market Prozac to people with PMDD. It sponsored a round table discussion on Prozac’s effectiveness for PMDD, which found that at least 60% of patients with PMDD respond to antidepressants. It repackaged Prozac as “Serafem” a play on the word “seraphim”, the angels who guard the throne of God. If that wasn’t enough to drive home the point, they changed Prozac’s green and yellow pills to pink and purple. Finally, they rolled out a series of ads that suggested any woman who was feeling grumpy should consider medication. The ads didn’t distinguish between PMDD and PMS. Rather, they imply any sign of irritability in women should be treated.

This unleashed another wave of furor. Feminists were insulted—and rightfully so. The last thing the women’s movement needed was a drug companies suggesting a woman’s anger is a mental health condition. The media had a field day writing about drug companies “inventing” new diseases and marketing drugs without diagnoses. (Tragically, Prozac actually works on PMDD. Studies have found SSRIs like Prozac can relieve PMDD symptoms in up to twelve hours, even though it takes weeks to work for anxiety and depression.)

In some ways the feminists lost their battle. PMDD eventually did become a diagnosis in the DSM in 2013, nearly three decades after it’d been first debated in the ‘80s. In other ways, they won the war. Dr. Tory Eisenlohr-Moul, a premenstrual mood disorder researcher at the University of Illinois Chicago, estimates that the backlash around PMDD set care back by 10 to 15 years. She pointed out there’s been a chilling effect on the conversation around the link between menstruation and mood. Doctors are afraid to bring it up, because they don’t want patients to think they are sexist.

Yet, in reality, the biggest victims of the fight over PMDD’s existence are people who have PMDD and the people who love them. Today, thousands of people still suffer because they don’t know they have PMDD.

Tanya’s PMDD diagnosis changed her life. While she elected not to try medication, the diagnosis alone helped her understand her breakdowns, and helped her explain what was going on to family and friends. She decided to train in psychotherapy so she can help other people understand PMDD. “My tutor has been a counselor for 10 years and still hasn’t heard of PMDD,” she said. “I want to help people work through this pain.”

The basic structure of an essay always consists of an introduction , a body , and a conclusion . But for many students, the most difficult part of structuring an essay is deciding how to organize information within the body.

There are two main things to keep in mind when working on your essay structure: making sure to include the right information in each part, and deciding how you’ll organize the information within the body.

Parts of an essay

The three parts that make up all essays are described in the table below.

Order of information

You’ll also have to consider how to present information within the body. There are a few general principles that can guide you here.

The first is that your argument should move from the simplest claim to the most complex . The body of a good argumentative essay often begins with simple and widely accepted claims, and then moves towards more complex and contentious ones.

For example, you might begin by describing a generally accepted philosophical concept, and then apply it to a new topic. The grounding in the general concept will allow the reader to understand your unique application of it.

The second principle is that background information should appear towards the beginning of your essay . General background is presented in the introduction. If you have additional background to present, this information will usually come at the start of the body.

The third principle is that everything in your essay should be relevant to the thesis . Ask yourself whether each piece of information advances your argument or provides necessary background. And make sure that the text clearly expresses each piece of information’s relevance.

The sections below present several organizational templates for essays: the chronological approach, the compare-and-contrast approach, and the problems-methods-solutions approach.

The chronological approach (sometimes called the cause-and-effect approach) is probably the simplest way to structure an essay. It just means discussing events in the order in which they occurred, discussing how they are related (i.e. the cause and effect involved) as you go.

A chronological approach can be useful when your essay is about a series of events. Don’t rule out other approaches, though—even when the chronological approach is the obvious one, you might be able to bring out more with a different structure.

Essays with two or more main subjects are often structured around comparing and contrasting . For example, a literary analysis essay might compare two different texts, and an argumentative essay might compare the strengths of different arguments.

There are two main ways of structuring a compare-and-contrast essay: the alternating method, and the block method.

Alternating

In the alternating method, each paragraph compares your subjects in terms of a specific point of comparison. These points of comparison are therefore what defines each paragraph.

In the block method, each subject is covered all in one go, potentially across multiple paragraphs. For example, you might write two paragraphs about your first subject and then two about your second subject, making comparisons back to the first.

  • Disadvantages: Students must live close enough to attend, commutes may vary, classrooms not always accessible for disabled students
  • Advantages: Ease of picking up on signs a student is struggling, more personal interaction among students
  • Disadvantages: May be harder for students to approach teacher privately in person to raise issues

An essay that concerns a specific problem (practical or theoretical) may be structured according to the problems-methods-solutions approach.

This is just what it sounds like: You define the problem, characterize a method or theory that may solve it, and finally analyze the problem, using this method or theory to arrive at a solution. If the problem is theoretical, the solution might be the analysis you present in the essay itself; otherwise, you might just present a proposed solution.

Signposting means guiding the reader through your essay with language that describes or hints at the structure of what follows.  It can help you clarify your structure for yourself as well as helping your reader follow your ideas.

The essay overview

In longer essays whose body is split into multiple named sections, the introduction often ends with an overview of the rest of the essay. This gives a brief description of the main idea or argument of each section.

The overview allows the reader to immediately understand what will be covered in the essay and in what order. Though it describes what  comes later in the text, it is generally written in the present tense . The following example is from a literary analysis essay on Mary Shelley’s Frankenstein .

Transitions

Transition words and phrases are used throughout all good essays to link together different ideas. They help guide the reader through your text, and an essay that uses them effectively will be much easier to follow.

Various different relationships can be expressed by transition words, as shown in this example.

Because Hitler failed to respond to the British ultimatum, France and the UK declared war on Germany. Although it was an outcome the Allies had hoped to avoid, they were prepared to back up their ultimatum in order to combat the existential threat posed by the Third Reich.

Transition sentences may be included to transition between different paragraphs or sections of an essay. A good transition sentence moves the reader on to the next topic while indicating how it relates to the previous one.


 Distance learning, then, seems to improve accessibility in some ways while representing a step backwards in others.

However , considering the issue of personal interaction among students presents a different picture.

The structure of an essay is divided into an introduction that presents your topic and thesis statement , a body containing your in-depth analysis and arguments, and a conclusion wrapping up your ideas.

The structure of the body is flexible, but you should always spend some time thinking about how you can organize your essay to best serve your ideas.

An essay isn’t just a loose collection of facts and ideas. Instead, it should be centered on an overarching argument (summarized in your thesis statement ) that every part of the essay relates to.

The way you structure your essay is crucial to presenting your argument coherently. A well-structured essay helps your reader follow the logic of your ideas and understand your overall point.

Comparisons in essays are generally structured in one of two ways:

  • The alternating method, where you compare your subjects side by side according to one specific aspect at a time.
  • The block method, where you cover each subject separately in its entirety.

It’s also possible to combine both methods, for example by writing a full paragraph on each of your topics and then a final paragraph contrasting the two according to a specific metric.

You should try to follow your outline as you write your essay . However, if your ideas change or it becomes clear that your structure could be better, it’s okay to depart from your essay outline . Just make sure you know why you’re doing so.

Mental health care is hard to find, especially for people with Medicare or Medicaid

With rates of suicide and opioid deaths rising in the past decade and children's mental health declared a national emergency , the United States faces an unprecedented mental health crisis. But access to mental health care for a significant portion of Americans — including some of the most vulnerable populations — is extremely limited, according to a new government report released Wednesday.

The report, from the Department of Health and Human Services' Office of Inspector General, finds that Medicare and Medicaid have a dire shortage of mental health care providers.

The report looked at 20 counties with people on Medicaid, traditional Medicare and Medicare Advantage plans, which together serve more than 130 million enrollees — more than 40% of the U.S. population, says Meridith Seife , the deputy regional inspector general and the lead author of the report.

Medicaid serves people on low incomes, and Medicare is mainly for people 65 years or older and those who are younger with chronic disabilities.

The report found fewer than five active mental health care providers for every 1,000 enrollees. On average, Medicare Advantage has 4.7 providers per 1,000 enrollees, whereas traditional Medicare has 2.9 providers and Medicaid has 3.1 providers for the same number of enrollees. Some counties fare even worse, with not even a single provider for every 1,000 enrollees.

"When you have so few providers available to see this many enrollees, patients start running into significant problems finding care," says Seife.

The findings are especially troubling given the level of need for mental health care in this population, she says.

"On Medicare, you have 1 in 4 Medicare enrollees who are living with a mental illness," she says. "Yet less than half of those people are receiving treatment."

Among people on Medicaid, 1 in 3 have a mental illness, and 1 in 5 have a substance use disorder. "So the need is tremendous."

The results are "scary" but "not very surprising," says Deborah Steinberg , senior health policy attorney at the nonprofit Legal Action Center. "We know that people in Medicare and Medicaid are often underserved populations, and this is especially true for mental health and substance use disorder care."

Among those individuals able to find and connect with a provider, many see their provider several times a year, according to the report. And many have to drive a long way for their appointments.

"We have roughly 1 in 4 patients that had to travel more than an hour to their appointments, and 1 in 10 had to travel more than an hour and a half each way," notes Seife. Some patients traveled two hours each way for mental health care, she says.

Mental illnesses and substance use disorders are chronic conditions that people need ongoing care for, says Steinberg. "And when they have to travel an hour, more than an hour, for an appointment throughout the year, that becomes unreasonable. It becomes untenable."

"We know that behavioral health workforce shortages are widespread," says Heather Saunders , a senior research manager on the Medicaid team at KFF, the health policy research organization. "This is across all payers, all populations, with about half of the U.S. population living in a workforce shortage."

But as the report found, that's not the whole story for Medicare and Medicaid. Only about a third of mental health care providers in the counties studied see Medicare and Medicaid patients. That means a majority of the workforce doesn't participate in these programs.

This has been well documented in Medicaid, notes Saunders. "Only a fraction" of providers in provider directories see Medicaid patients, she says. "And when they do see Medicaid patients, they often only see a few."

Lower reimbursement rates and a high administrative burden prevent more providers from participating in Medicaid and Medicare, the report notes.

"In the Medicare program, they set a physician fee rate," explains Steinberg. "Then for certain providers, which includes clinical social workers, mental health counselors and marriage and family therapists, they get reimbursed at 75% of that rate."

Medicaid reimbursements for psychiatric services are even lower when compared with Medicare , says Ellen Weber , senior vice president for health initiatives at the Legal Action Center.

"They're baking in those discriminatory standards when they are setting those rates," says Steinberg.

The new report recommends that the Centers for Medicare & Medicaid Services (CMS) take steps to increase payments to providers and lower administrative requirements. In a statement, CMS said it has responded to those recommendations within the report.

According to research by Saunders and her colleagues at KFF, many states have already started to take action on these fronts to improve participation in Medicaid.

Several have upped their payments to mental health providers. "But the scale of those increases ranged widely across states," says Saunders, "with some states limiting the increase to one provider type or one type of service, but other states having rate increases that were more across the board."

Some states have also tried to simplify and streamline paperwork, she adds. "Making it less complex, making it easier to understand," says Saunders.

But it's too soon to know whether those efforts have made a significant impact on improving access to providers.

CMS has also taken steps to address provider shortages, says Steinberg.

"CMS has tried to increase some of the reimbursement rates without actually fixing that structural problem," says Steinberg. "Trying to add a little bit here and there, but it's not enough, especially when they're only adding a percent to the total rate. It's a really small increase."

The agency has also started covering treatments and providers it didn't use to cover before.

"In 2020, Medicare started covering opioid treatment programs, which is where a lot of folks can go to get medications for their substance use disorder," says Steinberg.

And starting this year, Medicare also covers "mental health counselors, which includes addiction counselors, as well as marriage and family therapists," she adds.

While noteworthy and important, a lot more needs to be done, says Steinberg. "For example, in the substance use disorder space, a lot of addiction counselors do not have a master's degree. And that's one of their requirements to be a counselor in the Medicare program right now."

Removing those stringent requirements and adding other kinds of providers, like peer support specialists, is key to improving access. And the cost of not accessing care is high, she adds.

"Over the past two decades, [in] the older adult population, the number of overdose deaths has increased fourfold — quadrupled," says Steinberg. "So this is affecting people. It is causing deaths. It is causing people to go to the hospital. It increases [health care] costs."
